HomeMy Public PortalAboutRES-CC-2001-10Resolution 10-2001 A RESOLUTION AMENDING THE CITY OF MOAB PERSONNEL POLICIES AND PROCEDURES MANUAL SECTIONS 4 13 and 4 14 REGARDING HEALTH AND DENTAL INSURANCE COVERAGE FOR EMPLOYEES WHEREAS, the City of Moab Personnel Policies and Procedures Manual sections 4 13 and 4 14 provide for health and dental health benefit coverage for classified City employees, and WHEREAS, classified City employees who discontinue employment with the City due to an approved disability currently lose their health and dental insurance coverage upon cessation of employment, and WHEREAS, disability insurance payments are typically not sufficient to cover the cost of health and dental insurance NOW, THEREFORE, we, the Governing Body of the City of Moab do amend the City of Moab Personnel Policies and Procedures Manual section 4 13 and 4 14 to read as follows 413 Health Insurance The City pays 100% of the premium on health insurance for classified employees, statutory appointees, contractual employees and their dependents The plan includes major medical provisions The City administrative offices will handle any problems relating to health insurance The City will reimburse exempt status employees an amount equivalent to the deductible charge assessed by the insurance company The employee shall submit documentation to support that a deductible was assessed and then paid by the employee Reimbursements are limited to the deductible 'mutations of the insurance policy in effect The City further limits reimbursements to $100 per year, per employee, or $300 per year, per family No other deductibles are reimbursable Subiect to the availability of funds, the City will pay 100% of the single coverage health insurance premium for classified employees who leave the_employment of the City due to an approved disability (as improved by the City's disabilit insurance carrier) for a period of 22 months following the last date of work or until such time as the employee receives Social Secuny Insurance payments, whichever comes first 4.14 Dental Insurance The City pays 100% of the premium on dental insurance for classified employees, statutory appointees, contractual employees, and their dependents The City administrative offices will handle any problems relating to dental insurance The City will reimburse exempt status employees an amount equivalent to the deductible charge assessed by the insurance company The employee shall submit documentation to Resolution 1t10 2001 Page 1 of 2 support that a deductible was assessed and then paid by the employee Reimbursements are limited to the deductible limitations of the insurance policy in effect Subject to the availability of funds. the City will par 100% of the single coveraee dental insurance premium for classified employees who leave the emolovment of the City due to an approved disability (as approved by the Cuv's disability insurance carried for a nenod of 22 months following the last date of work or until such time as the employee receives Social Security Insurance payments whichever comet first This resolution shall take effect immediately upon passage Passed and adopted by action of the Governing Body of the City of Moab in open session this 22od day of May, 2001 Attest Rachel Ellison City Recorder CITY OF MOAB By j.4e0.› )/ Karla R Hancock Mayor Resoluuoo #10 2001 Page 2 of 2
