Main Street, Crestview, FL 32536 Suite 1. The rental shall commence on 1 September 2016 and shall either continue: a. ■ month to month with no fixed end date or b. ❑ lease ends on 2. TENANT agrees to pay $ 630.00 per month on the fast day of each month, Rent includes items "a - n" PLUS rent also includes Communications Package "o - s" below: a. Fully Furnished Private Office b. Full-time Receptionist to Receive Guests c. Kitchenette with Microwave & Coffee d. Wireless/Wired Internet Access e. 24/7 Secure Access f. 10 Hours/month Conference Room Access g. Network Printer and Fax Access h. Utility and Janitorial Services Included i. Professional Business Address j. TT Computer Support k. Call Answering and Forwarding Services 1. Mail and Packages Received and Held m. Up to 5 Notary Services per month n. Discounts on Copying and Faxing Services o. Dedicated Landline, Number, Extensions, and Fax (w/Communications Package) p. Unlimited Local and National Calls (w/Communications Package) q. Remote Access to Voice Mail (w/Communications Package) r. Personalized Call Answering and Forwarding (w/Communications Package) s. Free rental of PC (w/Communications Package) 3. All rents are due and payable to the LANDLORD on or before the due date of each month. A late charge of 5% of the monthly rental will be charged after the third day if the rent has not been paid. An eviction notice will be issued after the tenth day unless a satisfactory agreement has been made with the LANDLORD. The TENANT will remit the rent to Main Street Suites, 550 N. Main Street Suites, Crestview Florida 32536 utilizing cash, check or credit card. 4. TENANT will pay the LANDLORD to secure the lease. This set-up fee will be paid to Main Street Suites to setup the necessary paperwork and files for the office. 5. The premises are rented for use only as a business office. TENANT will comply with all governing agencies and local zoning laws for TENANT's business. The office is located in the City of Crestview so the TENANT is fully liable for any or all required city or county license(s) to do business. 6. TENANT shall keep the premises rented for their exclusive use in good order and condition and pay for any repairs caused by their negligence of misuse or that of their invites. LANDLORD shall maintain any other parts of the property and pay for repairs not caused by TENANT'S negligence or misuse or that of that of their invitees. MAIN STREET SUITES 7. TENANT shall not disturb, annoy, endanger other tenants of the building or neighbors nor use the premises for any immoral or unlawful purposes, nor violate any law or ordinance, nor commit waste or nuisance upon or about the premises. 8. Either party may terminate the agreement in the event of a violation of any provision of this agreement by the other party. 9. In case of failure of the TENANT to pay the rent herein agreed upon when due or any action by the LANDLORD to enforce any terms of the agreement or recover possession of the premises, and the same is collected by suit, or through an attorney, the TENANT agrees to pay a reasonable attorney's fee, together with all costs and charges. 10. Time is of the essence. The waiver by LANDLORD of any breach shall not be construed to be a continuing waiver of subsequent breach. 11. Termination of this lease must be given in writing on the first of the month prior to termination at the end of that month. 12. Forwarding mail: The post office will not forward mail after termination and the TENANT agrees not to send forwarding card to them. TENANT may pick up any mail delivered after termination. This mail will be held by LANDLORD until picked up and returned to sender after 30 days if not picked up. LANDLORD will never be held liable for any mail problems. 13. The TENANT will also be responsible for any insurance to cover themselves, employees or guest in case of injury on premises. 14. Indemnification — TENANT shall hold LANDLORD harmless from any and all liability actions, claims and damages arising after the commencement of this lease, and which may be imposed upon or incurred by or asserted against LANDLORD by reason of any accident, injury or death of any person or damage to any property occurring on or about the lease premises or any part thereof or any use, non-use or condition of be leased premises or any part of the ownership, occupancy or use thereof. TENANT shall have the right to test the validity of any such claims regardless of who makes the claim, in the name of the LANDLORD, as the TENANT may deem necessary, provided that the expense thereof shall be paid by the TENANT 14. This is a non-smoking building. Smoking is prohibited at any time in the building or on any property owned by LANDLORD. 15. The TENANT will be required to lock and secure the lobby door when entering and leaving after normal work hours (8:00 A.M. to 5:00 P.M. Monday -Friday). AN OUTSIDE OFFICE BUILDING KEY IS PROVIDED AS WELL AS AN INDIVIDUAL OFFICE KEY. PLEASE DO NOT DUPLICATE THE KEYS. 16. Other Notes: a. Rent includes all items as listed in Section 2 (items "a — s"). b. Landlord agrees to waive the one time setup fee for the City of Crestview c.
