Pursuant to Florida’s public record laws, including but not limited to Chapter 119, Florida Statutes, please provide
the following documentation/forms at your earliest convenience. If you have any questions please do not hesitate to call

If payment for reasonable copying costs is required, please let me know and we will arrange payment immediately.

Please provide your current versions of the following in native format.
1.All documentation required by Gulf Stream to obtain a roofing permit for roofing work, whether new
construction, replacement, repair, or otherwise, including but not limited to:
a.Applications
b.Certifications
c.Affidavits
d.Disclosure Statements
2.All documentation required by Gulf Stream to complete and close a roofing permit for roofing work, whether
new construction, replacement, repair, or otherwise, including but not limited to:
a.Affidavits, certifications, notices or other documentation of compliance with roof deck attachment
and secondary water barrier.
b.Affidavits, certifications, notices, or other documentation of compliance with roof to wall connections.
c.Affidavits, certifications, notices, or other documentation of exemption from roof to wall connections.
d.Any other affidavits, certifications, notices, applications, requests, forms, or other documentation
necessary to close open roofing permits.
3.Any application or documentation necessary to re-open a roofing permit that has expired.
4.Any application to extend the expiration of a permit.
