Project (Location): Crestview Corners Access Road (Crestview, Florida)
This Service Authorization, when executed, shall be incorporated in and shall become an integral part of
the original Agreement for Consulting Services dated 12/1/2014 between the City of Crestview (CITY)
and Atkins (CONSULTANT).
I. Project Description:
Prepare construction plans for the Crestview Corners Access Road
II. Scope of Services:
Atkins understands that the City will build the project. The construction plans will be tailored to
provide sufficient details necessary to permit the project and construct the project.
The Consultant shall design and prepare construction documents for the proposed creation of a
new access road. The new access road will begin at Hospital Drive and extend north through the
Crestview Corners Shopping Center to East Redstone Avenue. This project will include survey,
geotechnical investigation, drainage, and roadway design. The design will consist of preparing
construction drawings and permitting for the access road. The Northwest Florida Water
Management District (NWFWMD) will require a stormwater permit for these improvements.
Atkins will coordinate with NWFWMD and FDEP to determine the specific requirements for this
project but it is anticipated that the new work will have to meet current permit requirements.
However, it is anticipated that no wetland permit will be required.
Atkins staff will perform field reviews on several separate occasions to analyze the current queue
length during peak times for the left turn lane on Redstone to determine if it will impact the
proposed connector road as traffic exists today. Atkins will prepare a memo with the findings of
the observations.
Atkins staff understands that the Krystal will be responsible for design, permitting and
construction of their tie-in to the proposed connector roadway.
See Attachment "A" for full scope.
III. Items to be furnished by CITY at no expense to the CONSULTANT:
Coordination with different property owners regarding the required permanent easement for the
access road. Permits will require this to be completed.
IV. Time of Performance
The Scope of Services will be completed within 120 days after receipt of Notice to Proceed,
except for delays beyond the reasonable control of the CONSULTANT. The above noted items
of performance are not, however, a warranty or guarantee that the noted services will be
completed within such period.

V. Fees to be paid
For the Scope of Services as described, the CONSULTANT shall be paid a lump -sum fee of
$36,297.68 for professional services, according to the Fee Proposal contained in the attached
proposal letter first referenced above.

Scope of Services
The following ioascope mfservices which dofioeathose tasks necessary lo complete the design of the Crestview
Comers Access Road Project in Crestview, Florida. Herein uDor all references to the City will nncuu Crestview
and all references to the Consultant will mean Atkins.

To accomplish the objectives outlined above inumanner that meets theC'`u requirements, the following scope
items have been identified.
1.0 SURVEY
Rcbo|-Battle and Associates (RBA) will establish horizontal and vertical control, and pick up topographic
features, pipe and drainage structure inverts within the uzouu where now pavement and xkonn water facilities will
he constructed. Additionally, utility locates will he called in and locations surveyed within opeoiOmj areas.
Significant trees, above ground appurtenances, drainage structures, sidewalk, and fencing will bcsurveyed within
the specified urcuy. Gco\echnica| boring locations will be |uou1ed within the project |inni1o. RBA will prepare
legal descriptions for each of the parcels impacted by the proposed roadway which will be maintained by the City
after construction. The easement documentation preparation will be handled by the City. Attached iuR0A`s
detailed scope and fee. RBA`u fee only includes costs to survey the areas where the new pavement will be
constructed and the existing retention facility. ltioassumed that all improvements proposed will occur within the
surveyed areas and that all necessary storm water can be handled iothe area. No additional survey uzcus behind
the shopping center are included. If areas outside of the survey area are required to meet NWFWMD criteria, a
supplemental will be required. All options will be exhausted prior to inclusion of areas outside the areas
described within this scope.
2.0 GEOTECHNICAL
Nova Engineering and Environmental will perform geotechnical exploration for the proposed stormwater pond
modifications. In addition to the stormwater investigation, Nova will perform exploration for the potential
retaining walls that may be required to maximize use of vacant property on the site for stormwater retention.
Nova will prepare a geotechnical report detailing the soil findings and design parameters. Attached is Nova's
detailed scope and fee. It is assumed that all improvements proposed will occur within the surveyed areas in the
vicinity of the proposed access road and that all necessary storm water can be handled in the area. No additional
borings outside the areas behind the shopping center are included. If areas outside of the survey area are required
to meet NWFWMD criteria, a supplemental will be required to perform additional borings. All options will be
exhausted prior to inclusion of areas outside the areas described within this scope.
3.0 ENGINEERING DESIGN
The Consultant shall design and prepare construction documents for the new access road. The following design
items will be incorporated as part of the project's construction plans:
The Consultant will utilize the existing parking lot where possible with only minimal improvements. It is
anticipated that restriping of the parking lot will be required at a minimum. The new sections of roadway will
have I l foot travel lanes. The existing pavement in the parking lot will be utilized for the new access roadway
with only thin overlays possible where new sections of roadway meet the existing pavement. The use of curb and
gutter will be evaluated for the new sections of roadway and may be needed to convey storm water. A drainage
collection system and enhancements to existing stormwater facilities will be required as part of the project. The
existing drainage flow from existing facilities will have to be maintained during and after construction.
The Consultant shall prepare traffic control plans and details for construction activities within the project limits.
The Consultant will coordinate closely with the City and surrounding businesses regarding construction schedule,
impacts, concerns, or potential construction conflicts.
Roadway Design — Review existing roadway features including utilities, clear zone, alignment, profile, and cross
slope, Roadway design will include the horizontal and vertical roadway geometry, establishing roadway
construction limits, and development of the roadway typical section. The Consultant will design a new connection
to Hospital Drive. Plans will be developed sufficiently to define right of way or easements. Additionally, the
Consultant shall provide driveway access for adjacent properties (with the exception of Krystal's) to the new
access road. The Consultant shall prepare roadway cross sections within the new roadway sections only.
Drainage Design — The Consultant will design the proposed drainage system for the proposed roadway extension
and modifications to the existing storm water pond. The drainage design will may curb inlets, ditch bottom inlets,
and manholes. The existing pond will be modified based roadway impacts. Consultant will evaluate the existing
private storm water pond to determine if the pond can be modified to meet current stormwater regulations. The
drainage design will be based on City code and NWFWMD requirements. The stormwater will treat and attenuate
the roadway runoff per City and NWFWMD design standards.
Utility Coordination — The Consultant shall review existing utilities within the project limits and determine if
there are any potential impacts. The Consultant will coordinate with utility agencies regarding impacts, concerns,
and potential construction conflicts.

for the project. The plans may include a key sheet, general notes (including erosion control details), plan sheets,
drainage structure, cross sections, signing and pavement markings, traffic control, and miscellaneous detail sheets.
The construction plans shall be submitted lutwo phases (hO%and Final). Two plan review meetings shall be held
with the City. The first meeting shall heafter the 60Y6plans review and the second meeting ubu|| boafter the
review of Final plans. The Consultant will reference City and FDUT specifications. Signing and Pavement
Markings — The Consultant will prepare signing and pavement marking plans in accordance with MOTCD.
FDOTund City standards. The sign posts will be in compliance with special City guidelines. Any existing signs
nrstriping that are in conflict orimpacted by construction activities will be removed and/or relocated as needed.

4.0 PERMITTING
The proposed roadway will require uotnrnwatcr ocrniL If possible, the existing private storm water ponds will
be designed to treat and attenuate the stormwaterrunoff based on the City and Northwest Florida Water
Management District ONWFWMD\regulations. The new section ofroadway near Redstone may behandled using
compensatory treatment in the proposed enhancements to the private storm water facility. The Consultant uhu\|
attend upre-application meeting with tbcNWPWMD to determine the exact requirements. An Environmental
Resource Permit will be prepared and submitted tothe NVYFVVMD. Permits ahu|| be prepared and submitted.

paid by the City.
Expenses: Atkins has included u lump sum expenses item that includes miscellaneous cxpunuux including
copying, printing,mJlemuc etc.
5.0 ADDITIONAL SERVICES
Services authorized by the City other than those specifically listed above shall beconsidered additional services
and The Consultant shall be compensated as described in the Fee Summary. Additional services may include, but
are not limited to the following:

any concems via phone and in visits on other projects in the area as necessary.
3. Signal warrant study for Hospital Drive and 8R 05. Signal warrant would require traffic counts on all
roadways connecting to the proposed mignui If the signal is not warranted and the City of Crestview still
desires k/purse the signal, all proposed development information would need to he obtained and traffic
projected in order to justify the new mi8ou\. /\ meeting with FDC)T would be required to ensure they
will allow the traffic to he projected based on potential median opening changes that could occur with
the signal addition.
4. Making revisions to drawings, specifications, or other documents when such revisions are inconsistent
with written approvals orinstructions previously given and are due 0ocircumstances beyond the control
ofthe Consultant.
5. Providing services of professional consultants other than as is specifically provided for under this scope
of services.
6. Preparing supporting data and other services in connection with agency approvals if extensive studies
and/or analysis are required beyond that which is incidental to this scope of services.
7. Any changes or modifications required due to changes in the project directed by the City.
8. Preparing to serve or serving as an expert witness in connection with any public hearing, arbitration, or
legal proceeding.
9. Providing assistance or preparation of additional documentation required for legal transactions, including
sale of property or preparation of lease agreements.
10. Design does not include any lighting, landscaping, hardscape, or signalization.

supplemental efforts will be required. The left turn lane traffic will have to be projected A supplemental
amendment will be required to obtain new traffic counts and all proposed development information will
have to be collected and the future traffic projected for the intersection. A SYNCHRO model depicting
the area would need to be created.
12. If the existing private storm water facility cannot be modified to handle the NWFWMD requirements
and areas outside the project areas such as modifications to the storm water facilities behind the existing
shopping center are required, supplemental survey, geotechnical and design efforts will be required.
6.0 CLIENT RESPONSIBILITIES
I. Designate in writing a person to act as the City representative with respect to the work to be performed
under this agreement. Such person shall have complete authority to transmit instructions, receive
information, interpret and define policies and make decisions.
2. The City shall pay all filing and permit fees, printing costs for approvals, permits, bid proposals,
advertising, and construction documents.
The City shall provide the Consultant with all available site data. This may include survey data, CADD
files, GIS data, utility markups, geotechnical information and/or field notes.
4. The City shall coordinate with the different owners of the properties regarding utility conflicts and
relocations and obtain an easement for future maintenance of the facility.
5. Pavement design including base required for the project since the City forces will be building project.
