Additional Construction Management Services for
3140 Polo Drive Drainage Outfall

Additional services are needed to continue site observations and submittal reviews associated with the
construction of the 3140 Polo Drive Drainage Outfall Pipe which has taken longer than expected.
Construction was originally estimated to be complete within three weeks. At four weeks, it is estimated
that less than half of the work has been completed with the more sensitive aspects of the project still
remaining (road and seawall work). As a result, the not -to -exceed amount of $5,000 will soon be reached.
Based on the contractor's performance and the work still needed, it is recommended that the not -to -
exceed amount be increased by $5,000 for a total of $10,000. The services and assumptions will be
consistent with the contract signed July 21, 2018 and performed at the hourly rates set in that agreement.
IN WITNESS WHEREOF, the parties have made and executed this Agreement on the respective dates under
each signature. TOWN OF GULF STREAM, through an authorized official of the Town and Mathews
Consulting, a Baxter & Woodman Company signing by and through Rebecca Travis (Vice President) duly
authorized to execute same.
