THIS ADDENDUM, (Number 1 to Contract No. 163-2013) is made and entered into
this 10'"' day of —Maq , 2016, by and between the City of Richmond, Indiana, a
municipal corporation acting by and through its Board of Parks and Recreation, with its office at
50 North 51h Street, Richmond, Indiana, 47374 (hereinafter referred to as "City"), and Cintas
Corporation, 601 E. Main Street, Muncie, Indiana, 47305 (hereinafter referred to as
"Contractor").
WHEREAS, City and Contractor previously entered into Contract No. 163-2013 on December
2, 2013, which contract was for services in connection with the bi-weekly
cleaning and replacement of uniforms, uniform jackets, shop towels, and office
mats for the Richmond Parks Department; and
WHEREAS, The parties wish to amend said Agreement to include 4 x 6 brown mats for the
Senior Center for the Parks Department as set forth within the scope of Contract
No.163-2013.
NOW, THEREFORE, in consideration of the mutual promises and covenants herein contained,
including the above stated recitals, City and Contractor hereby agree as follows:
1. The City and Contractor executed and entered into Contract No. 163-2013, on
December 2, 2013. Pursuant to Contract No. 163-2013, Section XI
(Miscellaneous), the Agreement may be altered or amended in whole or in part at
any time by filing with the Agreement a written instrument setting forth such
changes signed by both parties. The parties agree that Contractor shall be retained
to also provide cleaning and replacement services for the 4 x 6 brown mats for the
Senior Center for the Parks Department, with said services to be within the scope
of Contract No. 163-2013 and as identified in Exhibit A to this Addendum. Such
services shall be performed in a manner consistent with Contract No. 163-2013,
which is hereby incorporated by reference and made a part hereof. The contract
amount shall be increased to an amount not to exceed One Thousand Three
Hundred Dollars and Zero Cents ($1,300.00) during the 2016 calendar year.
2. City and Contractor expressly agree that all other terms, conditions, and
covenants contained in Contract No. 163-2013, shall be applicable to the work
performed pursuant to this Addendum, unless such term, condition or covenant
conflicts with this Addendum Number 1, in which case this Addendum Number 1
shall be controlling.

3. Both parties hereby agree that with the exception of the increased contract amount
and the inclusion of the 4 x 6 brown mats for the Senior Center as set forth in
Exhibit A to this Addendum, Contract No. 163-2013 shall remain intact and
unchanged and in full force and effect.
4. Both parties agree that any person executing this Contract in a representative
capacity hereby warrants that he or she has been duly authorized by his or her
principal to execute this Addendum.
