ADDENDUM
ZATHIS ADDENDUM, (Number 1 to Contract No. 143-2015) is made and entered into this
day of 2016, by and between Richmond, Indiana, a municipal
corporation acting through its bard of Public Works and Safety with its office at 50 North 5th Street,
Richmond, Indiana, 47374 (hereinafter referred to as "City"), and Robert Donathan DB/A Donathan's
Inspections and Removal Services, 3504 North Linden Street, Muncie, Indiana, 47304 (hereinafter
referred to as "Contractor").
WHEREAS, City and Contractor entered into Contract No. 143-2015 on December 10, 2015,
which Agreement retains Contractor to provide services in connection with asbestos
abatement for seven (7) properties for the City's Blight Elimination Program ("BEP"
per Indiana Housing and Community Development ("IHCDA") Guidelines; and
WHEREAS, The parties to said Agreement wish to allow for an increase in the contract amount
due to additional asbestos that was undocumented in the asbestos inspection reports
,provided to the parties; which asbestos must be abated and which services are to be
conducted pursuant to the Agreement under the same terms and conditions as set
forth in Contract No.143-2015.
NOW, THEREFORE, in consideration of the mutual promises: and covenants herein contained,
including the above stated recitals. City and Contractor hereby agree as follows;
1 The City and Contractor executed and entered into Contract No. 143 2015, on
December 10, 2015_ Pursuant to Contract No. 143-2015, Section XII Miscellaneous,
the Agreement may be : altered or amended in whole or in part at any"tithe by filing
with the Agreement a written instrument setting forth such changes signed by both
parties. As set forth above andas set forth on Contractor's quote, attached as Exhibit
A to this Addendum, services in connection. with the abatement of the additional
asbestosshall be provided as set forth within Contract No. 143-2015. Such services.
shall be performed in a manner consistent with Contract No. 143 2015.
2. Section Ill Compensation of said Agreement shall be altered in order that City shalt
pay Contractor an additional amount of Six Thousand )live Hundred Dollars and Zero
Cents ($6,500.00) for the additional services, with said final contract amount to be
increased to a total amount not to exceed Seventeen Thousand Dollars and Zero
Cents ($17,000.00).
3. City and Contractor expressly agree that all other terms, conditions, and . covenants
contained in Contract No. 143-2015, shall be applicable to the work performed
pursuant to this Addendum, unless such term, condition or covenant conflicts with
this Addendum Number 1, in which case this Addendum Number 1 shah be
controlling.

4. Both Parties hereby agree that with the exception of the additional abatement services
to be provided and increased contract amount Contract No. 143-2015 shall remain
intact and unchanged and in full force and effect.
5. Both parties agrte that any person executing this Contract in a representative capacity
hereby 'warrants that he or she has been duly authorized by his or her principal to
execute this Addendum.
