The Pro Forma invoice includes regular monthly charges for your online subscription(s)
only as covered by your order# 0202021597 dated October 20, 2016. It does not reflect
any additional charges that may be incurred due to the following: user(s) accessing
databases or transactions that are not included in the plan, tax, price increases due to a
contract term ending and the start of month-to-month billing or annual price increases
due to multi-year agreements which were not calculated in the Pro Forma agreement.
I understand and agree that we: GULF STREAM are responsible for and agree to pay any
additional charges not covered by this Pro Forma Invoice request.
