TO PERFORM STRUCTURAL ANALYSIS AND DESIGN SERVICES F'OR WEST MASHTA DRIVE BRIDGE REMEDIAL REPAIRS; PROVIDING FOR AUTHORIZATION; AND PROVIDING FOR AN EFFECTIVE DATE. \ilHEREAS, the Village of Key Biscayne (the "Village") issued Request for Qualifications No. 2016-02-09 (the "RFQ") for continuing professional engineering services; and WHEREAS, pursuant to the RFQ, the Village Council selected the Corradino Group, Inc. (the 'oConsultant") as one of the consultants to provide continuing professional services relating to general civil engineering, transportation planning and engineering, environmental engineering, and landscape architecture, and authorized the Village Manager to execute an agreement with Consultant; and \ilHEREAS, on October 19, 2018, the Village Council approved the issuance of a Work Order to Consultant to perform Structural Analysis and Design Services for West Mashta Drive Bridge Remedial Repairs (the "Project"); and WHEREAS' an underwater inspection of the V/est Mashta Drive Bridge needs to be performed in order for Consultant to complete the Project; and WHEREAS, the Consultant has provided a proposal, attached hereto as Exhibit "4," (the "Change Order") for the underwater inspection; and WHEREAS, the Village Council desires to approve the Change Order; and WHEREAS, the Village Council finds that this Resolution is in the best interest and welfare of the residents of the Village. NOW, THEREFORE, BE IT RESOLVED BY THE VILLAGE COUNCIL OF' THE VILLAGE OF KEY BISCAYNE, FLORIDA, AS FOLLOWS: Section 1. Recitals Adopted. That each of the above-stated recitals are hereby adopted, confirmed, and incorporated herein. Section 2.Chanse Order Approved. That the V illage Council hereby approves the Change Order attached hereto as Exhibit'0A." Section 3. Authorization. That the Village Manager is hereby authorized to execute the Change Order attached hereto as Exhibit o'A" and take any and all action necessary to implement the purposes of this Resolution. SCOPE OF SERVICES The Village of Key Biscayne requires the following additional scope of services for the above- mentioned structure which include. a Conduct underwater inspection services (see attached scope by lndustrial Divers Corporation "Exhibit A") SCHEDULE The scope of work is anticipated to be completed within three (3) weeks from your notice to proceed. COMPENSATION CORRADINO agrees to provide the scope of services above for the lump sum compensation of $ $ 12,874.24, based on a Lump Sum contract. This proposal includesthe servicesfor lndustrial Divers Corporation for unden¡vater inspection services (see attached scope). Additional work not anticipated in this proposalwill be billed as additionalservices as a negotiated lump sum. Written authorization will be required prior to the commencement of additional services.
