Subject: Follow up request for Certified Copies of public records - Re: GS #2328 (Architectural
Review of 1314, 1320 and 1330 NOB)

Dear Custodian of Records,
Ms. Rowan Basel responded to my recent request to inspect public records. You labeled that request
as GS #2328. 1 followed the general links provided to me in Ms. Rowan Basel's response. Upon
further searching I found records located at the specific URL address: hftp://WWW2.qulf-
stream.org/weblink/O/doc/8642/Pagel.aspx. The online file I found at this address is labeled APP
011-06 (Large Plan In File) and contains pages numbered 1 through 85:
Of the 85 pages contained in this file I request certified copies of the following pages:
1 - 5, 13, 31 - 33, 37, 41 - 44, 48
By my calculation I understand there to be 15 pages of records I am requesting to be
certified. Attached to this email is a .pdf of those 15 pages of public records. I provide this for your
convenience and to further identify which records I request to obtain as certified copies. I further
understand these copies cost $1 each. I agree to pay this amount.
Please let me know when I can come by later today to pay you and pick up the 15 certified records.
Thank you for your assistance in this matter.
Sincerely,
Chris O'Hare
