PART 1
1.2 QUESTIONS AND ANSWERS DURING PRE -BID MEETING ON JUNE 1, 2015
1. Question: Which untility provides electrical power to the facility?
Answer: Richmond Power and Light.
2. Question: Does the City of Richmond bill itself and pay itself for sanitary sewerage service and
is the Contractor responsible for this cost?
Answer: Yes. For a groundwater remediation project such as this, the Contractor will be billed on
a quarterly schedule by the Richmond Sanitary District an amount of $50 for the first 39,000 (or
fewer) gallons of water discharged each month plus $0.0027/gallon for each gallon exceeding the
first 39,000 gallons discharged In that month. The Contractor will furnish and install a totalizing
flow meter on the remediation system to measure, totalize, and report the flows to the Engineer
and the Richmond Sanitary District each month. The Contractor's bid price for this item will be
measured and paid to the Contractor in two parts. The first part will be the Contractor's bid price
for the first 39,000 (or fewer) gallons per month. The second part will be the Contractors bid
price per gallon for discharges exceeding 39,000 gallons per month. The Bid Form, Section 00300
of the Bidding documents, is modified to reflect this.

3. Question: Who will determine whether the optional remediation well (RW6) shown in the
approved Remedation Work Plan is to be installed as part of the Work?
Answer: Remediation well RW6 shall be part of the work and is not optional.
4. Question: Are there underground utilities present within the limits of the work? If so, are
there any construction plans for the facility illustrating their nature and location(s)?
Answer: Yes, there are existing underground utilities within the limits of the work, including but
not necessarily limited to electric, potable water, fire protection water, natural gas, sewer, and
telecommunications. Yes, the Engineer has three sets of historical plans for the facility in his
possession but has determined that because each set is different (and incomplete relative to
recent developments), they are not usable for planning purposes. The Contractor is advised to
begin his process for identifying underground utilities by calling Indiana 811. The Contractor is
further cautioned to engage the services of an independent locator such as Lucky Locators or
Bloodhound as part of his remediation system layout process as described in the specifications.
S. Question: Will there be progress payments to the Contractor as the Work progresses?
Answer: Yes. The Contractor will prepare a monthly Schedule of Values and submit it for the
Engineer's approval for payment by the Owner. Lump sum items only partially complete at the
end of a particular monthly billing cycle can be claimed as a percentage of the lump sum bid price
for the effected item.
6. Question: Is closure of the existing monitoring wells at the conclusion of the project part of
the Work?
Answer: No.
7. Question: What Is the extent of site restoration at the conclusion of the project?
Answer. This is described in the project specifications. The remediation wells shall be sealed and
abandoned as specified. Unless otherwise specified, remediation wellhead road boxes shall be
removed and the resulting void shall be restored using Portland cement concrete. Any service
lines that shall remain in the ground at the conclusion of the project shall be secured at all open
end(s), buried, or covered with restorative Portland cement concrete pavement.
8. Question: Is the Contractor responsible for sampling, testing, or monitoring?
Answer: Yes. The Contractor shall sample, test, or monitor as required in any permits he must
obtain for implementation of the project and as otherwise required to comply with environmental
and occupational safety and health rules affecting his prosecution of the Work. The Contractor
shall obtain an industrial pretreatment discharge permit for operation of the project as provided
in the project specifications. The application for this permit must be submitted at least 90 days
before treated water is discharged to the sanitary sewer. This permit application must be

submitted early in the process to minimize unnecessary delays. Information regarding the City of
Richmond's Pretreatment Sewer Usage is available at

Testing and monitoring under this discharge permit will require that the Contractor sample,
analyze, and report the concentration of VOCs in the treated water effluent not more frequently
than monthly. The analytical method required by the local sanitary district shall be EPA Method
624, quantitating at least the following site -specific list of chemicals of concern to reporting limits
not lower than 5 WL:
1. Benzene
2. 2-Chlorotoiuene
3. Ethylbenzene
4. n-Hexane
5. Methyl-Tert-Butyl Ether (MTBE)
6. Naphthalene
7. n-Propylbenzene
8. Toluene
9. 1,2,4-Trimethylbenzene
10. 1,3,5-Trimethylbenzene
11. Vinyl Chloride (2.3 WJL in a single groundwater sample)
12. Xylenes
References to SW-846 Method 8260E or 8260C, included in the Summary of Work, Section 01010,
shall be considered amended to read EPA Method 624. The Bid Form, Section 00300, is modified
as attached to provide a unit -rate line item for the above -listed sampling, analysis, and reporting
to be provided by the Contractor. Twenty-four (24) units are provided, but the actual number of
units that will be required by the Contractor's permit may be less according to sanitary district
personnel. Whether sampling is required on a monthly schedule or less frequently, the
Contractor operating the remediation system shall report the total system groundwater
discharges each month using a form to be provided by the Richmond Sanitary District. Operation
of the system shall be by, or under the supervision of, the Contractor's appropriately qualified and
responsible person who will also sign the monthly discharge reports. The Contractor's responsible

person may be a Senior Project Manager, P.E., LPG, or similarly qualified and experienced person.
There is no requirement for a licensed waste water treatment system operator for this project.
The Engineer shall be responsible for other sampling, testing, and monitoring as needed to assess
the performance of the remediation relative to remaining concentrations in soil, vapor, or
groundwater as remediation progresses.
9. Question: Is vapor phase treatment necessary to control the emission of petroleum
hydrocarbon volatile organic compounds (VOCs)?
Answer. Yes. Vapor phase emissions from the remediation system will be malodorous, especially
early in the remedial operation. The specifications require that no malodorous conditions be
created during operation. Regarding the mass of VOCs in the ground, the Engineer estimates a
total emission potential of approximately 2.5 tons (5,000 pounds) over the life of the project. The
Engineer estimates that the hourly potential to emit VOCs is less than 3 pounds per hour and less
than 15 pounds per day, exempting the project from air pollution control or permitting
requirements. The Engineer will perform vapor -phase air monitoring or specific calculations that
might be required to demonstrate this. The Engineer will also determine when vapor phase
treatment is no longer necessary to prevent malodors. Timed operation of the system, such as
overnight if the site is vacant, may be considered as a possible means to mitigate malodors.
1.3 SECTION 00300 - BID FORM
A. Attached to this Addendum is a revised Section 00300 Bid Form, modified as described
above to provide for unit rate measurement and payment for monthly sewerage fees
and sampling analysis/reporting that will be required in the Contractor's Sanitary
Sewer Discharge Permit.
1.4 SECTION 00822 - WAGE RATES
A. Attached to this Addendum is the General Decision Number: IN150006 OS/29/201S
IN6, Superseded General Decision Number: IN20140006. State: Indiana. Construction
Types: Heavy and Highway. This is the wage determination that will be included in
Contract Documents for this project.

Pursuant to the published "Advertisement for Bids", the undersigned has investigated the conditions
affecting the cost of the proposed Former Carpenter Manufacturing Site Remediation Work — Phase 1,
and having examined the Site and understanding the requirements set forth in the Contract Documents,
hereby proposes to provide and furnish all labor, materials, tools, equipment and all utility and
transportation services necessary to perform and complete, in a workmanlike manner, all the above
work as required by said Contract Documents, including any and all addenda now on file in the Office of
Weaver Consultants Group, 7121 Grape Road, Granger, IN 46530.
BASE BID
Bidder agrees to perform all of the Work described in the Contract Documents for the following best an
final prices listed on the Bid Form below.
The unit and lump sum prices shown In the Bid Form below include all labor, materials, bailing, shoring,
removal, overhead, profit, insurance, taxes, and other costs necessary to complete the furnished work of
the several kinds called for. The Bidder is solely responsible to provide sufficient materials, equipment,
and effort to complete the work.
The Bidder declares that it understands (a) that the quantities shown for unit price items are
approximate only and are subject to either increases or decreases, (b) that, if the quantities of any of the
items or work are increased, the Bidder proposes to do the additional work at the prices stated herein;
and if such quantities are decreased, the Bidder also understands that payment will be made on the
basis of the actual quantities at the unit price bid and will make no daim for anticipated profits for any
decreases In quantities, and (c) that actual quantities will be determined upon completion of the Work
at which time adjustments will be made to the contract amount by direct increase or decrease.

in Sections 00610, 00615, and 00700 of the Contract Documents. The surety providing the Bond(s) must
be on the U.S. Treasury Department's list of approved bonding companies and the EJCDC forms C-610
and C-615 with affixed corporate seals are the only approved formats.
B D ITEM MEASUREMENT AND PAYMENT
Item 1— Preconstruction Activities and Mobilization - The lump sum price for this item shall be payment in
full for mobilization of all parts, material, and equipment to the site as well as Contractor -provided utilities
and ongoing related expenses considered normal for administration of the work. included in this bid item
shall also be the cost associated with up -front permitting and plan preparation including, but not limited to,
obtaining the industrial pretreatment discharge authorization permit, building permit (if any), and
authorization to construct and operate from the Office of Air Management in conformance with Section
01010, and preparation of the site specific health and safety plan in conformance with Sections 01500,
01501, and 01700.
item 2 — Field Enaineerine. Underground Utility Locating, and Remediation System Layout - The lump sum
price for this item shall be payment in full for all work necessary to verify existing underground utilities,
layout the remediation wells, and the remediation system, and to prepare the Record Drawings in
conformance with Sections 01010 and 01050. Payment will follow Engineer's approval of the Contractor's
required submittals.
Item 3 — Furnish and Utilize Security. Barricades, and Temporary Fence for Public Protection During
Remediation System Construction — The lump sum price for this item shall be payment in full for all work
necessary to furnish and maintain security of the Contractor's operations and laydown area, as well as
barricades and temporary fence that will be needed to protect the tenants and members of the public who
work at or visit the facility in conformance with Section 01501. Progress payments may begin fdbwing
Engineer's approval of the Contractor's protective measures.
Item 4 — Drilling and Installation of HVDPE Remediation Wells —The unit price per well for this item shall be
payment in full for all furnishing and installation of all materials, parts, equipment, and labor to construct
the HVDPE remediation wells in conformance with Sections 02050, 02521, and 03300. Progress payments
may begin following the completion and Engineer's approval of each HVDPE remediation well.
Item 5 — Furnish and Install Remediation Well HVDPE Service Unes and Related Interconnections — The
lump sum price for this item shall be payment in full for all furnishing and installation of all materials, parts,
equipment, and labor to construct the dual -phase service lines and related interconnections that may be
required by the Contractor's design in conformance with Sections 02050, 02316, 02519, 03300, and 15200.
This item is anticipated to be completed under a single progress payment.
Item 6 — Furnish and Install Remediation System Equipment. Controls. and Enclosure — The lump sum price
for this item shall be payment in full for all furnishing and installation of all materials, parts, equipment, and
labor to construct, furnish, deploy, and connect the remediation system equipment with the HVDPE
remediation wells in conformance with Sections 11300, 13120, 15200, 16050,16402, 16446, 16510. This
item is anticipated to be completed under a single progress payment after the Engineers approval of the
delivery and installation of an acceptable remediation system.
Item 7 — Remediation System Startup. Shakedown, and Optimization - The lump sum price for this item
shall be payment In full for all furnishing and installation of all materials, parts, equipment, and labor to
start, test, and optimize the remediation system operation over the minimum 7-day commissioning period
in conformance with Section 1010 and 01810. This item is anticipated to be completed under a single

progress payment after the Engineer's approval of the commissioning process and Contractor's
commissioning report.
Item 8 — Construction Demobilization and Record Drawings — The lump sum price for this item shall be
payment in full for all furnishing and installation of materials, parts, equipment, and labor to demobilize all
construction equipment and materials at the conclusion of construction, as well as to provide record
drawings of the remediation system and WDPE wells in conformance with Sections 01010, 01050, and
01700.
Item 9 — Monthly Operation and Maintenance - The monthly price for this item shall be payment in full for
all furnishing and installation of materials, parts, equipment, labor, electrical power, and all other costs for
operation and maintenance of the remediation system in conformance with Sections 01010 and 01830.
Monthly progress payments will follow the Engineer's approval of the monthly 0&M report submittals as
required.
Item 10 — Sewerage Fee for Discharge of Treated Water, First 39,000 Gallons/Month — This item shall be
measured as one unit for each calendar month the Contractor operates the remediation system. This item
shall be payment in full for the Contractor's pass through of sanitary sewerage expenses for discharging the
first 39,000 (or fewer) gallons of treated water each month in conformance with Addendum No. 001.
Item it — Sewerage Fee for Discharge of Treated Water. Exceeding the First 39,000 Gallons/Month — This
item shall be measured by reading the totalizing flow meter furnished by the Contractor who will calculate
and report the total flow in any month that exceeds 39,000 gallons. This Rem shall be payment in full for
the Contractor's pass -through of sanitary sewerage expenses for discharging water in excess of the first
39,000 gallons per month in conformance with Addendum No. 001.
Item 12 — Sample. Analyze, and Report Flows and EPA Method 624 Results for Industrial Discharge — This
Item shall be payment in full for the Contractor to collect, sample, analyze, and report the results for a grab
sample of the treated water discharge each time such a sample is required in conformance with Addendum
No. 001.
Item 13 — Site Resto tin and Demobilization — The lump sum price for this item shall be payment in full
for final decontamination of all equipment, removal of all temporary facilities (including but not limited to
the remediation system equipment and enclosure), sealing and abandonment of all remediation wells in
accordance with State law, demobilize all equipment and materials from the site, and final restoration of
the site to its original condition in conformance with Sections 01010, 02316, and 02519.
SCHEDULE
The undersigned further agrees to be substantially complete with the furnishing and construction of all
such work within 180 consecutive calendar days from and including the date of commencement to be
stipulated in a written "Notice to Proceed" to be issued by the Board of Sanitary Commissioners. All
construction work shall be completed and ready for final payment within 210 days of the "Notice to
Proceed."

operation to which welding is incidental.
Unlisted classifications needed for work not included within
the scope of the classifications listed may be added after
award only as provided in the labor standards contract clauses
(29CFR 5.5 (a) (1) (11)).
The body of each wage determination lists the classification
and wage rates that have been found to.be prevailing for the
cited type(s) of construction in the area covered by the wage
determination. The classifications are listed in alphabetical
order of "identifiers" that indicate whether the particular
rate is a union rate (current union negotiated rate for local),
a survey rate (weighted average rate) or a union average rate
(weighted union average rate).
Union Rate Identifiers
A four letter classification abbreviation identifier enclosed
in dotted lines beginning with characters other than "SU" or
"UAVG" denotes that the union classification and rate were
prevailing for that classification in the survey. Example:
PLUM0198-005 07/01/2014. PLUM is an abbreviation identifier of
the union which prevailed in the survey for this
classification, which in this example would be Plumbers. 0198
indicates the local union number or district council number
where applicable, i.e., Plumbers Local 0199. The next number,
005 in the example, is an internal number used in processing
the wage determination. 07/01/2014 is the effective date of the
most current negotiated rate, which in this example is July 1,
2014.
Union prevailing wage rates are updated to reflect all rate
changes in the collective bargaining agreement (CBA) governing
this classification and rate.
Survey Rate Identifiers
Classifications listed under the "SU" identifier indicate that
no one rate prevailed for this classification in the survey and
the published rate is derived by computing a weighted average
rate based on all the rates reported in the survey for that
classification. As this weighted average rate includes all
rates reported in the survey, it may include both union and
non -union rates. Example: SULA2.012-007 5/13/2014. SU indicates
the rates are survey rates based on a weighted average
calculation of rates and are not majority rates. LA indicates
the State of Louisiana. 2012 is the year of survey on which
these classifications and rates are based. The next number, 007

in the example, is an internal number used in producing the
wage determination. 5/13/2014 indicates the survey completion
date for the classifications and rates under that identifier.
Survey wage rates are not updated and remain in effect until a
new survey is conducted.
Union Average Rate Identifiers
Classification(s) listed under the UAVG identifier indicate
that no single majority rate prevailed for those
classifications; however, 100% of the data reported for the
classifications was union data. EXAMPLE: UAVG-OH-0010
08/29/2014. UAVG indicates that the rate is a weighted union
average rate. OH indicates the state. The next number, 0010 in
the example, is an internal number used in producing the wage
determination. 08/29/2014 indicates the survey completion date
for the classifications and rates under that identifier.
A UAVG rate will be updated once a year, usually in January of
each year, to reflect a weighted average of the current
negotiated/CBA rate of the union locals from which the rate is
based.

WAGE DETERMINATION APPEALS PROCESS
1.) Has there been an initial decision in the matter? This can
be:
* an existing published wage determination
* a survey underlying a wage determination
* a Wage and Hour Division letter setting forth a position on
a wage determination matter
* a conformance (additional classification and rate) ruling
On survey related matters, initial contact, including requests
for summaries of surveys, should be with the Wage and Hour
Regional Office for the area in which the survey was conducted
because those Regional Offices have responsibility for the
Davis -Bacon survey program. If the response from this initial
contact is not satisfactory, then the process described in 2.)
and 3.) should be followed.
With regard to any other matter not yet ripe for the formal
process described here, initial contact should be with the
Branch of Construction Wage Determinations. Write to:
