TASK ORDER 2
This task order amendment is issued pursuit to the Professional Engineering Services
Continuing Services Contract ("AGREEMENT") deted February 9, 2015, between
CH2M HILL Engineers, Inc. ("CH2M HILL") and City of Crestview, Florida
("CLIENT") for services performed for the CLIENT on a Task Order basis.

the Gil Ava Street Stormwater System Reconstruction
Article A. Purpose:
Introduction
The purpose of this Amendment 1 to Task Order 2 is to amend Article B. Scope of Work,
Article C. Compensation Provisions, and Article D. Period of Service. The compensation
provisions were modified to incorporate additional scope of work, which is provided in
Article B below, and the extension of the period of service. The period of service was
increased due to the extension of the construction schedule for the reconstruction work.
Article B. Scope of Work:
Task 1—Design Services
The additional design services that were not included in the original task order but required
to complete the project are summarized below:
• Preparation of conceptual design and attendance at subsequent workshop
• Development of a stormwater runoff model and calculations for the Gil Ava
drainage basin
• Preparation of a design summary memorandum
• Preparation of three requests for NRCS grant extensions
Task 3 - Services During Construction and Post -Construction Services
The additional services during construction that were not included in the original task order
but required to complete the project are summarized below:
• Twelve additional site visits during construction due to the extension of the
construction schedule and to observe additional milestones and differing site
conditions related to the project.
• Engineering services to modify the plans and specifications to provide additional
stabilization of the side slopes and to capture and direct the runoff from the top of
banks. Negotiations with contractor for a change order related to the modifications.
• Preparation of an Operation and Maintenance Plan and Quality Assurance Plan, as
required by the NRCS.
