C. Maximum Grant Amounts
The maximum grant amount is $10,000 for a single family residence.
_ Note: Since a Grant does not get paid back, a lien will not be placed on your home. (The contractor has
� the right, under State Law, to place a lien on a property for non-payment therefore it is
imperative that the homeowner signs the final Inspection Checklist when the job is finished.)
D. Gr�nt Conditions
The applicant(s) will agree to comply with following terms and conditions:
" 1. Applicants are considered on a first come first served basis. Only complete applications will be
considered for approval, and only if grant funds are available. Application documents must not
� be older than six months.
.
2. The Grants will address Code Violations, Health/Safery Issues, and visible eaterior
improvements.
- 3. All contractors must submit an application, a copy of an updated pocket_ license, a copy of
Workman's Compensation and a copy of $1,000,000 liability insurance. The City of Lynwood
requires that all selected contractors read and agree to the General conditions. No owner/builders
will be accepted. A minimum of three bids is required. Contractor is chosen by the homeowner.
4. A final inspection with appropriate sign-off from the owner is required before issuing payment.
' S. Applicant(s) may only apply once every five years.
` II: Grant Procedures
A. Application Process
; 1. Application Papers and Documents
� The ConsulYant will require that the applicant fill out, sign, and date the application as well as
sign and date the following documents*:

� Acknowledgment of Lead Based Paint Information Booklet -signed by all applicants
� ► Social Security Statement- needs to be filled out by person(s) receiving Social Security
benefits. -
� Certification of Income Tax Refum Not Required - signed by anyone in the household

The borrower must supply the Consultant with copies of the Pollowing ;
► A recorded Grant Deed (if other person on title with applicant, a letter of residence must
' be provided by each additional owner)
� � ► Property Tax Statement (current year)
, ► Fire Insurance Policy (current year)
► Federal Income Taac Returns along with W-2's (previous year; self-employed previous
2'years) or statement of non-filing
, � Current paycheck stubs, and/or other income documentation (at least two months for
each separate employment of anyone in the household 18 years of age and over)
► Mortgage payment book or mortgage statements for alf mortgages (at least two months
must be provided)
► AFDC, Social Security, pensions, dividends, interest, child or spouse support, rental
income, etc. (two proofs of anyone in the household 18 years of age and over)
� ► Complete (all pages) Bank StatemenCs for all bank accounts (at least two months for each
and every account of anyone in the household 18 years of age and over)
(The time line on this process is up to the applicant, but the application will not be accepted until
it is complete.) * ONLY COPIES WILL BE ACCEPTED. -
B. Bidding and Construction Process
I. InirialInsnection (Approx. 1-2 weeks)
The Agency's Inspector will visit the properCy and identify all deficiencies to be correeted to
bringthepropertyintocompliancewith_localrehabilitationstandards. Theapplicantmustensure
that there is accessability into every room of the house, including the garage. Pictures ofthe work
� and/the property will be taken.
2. Work Write-Up (Approx. 1 week)
The Agency's Inspeetor will review all requests and incorporate them into the original work
write-up. The Tnspeetor will review (and revise if necessary) the work write-up with the
, applicant(s). Applicant(s) shall sign and date the work specification sheet.
3. Bid Process (Approx. 2 weeks)
Bid packages are prepared and given to the homeowner. The City of Lynwood will keep a list
of eligible contractors that homeowner(s) may use. The program does not guarantee nor in any
� way endorse the contractors; the lisY mexely includes contractors who meet program eligibility
requirements. Contractors must submit their bids by date specified to the Housing Consultant.
The Housing Consultant will ensure that all the bids are fair and reasonable. The Loan
Consultant will review the bids with the homeowner(s). The homeowner is responsible for
_ selecring the fmal contractor.
�� 4. Historical Preservation Cleazance (Approx.4-6 weeks)
All homes must be submitted for review by SHPO for clearance. If the property is more than 50
years old, the applicant must notify the Inspector, and additional pictures must be taken of the
surrounding properties and sent in to determine eligibiliTy.

� �� 5. Contract & Notice to Proceed � (Approx. 1 week) �
After clearance and low bidder is selected, Yhe contractor and the applicant sign t11e Contract and
any necessary Change Order. After a Notice to Proceed is issued, the contractor has 14 days to
- contact the Homeowner and set a start date. An Agency sign will be placed on your fronC lawn. '
6. Hold Harmless Clause
The following clause is included in the contract and must be brought to the attention of the
conh and the borrower(s) by the Consultant:
� "Hold Harmless: The parties hereto agree to indemnify, defend, save and hold harmless the City
of Lynwood and their Agents, employees, officers or consultants against any and all personal
injuries, liabilities, costs, damages, ciaims, suites, or expenses, including reasonable attorney's
. fees arising out of any act omission of any officer, agent, or employee of the Contractor or
- resulting from the condition of any property or materials owed or controlled by the contractor
in the performance of this Agreement. It is understood that the employees of the contractor are
not agents or employees of the City of Lynwood".
6. Period of Rehabilitation
Time allowed for the rehabilitation process is as follows: •

Exceptions: Tnneofyearandnatureofrehabilitationworkshouldbetakenintoconsideration.
C. Change orders
The homeowner will refrain from making any "side deals" with the contractor while the work is in
progress. Any change arders, whether additions, deletions, or substitutions, must be written up and
, approved by Redevelopment Agency/Housing Division before any changes to the scope of work may
be permitted. Change orders will be kept to a minimum and to emergencies only without going over the
allowed maximum. Additional funds may be contributed by Homeowner for work that exceeds the limit.
D. Resolving Conflicts
The homeowner is responsible for resolving conflicts that arise during the performance of the
rehabilitation. Only after the homeowner and contractor have made a good faith effort to resolve their
differences will the Redevelopment Agency/Housing Division step in. The Redevelopment
Agency/Housing Division wifl review all the facts carefully and objectively, and all parties must agree •
to abide by the fmdings or pursue formal mediation.
E. Fina1 Inspecfion/Project Completion
The contractor will request the final inspection only after the homeowner has agreed that the
. rehabilitation work has been completed in accordance with the terms of the work specification. When
fin<21 inspection is scheduled, owner must sign lnspection Checklist and Change order (if applicable).
Pictures of the work performed will be taken and kept on file. The contractor will provide the
Redevelopment Agency/Housing Division with an originai invoice, a copy oFthe final sign-off from the
Building Department (if applicable), a signed change order (if applicable), and a lien release in order to
ensure payment.
