After a brief inspection of the project, it is our opinion that the flume is installed correctly and the back slopes have been sodded. We have also looked at the grassing to the south of the building and it is our opinion that the final stabilization is at 50% to 75% in place and the silt fences may be removed. This swale runs approximately from the middle of the building and to around 30' east of the southwest corner of the building. It shows well on the west end. However, from around 30' east of the SW building corner to around 50' west, it is not clearly shown. The 8.65 elevation is upstream of (higher than) the 8. contour. From that point to around 4' east the contours indicate slope drainage onto the property to the south. Tybee needs the swale clearly shown for documentation that the stormwater is being conveyed from this property to City ROW's or easements without the use of property of others. If there are easements on those properties they should be shown. Based on my observations, the swale appeared to be properly installed. However, this asbuilt shows the swale to be too close to the south property line. See the invert elevation 8.23 east of the dumpster. Based on the design, the swale in this area being only 1' +/- from the property line will require the flow of water on the property of others. Please advise when revised asbuilts are available. That is my understanding based on my conversations with Walt Rocker and Brad Proudfoot. 2) Show the swale extending along the added elevations in the southwest corner of the site. This can be simply by showing flow arrows connecting the asbuilt elevations and labeling it as a swale. Recontouring is not required. 3) The Engineer's signed seal is required. Downer. The supt. advised they would work on this today. He advised these areas were supposed to have been seeded and strawed. 2) To compound the problem of #1, there is a blowout in the SE corner of the site's perimeter silt fence. This needs to be repaired immediately. The above 2 items need to be addressed before the additional predicted rainfall arrives. 3) The western silt fence is acting as a bulkhead. The excess silt needs to be removed. The Supt. advised they A0141 would address the erosion downstream of the flume. There was no apparent sediment spill due to this. It will be shown on a design revision submitted by the Owner's Engineer and the final asbuilts. The additional parking is a result of the 2nd floor which was not on the site drawings until the Fall. 2) The asbuilts will be revised to reflect the actual swale at the rear being built up (by a berm rather than excavation) in portions. It appears it needs to be completed within the last 100'. 3) The site is not stable and in violation of the soil erosion regulations of City of Tybee Island and the State of Georgia. 4) The as built will be revised to show the true width of the driveway on HWY 80. It actually measured in excess of 24' (measured from the outside edges of asphalt at the narrowest point of the drive. 5) The Owner is completing the stabilization of the western flume on the parking lot and the stabilization of the unpaved disturbed areas including the large area offsite to the south. 6) I advised the onsite Supt. that it was the Owner's responsibility to contact the DOT for a final inspection of work in the DOT R.O.W. This DOT final is required by the DOT prior to their releasing the DOT R.O.W. bond. 7) E &S matters will be addressed in a separate email to follow. I recommend once the design changes are made and the asbuilts are revised, the Engineer signing off on the completed project provide a current certification letter. Additionally, if the Engineer is not the registered professional producing the asbuilt drawing(s), I recommend the certification letter reference the asbuilt drawing(s). Please advise when we need to reinspect and when revised asbuilt documents are available. If it is not installed, an alternate drainage plan must be provided by the Owner's Engineer and approved by the City. The asbuilt is to come from the Owner's Engineer. To ensure that the Engineer is aware of and concurs with the postconstruction topographic survey. 2) The widths of the two drives are less than required by the permit according to the scaled widths on the survey. I recommend we pull a tape on these widths in the field this morning. a) .The width of the asphalt on the eastern drive off of Miller Avenue does not appear to be 24' as approved. I scaled less than 22'. b) The width of the asphalt on the main drive is less than 22'. Dependent on the presence of roll -type curb or lower height concrete borders, this may be determined to be acceptable. However, as the DOT does not consider concrete gutter in the drive lane width and as their minimum commercial drive width is 24', I recommend concurrence by the DOT for any significant width reduction. (Less than 23.8'.) Additionally, I'd like to pull 1 or 2 dimensions at the site if anyone has a 100' tape. (I left mine in the other vehicle.) 3) The water meter is not in the unopened 5th St. R.O.W. (SP Nature Trail) as shown on the plans. This meter and the adjacent backflow preventor are around 15' into the property. 4) The parking summary on the approved plans considers parking for only 5,000sf. The base footprint of the building (the first floor only) is 5,000sf. The parking summary shows only the minimum required parking for this 5,000sf was provided with no overflow or additional parking provided. (Please note that the handicapped parking space is not in addition to but is counted in the number of total parking spaces provided. It appears the 2nd floor may create a parking shortage. I will be by your office a little after 8am this morning. I would like to review the entire set of plans to ensure we address all other site improvement requirements. There may be an additional invoice for engineering fees as Downer continues to work toward acceptance of this project. Rockby is seeking a CO but the items in Downer's email below remain outstanding. Downer recommended a performance bond in the email. He and I discussed $5,000 during a telephone conversation this morning. Did you discuss a bond with Rockby? The recorded recombination plat has been received. A recorded limited warranty deed related to the floodproofing has also been received. This is the first such document that Tybee has required. I would like to compare it to the sample that the City of Savannah kindly provided. However, it is Planning Commission packet day and I am extremely behind from having been out of the office for the GAZA Conference. I will not have time to review the document today. Attached is a revised as -built that was received on 02/04/2011. It appears Downer has not reviewed it. Mark Boswell has not issued a concurrence letter for it. When I did the FEMA inspection last week I noticed that the dumpster enclosure did not have doors. Joe Wilson has not accepted the site. An email you sent me on Friday said that the CO may be issued today (see attached). Do you want the CO issued today despite the above issues? If yes, is a performance bond required? Should it be a temporary CO? If so, for what length of time? If you do want the CO issued I would prefer to not use the ones you prepared on Friday. I can reprint to reflect today's date and use your signature stamp to ensure correct representation to the owner and to Chatham County of the date the Certificate of Occupancy (temporary?) was actually issued. The application of hulled and unhulled grass seed with the straw mulch cover is an acceptable method of achieving permanent stabilization. However, I recommend the CIty have a mechanism to assure the Owner will reseed and take other measures necessary to establish a suitable ground cover as soon as reasonable possible. 2) The stormwater discharge from the parking lot at the west end is functioning at this time with rip -rap. The 2 curb in that area has not been backed up with soil. As it appears that the contractor has not been able to complete this in compliance with the approved plans, a design change appears eminent. 3) I have not reviewed and concurred with a complete asbuilt. It is my understanding that the contractor and Engineer were working on this yesterday. As you will remember, I had recommended that the Engineer issue a current certification letter with the revised asbuilts. I recommend the above three items be bonded. A. Slab: The drawing required an 18" thick concrete slab with rebar (see drawings) to counteract the uplift forces during a flood. The foundation and slab were installed per plans and specifications, and they were inspected by my office and accepted. B. Mechanicals: All mechanicals are installed above flood elevation (see pictures and letter from electrician) per drawings and specifications. Hot water heater tank is located below the flood elevation with all hookups and electrical work above flood elevation. Sewer system does not require a back flow preventer because it is gravity feed system. They were inspected by my office and accepted. C. Concrete block wall: CMU walls were installed per structural plans and specifications. Water- proofing membrane was applied to the CMU wall to counteract water leakage during a flood. The CMU walls were inspected and accepted by my office. D. Flood panels: The flood panels were custom fabricated for each door location and numbered. Each panel is 1/4" thick aluminum with neoprene gaskets, designed to resist the passage of water and withstand the lateral loads during a flood. The panel system is anchored to each door opening wall with three (3) 5/8" threaded bolts. The panels are stored above the office area. PURPOSE This document outlines the actions and responsibilities required to maintain and protect Village Plaza, LLC, located at 502 1st Street, from a flood. This flood operation plan includes requirements for the Emergency Action Plan, Annual Inspection and the Maintenance and Training Plan required to retain the Certificate of Occupancy for the building. 2. EMERGENCY ACTION PLAN The following steps are to be taken by the building owner, Village Plaza, LLC, during a flood. A Chain of Command 1. Responsibility: The primary responsibility for maintaining and carrying out the flood emergency plan shall lie with a Member of Village Plaza, LLC, William Navon. 2. Notification: The primary responsibility for notification of a flood event lies with the Executive Assistant of Village Plaza, LLC, Sue Greene. The Executive Assistant will monitor weather reporting agencies and be the main point of contract with emergency management officials. In the event that the Executive Assistant is unavailable, the responsibility will lie with the Member, William Navon. 3. Preventative Action: The primary responsibility for ensuring the preventative actions are put into place shall lie with the Managing Member, William Navon. He will coordinate the installation of all of the flood deterrent measures. In the event the primary contact is unavailable, Sue Greene will implement the preventative actions. 4. Maintenance Action: The primary responsibility for ensuring the maintenance action plan is maintained shall lie with the Managing Member, William Navon. He will coordinate annual installation drills, review and update contents and accuracy of the plan, review storage and general condition of the flood deterrent measures and review the overall performance in ensuring the safety of the building, its occupants and its content at least once per year. In the event that the Managing Member is unavailable, the responsibility falls upon Sue Greene, the Executive Assistant. Procedures 1. Monitoring and initial notification: Ongoing monitoring for potential flood events will be the responsibility of the Executive Assistant. The owner will subscribe to a weather alert service that issues automated warnings of potential flood events on a 24/7 basis. Responsibility for maintaining and monitoring the weather alert system will fall to the Executive Assistant. A flood will be considered as pending when an official "watch" or "warning" is issued by the National Weather Service for any of the following: a. Flood b. Tropical Storm /Gale c. Hurricane 2. Procedures: The following procedures shall be followed during a pending flood event: a. Notification: When a potential flood event is publicly announced, the Administrative Assistant will notify the Managing Partner and the Maintenance Manager and at least one additional partner in the ownership group. b. Decision: The Managing Partner will assess the potential impact and timing of the flooding threat. Based on a consensus of the available other responsible parties, he will determine if implementation of the flood measures is necessary. c. Implementation: If implementation is mandated, the Managing Partner will call the Maintenance Manager or his assign to install the flood deterrent measures. d. Evacuation: Simultaneously, the Managing Partner will contact the building tenant to coordinate the evacuation of staff from the facility. The Tenant will contact Managing Partner upon completion of the evacuation. e. Confirmation: Upon completion of the implementation of the flood deterrent measures and the evacuation of the facility, the Managing Partner will contact the other members of the ownership group and the Tybee Island Community Development Dept.. f. Response Time: The Managing Partner will consider the response time required to implement the flood prevention measures in determining the implementation. 3. Restoration of Standard Activities: Once a "watch" or "warning" has been lifted by the National Weather Service, the Managing Partner will reconvene the available members of the ownership group to confirm the restoration of normal activities. Pending consensus of the ownership group, the Managing Partner will notify the Tenant that they can return to the facility and restart operations. The Managing Partner will also instruct his staff and the Maintenance Manager to remove the flood deterrent measures and return them to their proper storage. 3. ACTIVE FLOOD DETERRENT MEASURES The following outlines the physical flood deterrent measures that require installation of or action by the Owner's staff to put in place. These measures must be implemented in an emergency situation at the building. The flood deterrent measures have been set in place to create Class I flood resistance throughout the building. 1. Doors: There are three exterior doors that require flood - proofing measures. The doors are the three storefront doors on the front of the building. a. Aluminum Plates: There are three custom fabricated aluminum plates fabricated to fit over each door opening. Each plate is permanently marked to indicate which door they fit and which way the plate faces (up, down, right, left). Each plate was fabricated from aluminum and weighs approximately 50 pounds. Each will require one person to set into place. These plates are placed above the office area. b. Seals: Each plate is fitted with a % "neoprene seal. The seal is permanently attached to each plate. c. Bolts: Each plate will require six (6) 5/8" thick bolts to install, three on each side. All bolts /brackets shall be left connected to building. 2. Plumbing System: a. Domestic Water System: There is a main shut -off valve located to the right side (west) of the building. The valve should be closed to the off position and locked during a flood event. 3. Electrical and Mechanical systems: All electrical and mechanical systems are above flood plain elevation (see pictures and letter in field report) 4. PASSIVE FLOOD DETERRENT MEASURES The following outlines physical flood deterrent measures that have been permanently set in place. While these require no action by the owner's staff during a potential flood event, they will require periodic maintenance and inspection to ensure their ongoing stability and functionality. 1. Architectural: a. Waterproofing: All masonry walls have been coated with a waterproofing system from 12" below ground level to the top of the masonry wall. This system was applied up to 6" above the flood elevation. The waterproofing surface is concealed by the exterior stucco. b. Doors: Please see above door system. 2. Structural: a. Slab: The drawing required an 18" thick concrete slab with rebar (see drawings) to counteract the uplift forces during a flood. b. Concrete block walls: The exterior walls are 8" thick and have additional reinforcing to resist the lateral pressure of a flood. The entire block wall is filled solid. 3. Plumbing Systems: All fittings in the flood - proofed rooms are equipped with water -tight collars to prevent water infiltration and seepage. The main water line is outfitted with a backflow preventer. 4. Mechanical and Electrical systems: All mechanicals are above flood elevation (see field report letter and pictures). 5. DUTIES Two members of the on -call maintenance staff will execute the following activities upon notification from the Manager: 1. Door Protection a. Go to area above office and locate necessary materials b. Locate the 3 door panels c. Set door panels with corresponding door in proper orientation d. Verify neoprene seals are in place e. Install door panels 2. Water Shutdown a. Verify all water valves are in the off position b. Locate the main shut -off valve on the right side (west) of building and shut off 3. Mechanical and Electrical Shutdown a. Turn off all Mechanical equipment b. Set main disconnect in main power panel to OFF 6. ANNUAL INSPECTION, MAINTENANCE AND TRAINING PLAN The manager partner or assign will require an annual inspection, maintenance and training as follows: 1. Inspection and Maintenance: The owner recognizes that there shall be an annual routine inspection that these measures remain viable. The following is an inspection schedule for the flood deterrents. The reports should be filled out annually with the managing partner and filed at their home office. Every five years, the report, accompanied by the report of a registered structural engineer and the staff reports from the previous years, will be filed with a summary cover letter to the Tybee Island Community Development Dept., noting the condition and maintenance of the flood - proofing systems as outlined below. a. Doors protection i. Aluminum Plates: Annually inspect to verify that all panels are properly stored. Staff will annually install the panels in accordance with the procedures listed above. Every five years, a registered structural engineer should also inspect and report on the observed condition of the door protection system. ii. Seals: Annually inspect all seals to ensure that seals remain resilient and watertight. Inspections should note any cracks or defects in the gaskets. iii. Bolts: Verify all bolts are present for all doors and properly stored. b. Waterproofing: On an annual basis, the owner will water down the stucco portion of the CMU wall and notice any water intrusion into the building. Inspections should note any cracking, damage areas or loss adhesion. Every five years, a structural engineer should also inspect and report on the observed condition of the waterproofing. c. Interior Finishes: Annually review the interior finishes in the building to determine if there are any defects or failures in the integrity of the surface. Inspections should note surface cracking, loss of adhesion and other potential failure indicators. d. Slab and Concrete Masonry Walls: Staff should provide an annual review and report any surface cracks, settling, depressions or heave in the concrete slab and masonry walls. Every five years a structural engineer should also inspect and report on the observed condition of the slab and masonry walls. e. Plumbing Systems i. Domestic Water System: Annually verify that all valves are in working condition. Any defects should be corrected immediately. ii. Fittings: Verify annually by use of a pressure test that all fittings remain watertight to a pressure of 40 psi. Any defects should be corrected immediately. f. Mechanical and Electrical Systems: Annually verify disconnects and other equipment is operational and provide safe shut down of all electrical and mechanical systems to the facility. 7. TRAINING On an annual basis, the Managing Partner or assign will conduct a training session for all employees assigned to implement the flood - proofing measures. Tybee Island Community Development Dept. will be notified at least one week in advance of the training session. The session should include the following: a. Review of potential dangers and hazards associated with occupying a building located within a floodplain. b. Review of the physical flood - proofing measures required to secure the structure. Review the components and processes required to install the active flood - proofing measures as well as an overview of the inactive measures already put in place. c. Review of the evacuation plan for staff associated with the facility. This should also include a review and confirmation of the alternate relocation sites and alternate travel routes. d. Review annual and five -year inspection and maintenance reports. e. Response Time: On an annual basis, the Managing Partner will document and forward for review by the governing authorities the response times to implement the flood prevention measures. The response times will include travel time to and from installers' homes and time to notify that the measures have been put in place. Annual drills will be held to certify and update the response time. 8. SURVIVORSHIP The requirements and conditions of this document will be recorded by reference as part of the title to the property and shall run with the land to all future owners and tenants. Any changes or modifications to this document require the review and consent of the Tybee Island Community Development Dept.. Use CDX grade, or better, plywood that is in good condition. Do not use plywood that has delaminated, is easy to bend, or is badly warped. Step 1 Measure the length and width of each window. Subtract 1/2" from the width measurement and 1/4" from the height measurement. Record the measurements in this measurement cha rt. Step 2 Cut the plywood covers for each window. Some hardware stores will cut the plywood for you. Use good quality (CDX or better) 3/4" plywood. Plywood comes in 4' x 8' sheets. Windows bigger than 48" x 48" will require 2 or more sections of plywood. On the opposite side of the plywood cover make sure the set screws are flush on the clips. Insert the plywood cover into the aluminum window casing until the clip touches the frame. Using a 5/32 Allen wrench, firmly tighten the flush set screws. Then tighten the set screws on the opposite side at least 1/2 turn. Removing PLYLOXTM is easy! You can remove PLYLOXTM clips quickly and easily. Use the Allen wrench and loosen the set screws. This releases the PLYLOXTM clip and the plywood is easily removed. The PLYLOXTM clips can then be stored and reused the next time you are threatened. While the deficiencies are not necessarily the fault of the Owner or his agent, they are their responsibilities. The two areas of deficiencies are in the two most basic and common BMPs on local projects. They are: Co — Construction Exit Shc — Sediment Barrier Correct installation information can be found not only in the "Green Book" but in the Field Manual for Erosion and Sediment Control in Georgia, Fourth Edition 2002, 4310 Lexington Road, P.O. Problems with the Co is not limited to the installation, but to the material. Specifically, the job site personnel are telling me the stone delivered is the 1.5 " -3.5" stone they requested. I have experienced on my own projects suppliers not adhering to the specs they are given with the orders. While I regret this situation, I will no longer be accepting any deviation from the State requirements. The stone will be a representation of 1.5 " -3.5" stone or larger. Gradations that are obviously smaller will no longer be tolerated. The smaller stone allows for a smoother surface with smaller voids, thereby reducing the function of the construction exit. Similarly, I am finding Type A sediment barriers installed where Type C is required and shown on the permit drawings. I have attempted to work with the Owners through increased inspections, additional backup BMPs, etc. This has evidently become common knowledge based on the installations I am finding. Where two rows are called for they will be installed with a separation that allows for the first one to fail (fall over) without impacting the second one. The complete assembly and installation must be compliant; steel or wood posts, post spacing, Type C or A. There has been no discharge into state waters due to previous occasional deficiencies. However, the increased frequency has created unnecessary exposure to: 1) Sediment discharge into nearby waters of the State, 2) Increased maintenance efforts by the DPW on downstream lines. 3) Due to #2, higher cost to island taxpayers. I will be adhering strictly to the manual on all BMPs and not just these two.
