LEAD -BASED PAINT RRP RULES
Ordinary renovation and maintenance activities can create dust that contains lead. Contractors who perform
renovation, repairs, and painting jobs in pre -1978 housing and child- occupied facilities must, before beginning
work, provide owners, tenants, and child -care facilities with a copy of EPA's lead hazard information pamphlet
Renovate Right: Important Lead Hazard information for Families, Child Care Providers, and Schools.
Contractors must document compliance with this requirement. After April 22, 2010, federal law will require you
to be certified and to use lead -safe work practices. Georgia Environmental Protection Division Lead -Based
Paint and Asbestos Program (404) 363 -7026.
Projects must be done by a Certified Lead Renovation Firm AND a Certified Renovator
When disturbing lead -based painted or coated surfaces or components more than 6 ft2 per interior
room or 20 ft2 of exterior surfaces AND all window replacements and partial demolitions of any size.

Asbestos /Environmental Notification to Georgia EPD for
Projects Involving Demolition, Wrecking, or Renovation
The undersigned hereby acknowledges that the issuance of this permit does not in any way grant permission
to the owner, owner's representative, or permit holder to proceed with demolition, wrecking, or renovation of a
structure prior to the filing of any required ten (10) day "Project Notification for Asbestos Renovation
Encapsulation or Demolition" form in accordance with the Georgia Asbestos Rules. The Georgia
Environmental Protection Division administers the rules. In most cases, the rules require both the owner and
the involved contractors to assure the portion of the building involved in the project is thoroughly inspected by
an Accredited Asbestos Inspector for materials that contain asbestos; and the removal of the asbestos before
renovation, wrecking, or demolition begins almost without exemption. Georgia EPD requires a completed
demolition notification from be submitted 10 workings days in advance even if no asbestos is present in the
building. Further guidance for regulatory compliance and contact telephone numbers are provided by the
brochures entitled Asbestos & Renovation and Asbestos and Demolition. Other environmental issues such
as asbestos removal techniques, lead abatement, ground contamination, or unusual site conditions may have
EPD regulations that could affect the project.

BMPs
While BMP deficiencies are not necessarily the fault of the owner or his agent, BMPs are their responsibility.
Two areas of deficiencies are in the most basic and common BMPs; Co — Construction Exit and Sd1 —
Sediment Barrier. Correct installation information can be found in the Field Manual for Erosion and Sediment
Control in Georgia, Fourth Edition 2002, Georgia Soil and Water Conservation Commission;
http://www.gaswcc.org/docs/field_manual_4ed.pdf.
Problems with the Co is not limited to the installation, but to material. The stone will be a representation of
1.5 " -3.5" stone or larger.
Type A sediment barriers have been installed where Type C is required and shown on the permit drawings.
Where two rows are called for they will be installed with a separation that allows for the first one to fail (fall

This letter summarizes the results of HGBD's review of the Permit Submittal for the attachment of new
antennas to the existing water tank. The submittal included drawings, originally dated May 30, 2013
and revised June 11, 2013, prepared by PM &A; the Application for Building Permit, dated May 16,
2013; and the Structural Evaluation dated May 15, 2013, revised May 29, 2013 and subsequently
revised on June 10, 2013. The Drawings T -1, C -1 - C -7, S -1, E -1 - E -4 included layout, details,
specifications and coatings information.
HGBD's review was undertaken in order to assess, in general, if the design documentation for the
proposed improvements appears to be consistent with applicable codes and good professional practices.
The reviewer has not undertaken a detailed analysis of the calculations or verified the technical
sufficiency of each design element, responsibilities that remain solely with the designer-of-record. This
review was limited to the structural portions of the submittal only.
We have reviewed the submittal and understand that the Tenant, AT &T, desires to replace 3 antennas
on the existing water tank located at 111 Butler Ave. The new antennas will require new ancillary
equipment on the existing concrete pad. It is our understanding that the tank currently has numerous
antennas and other hardware from different companies at this time. Based on our review of the
calculations and drawings, we recommend that the project be approved. If the City of Tybee Island has
a maintenance agreement with a utility services company or similar company then the maintenance
company shall review the plans.

1.0 SUBJECT AND REFERENCES
The purpose of this analysis is to evaluate the structural capacity of the wireless
telecommunication installation on the existing water tank, located at 111 Butler Avenue,
Tybee Island, GA 31328, for the additions and alterations proposed by AT &T.
Structural Analysis is based on the following information provided to Destek Engineering, LLC
(Destek):
• Tower Mapping Report prepared by Tower Engineering Professionals, dated
09/19/2012.
• Structural Evaluation Report prepared by AW Solutions Inc., dated 09/24/2012.
• Structural Analysis Report prepared by AW Solutions Inc., dated 11/06/2012.
• Existing and proposed antennas information provided by AT &T.
1.1 STRUCTURE
The structure is a 151 foot tall pedisphere water tank. The water tank capacity is 150,000
Gallons as reported on the tank ID tag and the measured diameter of the tank is reported as
34.5 feet. AT &T antennas are attached to a corral structure at the top of the tank, formed
by 4' -8" tall (1)2.5 standard vertical pipes and L3x3x5/16 angles. Each pipe is braced with
L2.5x2.5x1/4 angle kickers.

3.0 CODES AND LOADING
The analysis is in accordance with:
• 2006 International Building Code
• ASCE 7 -05, Minimum Design Loads for Buildings and Other Structures
• AISC, Manual of Steel Construction, ASD 13th Edition.

4.0 STANDARD CONDITIONS FOR ENGINEERING SERVICES ON EXISTING STRUCTURES
The analysis is based on the information provided to Destek and is assumed to be current
and correct. Unless otherwise noted, the structure and the foundation system are assumed
to be in good condition, free of defects and can achieve theoretical strength.
It is assumed that the structure has been maintained and shall be maintained during its
service. The superstructure and the foundation system are assumed to be designed with
proper engineering practice and fabricated, constructed and erected in accordance with the
design documents. Destek will accept no liability which may arise due to any existing
deficiency in design, material, fabrication, erection, construction, etc. or lack of
maintenance.
The analysis results presented in this report are only applicable for the previously mentioned
existing and proposed additions and alterations. Any deviation of the proposed equipment
and placement, etc., will require Destek to generate an additional structural analysis.
5.0 ANALYSIS and ASSUMPTIONS
The structure is considered to have adequate strength for the proposed loading, if the
existing structural members which will be used to support the proposed equipment are
structurally adequate per the current code criteria or the additions or alterations to the
existing structure do not increase the force in any structural element by more than 5 %.

6.0 RESULTS AND CONCLUSION
H- Frame: The proposed cabinets should be mounted to the new H -Frame formed by 4"
diameter standard pipes welded to %" base plates anchored to the existing concrete slab
with (4) %" Hilti Kwik 11 bolts with minimum 3 1/2" embedment. Minimum anchor edge
distance should not be less than 31/2 inches. Please refer to the attached calculations.
Water Tank: Using a load comparison analysis using AW Solutions Inc.'s Finite Element
Analysis Report dated 11/06/12, the tank is found to have adequate capacity for the
proposed additions by AT &T. As a maximum, the tank shaft will be stressed to 83% of
capacity. We recommend maintaining a minimum of 20000 gallons of water at all times.
Antenna Mount Corral: The existing antenna corral on top of the water tank is found to
have adequate capacity for the proposed changes. As a maximum the corral members are
stressed at 74% of its capacity.
The existing antenna pipes will have adequate capacity for the proposed changes once they
are upgraded per Destek drawings dated 06/10/2013.
Therefore, the alterations proposed by AT &T can be implemented as intended once the
antennas pipes are replaced and with the conditions and recommendations outlined in this

AT &T will be upgrading its equipment at existing cell site 410 -013 which is the water tank located
at 111 Butler Ave. The scope of work that will be completed is as follows:
*To the tower: swapping 3 antennas. There are currently 6 total antennas. A total of 6 will remain
after 3 of the existing are swapped with 3 new antennas. Ancillary equipment will also be added.
*On the ground: ancillary equipment will be added to the existing pad.

THIS FIRST AMENDMENT TO WATER TOWER ATTACHMENT OPTION AND LEASE
AGREEMENT ( "Amendment ") dated as of the date below, by and between The City of Tybee Island, Georgia,
having a mailing address at P.O. Box 2749, Tybee Island, GA 31328 (hereinafter referred to as "Landlord ") and
New Cingular Wireless PCS, LLC, a Delaware limited liability company, successor in interest to BellSouth
Personal Communications, Inc., a Delaware corporation, d/b /a BellSouth Mobility DCS, having a mailing
address at 6100 Atlantic Boulevard, Norcross, Georgia 30071 (hereinafter referred to as "Tenant ").
WHEREAS, Landlord and Tenant entered into a Water Tower Attachment Option and Lease Agreement
( "Lease ") dated October 10, 1997; whereby Landlord leased to Tenant certain Premises, therein described, that
are a portion of the Property located at 211 Butler Avenue, Tybee Island, GA 31328 ( "Agreement "); and
WHEREAS, Landlord and Tenant desire to amend the terms of the Agreement; and
WHEREAS, Landlord and Tenant desire to modify, as set forth herein, the Rent payable under the
Agreement; and
WHEREAS, Landlord and Tenant desire to modify, as set forth herein, the Tenant's obligations to pay
Rent to Landlord for a Rent Guarantee Period; and
WHEREAS, Landlord and Tenant, in their mutual interest, wish to amend the Agreement', as set forth
below accordingly.
NOW THEREFORE, in consideration of the foregoing and other good and valuable consideration, the
receipt and sufficiency of which is hereby acknowledged, the Landlord and Tenant hereby agree as follows:
1. Term. All references to the Term of the Agreement shall be amended to provide that the
Agreement has a Term of 60 months ( "Initial Term "), commencing on January 1, 2006. The Term will be
automatically renewed for up to 5 additional 60 month terms (the "Extension Term ") without further action by
Tenant for additional periods of 60 months.
2. Termination. After the Rent Guarantee Period, Tenant may terminate the Agreement at any
time with 30 days prior written notice to Landlord; provided, that Tenant pays to Landlord an amount equal to
12 months of the then current Rent.
3. Modification of Rent. Commencing on January 1, 2006, the Rent payable under the Agreement
shall be $1,000.00 monthly, and shall continue during the Term, subject to adjustment as provided below.
Notwithstanding the foregoing, nothing herein shall be deemed to modify the additional rent derived from any
and all subleases under the provisions of paragraph 16 of the Agreement.

4. Modification of Tenant's Obligation to Pay - Rent Guarantee. Notwithstanding Tenant's
obligations to pay Rent set forth under the Agreement, for a 72 month period commencing January 1, 2006 and
ending December 31, 2011 ( "Rent Guarantee Period "), Tenant hereby agrees that Tenant will be obligated to
pay Rent due and such obligation will not be subject to offset or termination by Tenant.
5. Future Rent Increases. The Agreement is amended to provide that commencing on January 1,
2011, Rent shall be increased by 12.00% and every 5 years thereafter by 12.00% of the then current Rent.
6. Notices. Section 19 of the Agreement is hereby deleted in its entirety and replaced with the
following: NOTICES. All notices, requests, demands and communications hereunder will be given by first
class certified or registered mail, return receipt requested, or by a nationally recognized overnight courier,
postage prepaid, to be effective when properly sent and received, refused or returned undelivered. Notices will
be addressed to the parties as follows: As to Tenant, New Cingular Wireless PCS, LLC, c/o Network Real
Estate Administration, Cell Site # 10017258, Cell Site Name 410 -013, 6100 Atlantic Boulevard, Norcross,
Georgia 30071, with a copy to Cingular Wireless Attn.: Legal Department, Re: Cell Site # 10017258, Cell Site
Name 410 -013, 15 East Midland Avenue, Paramus, NJ 07652; and as to Landlord, P.O. Box 2749, Tybee
Island, GA 31328. Either party hereto may change the place for the giving of notice to it by thirty (30) days
prior written notice to the other as provided herein.
7. Acknowledgement. Landlord acknowledges that: 1) this Amendment is entered; into of the
Landlord's free will and volition; 2) Landlord has read and understands this Amendment and the underlying
Agreement and, prior to execution of the Amendment, was free to consult with counsel of its choosing regarding
Landlord's decision to enter into this Amendment and to have counsel review the terms and conditions of the
Amendment; 3) Landlord has been advised and is informed that should Landlord not enter into this Amendment,
the underlying Agreement between Landlord and Tenant, including any termination or non - renewal provision
therein, would remain in full force and effect.
8. Other Terms and Conditions Remain. The Lease is amended to incorporate all the provisions
set forth on Schedule I attached hereto. In the event of any inconsistencies between the Agreement, and this First
Amendment, and the provisions set forth on Schedule 1, the terms of this First Amendment and Schedule I shall
control. Except as expressly set forth in this First Amendment, the Agreement otherwise is unmodified and
remains in full force and effect. Each reference in the Agreement to itself shall be deemed also to refer to this
First Amendment.
9. Capitalized Terms. All capitalized terms used but not defined herein shall have the same
meanings as defined in the Agreement.

OPTION AND LEASE AGREEMENT
THIS WATER TOWER OPTION AND LEASE AGREEMENT ( "Agreement ") is
entered into this _el day of Oc f b e- , 1997 by and between THE CITY OF TYBEE
ISLAND, GEORGIA ( "Landlord "), and BELLSOUTH PERSONAL COMMUNICATIONS,
INC., a Delaware corporation, d /b /a BellSouth Mobility DCS ( "Tenant ").
PROPERTY
Landlord is the owner of certain real property (the "Real Property ") located in the City
of Tybee Island, Chatham County, State of Georgia, more particularly described in Exhibit
"A" attached hereto and made a part hereof, and Landlord is the owner of the water tower
situated thereon which is located at latitude 32° 00' 39" and longitude 80° 50' 31" (the
"Tower "). Tenant desires to obtain an option to lease a portion of the Real Property
containing an approximately 35 foot by 35 foot tract of land (the "Ground Space "), together
with certain positions on the Tower between the 125 and 135 foot levels on the Tower (or top
available mounting bracket location) (collectively, the "Tower Space ") for the mounting of
various antennas and related equipment, as such Ground Space is substantially shown on
Exhibit "A -1" attached hereto and made a part hereof (if no Exhibit "A -1" is attached, it shall
be attached in the future and shall be mutually acceptable), together with nonexclusive
easements and rights of way as are hereinafter described (the Ground Space, the Tower Space,
and such easements are hereinafter collectively called the "Property ").
OPTION
NOW THEREFORE, in consideration of the sum of Five Hundred and No/ 100 Dollars
($500.00) (the "Option Money "), to be paid by Tenant to Landlord upon Tenant's execution of
this Agreement, Landlord hereby grants to Tenant the exclusive right and option (the
"Option ") to lease the Property in accordance with the terms and conditions set forth herein.
Option Period. The Option may be exercised at any time on or prior to December 15,
1997 (the "Option Period "). At Tenant's election, and upon Tenant's written notice to
Landlord prior to expiration of the Option Period, the Option Period may be further extended
for an additional period of three (3) months, through and including March 15, 1998, with an
additional payment of Five Hundred and No /100 Dollars ($500.00) by Tenant to Landlord for
the extension of the Option Period. The Option Period may be thereafter further extended by
mutual agreement in writing. If Tenant fails to exercise the Option within the Option Period,
as it may thereafter be extended as provided herein, the Option shall terminate, all rights and
privileges granted hereunder shall be deemed completely surrendered, Landlord shall retain all
money paid for the Option, and no additional money shall be payable by either party to the
other.
