RE: EAP Contract Agreement Renewal
For Two -Year Term of 2015-2017
Dear Ms. Tracey Hause,
As a valued client, Community Action EAP Inc. is looking forward to working with the City of
Temple City for the next contract term. With July 1 being the renewal date, the purpose of this
communication is to renew our EAP contract agreement with the City of Temple City, specifying
that there are no changes in contract provisions and services— with the administrative monthly
fee being $300.00 per month.
In recognition that there are no contract changes from the original agreement, Community Action
EAP Inc. is seeking to renew its contract with the City of Temple City through the following
succinct agreement.
If the conditions given below are appropriate for meeting the terms of renewal, please sign the
agreement and send (via mail, email or fax) a signed copy to Community Action EAP Inc. The
original can remain with you.
CONTRACT AGREEMENT BETWEEN
THE CITY OF TEMPLE CITY
AND COMMUNITY ACTION EAP INC.
For the next contract term, starting July 1, 2015, through June 30, 2017, the City of
Temple City is renewing its EAP contract agreement with Community Action EAP Inc.
with the stipulation that all contract provisions remain the same.
