AGREEMENT NO: 68-4209-15-158 AMENDMENT NO. 3 TO NOTICE OF GRANT AND AGREEMENT BETWEEN UNITED STATES DEPARTMENT OF AGRICULTURE NATURAL RESOURCES CONSERVATION SERVICE AND THE CITY OF CRESTVIEW Purpose: To request a no -cost time extension to the performance period of this agreement by 143 days. Additional time is needed on this project because advertisement of the project was delayed due to the difficulties in securing and formally recording the applicable property easements, which was not accounted for in the original request. The following paragraph of the Statement of Work (Attachment A) for 2014 Panhandle Storm Emergency, is hereby revised: VI. PERIOD OF PERFORMANCE The period of performance for this agreement begins on the date of the last signature of all parties and will end on May 9, 2016. This agreement shall become null and void 180 calendar days after the date NRCS has executed this agreement if a contract has not been awarded or work has not commenced. Any funds not obligated by the Sponsor during the funding period will revert to NRCS upon the expiration or termination of this funding period. All other items remain the same. Page 3 of 3
