Contractors who perform renovation, repairs, and painting jobs in pre -1978 housing and child- occupied facilities must, before beginning work, provide owners, tenants, and child -care facilities with a copy of EPA's lead hazard information pamphlet Renovate Right: Important Lead Hazard information for Families, Child Care Providers, and Schools. Contractors must document compliance with this requirement. After April 22, 2010, federal law will require you to be certified and to use lead -safe work practices. Projects must be done by a Certified Lead Renovation Firm AND a Certified Renovator When disturbing lead -based painted or coated surfaces or components more than 6 ft2 per interior room or 20 ft2 of exterior surfaces AND all window replacements and partial demolitions of any size. * * * * * * * * * * * * * * * * * * * ** Asbestos /Environmental Notification to Georgia EPD for Projects Involving Demolition, Wrecking, or Renovation The undersigned hereby acknowledges that the issuance of this permit does not in any way grant permission to the owner, owner's representative, or permit holder to proceed with demolition, wrecking, or renovation of a structure prior to the filing of any required ten (10) day "Project Notification for Asbestos Renovation Encapsulation or Demolition" form in accordance with the Georgia Asbestos Rules. The Georgia Environmental Protection Division administers the rules. In most cases, the rules require both the owner and the involved contractors to assure the portion of the building involved in the project is thoroughly inspected by an Accredited Asbestos Inspector for materials that contain asbestos; and the removal of the asbestos before renovation, wrecking, or demolition begins almost without exemption. Georgia EPD requires a completed demolition notification from be submitted 10 workings days in advance even if no asbestos is present in the building. We estimate our completion date to be approximately 2 'h weeks from the start of the project. Once a signed proposal and deposit have been received, a project schedule will be submitted for review. It is the homeowner's responsibility to keep all children and pets away from the work area. The workmen can not be responsible for their safety. Please insure that all pets are confined to another area of the home or yard. Workmen often need access to attics, basements, and/or crawlspaces to complete their connections and inspections. We ask that you make these areas accessible to them. We recommend that the homeowner remove all valuable or fragile items from the work area or surrounding pathways. The homeowner agrees to carry insurance covering fire, theft, and storms in sufficient amounts to cover work and materials under construction and agrees to compensate Coastal Bath & Kitchen for losses sustained by these conditions. The total price does not include the repair of any hidden damage or removal or rerouting of any unseen obstacles that may be discovered during the renovation. While precautions are used to protect the entire site, Coastal Bath & Kitchen cannot be responsible for cosmetic damages to areas beyond the specified room(s) being remodeled. Damage to household items caused by vibration or any other normal construction activities are not covered by Coastal Bath & Kitchen. We will repair any nail -pops that occur, yet any repainting or refinishing of these areas will be the owner's sole responsibility. Any alteration or deviation from the project specifications that requires extra labor and/or materials will be executed only upon a written Change Order signed by the homeowner and will become an extra charge over the agreed amount. Change Orders requiring additional charges shall be paid in full at the signing of the form. Change orders resulting in a credit will be deducted from the final contract payment. Coastal Bath & Kitchen cannot be responsible for any existing code violations found by the local inspection department that are required to be corrected before the project can proceed. We will assist the homeowner in any way possible to make any previous violations meet today's code requirements but all costs will be the responsibility of the homeowner. Agreements made with mechanics or subcontractors on the job will not be recognized. No statement, arrangement, or understanding, expressed or implied, not contained herein will be recognized. No warranty work will be performed unless the contract is paid in full. We cannot warranty materials or services supplied by the homeowner. If defects in these products result in additional work, there will be an additional charge. All materials will be installed as specified and carry the manufacturer's warranty. All work will be completed in a neat and workmanlike manner using new material of good quality, and in compliance with all applicable building codes, ordinances, laws, statutes, rules, and regulations. If your home was constructed prior to 1978, the E.P.A. requires that we provide you with the latest Pre - Renovation Lead Base Paint brochure. By signing below you are indicating that you have received said document. This proposal is valid for 60 days. A fifty percent deposit is due upon acceptance of this proposal, twenty-five percent is due halfway through the project, and the remaining balance is due upon completion. `Halfway' will be verbally agreed upon by the signing customer and Coastal Bath & Kitchen and will be specified on the project schedule.
