CONSULTING SERVICE AUTHORIZATION (CSA) AMENDMENT 1
Service Authorization Number: 2016-1
Project (Location): PJ Adams Utility Relocation Segment 1 & 2 (Crestview, Florida)
This Amendment. when executed, shall be incorporated in and shall become an integral part of CSA 2016-1, dated
2/25/2016. and original Agreement for Consulting Services dated 12/19/2014 between the City of Crestview (CITY) and
Atkins (CONSULTANT).
1. Amended Scope and Tasks:
a. Four additional SUE borings by Cardno ($1,735)
b. Additional survey by Diversified Design & Drafting Services, Inc. ($1.292)
I1. Time of Performance
No additional time added to original Service Authorization.
III. Fees to be paid
Original fees for the project are increased by $3,027, as negotiated for the additional services.
All other provisions and stipulations of the original Consulting Service Authorization remain in effect.

SUBJECT: P.J. Adams Pkwy. Widening Project — Phases 1 & 2
Atkins Engineers Task Order - Amendment
They are asking for more money to do more water and sewer line depth verifications — called
"SUE" for subsurface underground engineering. At first I told them "NO". Then they came back
with a much lower amount. Originally they asked for $6,816.00. They came down to $3,027.00
after looking at what they really needed. The e-mails are attached.
We can take the money from the PWA Utility Fund Engineering Fees — which has $25,500.00
in it still, according to Carlos.
