THIS ADDE UM, (Number 1 to Contract No. 23-2016) is made and entered into this
day of 2016, by and between Richmond, Indiana, a municipal
corporation acting through its Board of Public Works and Safety with its office at 50 North 5t' Street,
Richmond, Indiana, 47374 (hereinafter referred to as "City"), and Rick L. McAvene DBA River
Valley Surveying, 3602 Woods Drive, Richmond, Indiana, 47374 (hereinafter referred to as
"Contractor").
WHEREAS, City and Contractor entered into Contract No. 23-2016 on March 17, 2016, which
Agreement retains Contractor to provide services in connection with land surveying
services and assistance to the Department of Public Works and Engineering; and
WHEREAS, The parties to said Agreement wish to amend said Agreement in order that Contractor
will be authorized to provide land surveying services to the Department of
Metropolitan Development and in order that the services will be conducted under the
same terms, conditions, and rates as previously set forth in Contract No. 23-2016.
NOW, THEREFORE, in consideration of the mutual promises and covenants herein contained,
including the above stated recitals, City and Contractor hereby agree as follows:
1. The City and Contractor executed and entered into Contract No. 23-2016, on March
17, 2016. The parties wish to amend said Agreement by filing this written instrument
setting forth the changes to said Agreement. As such, said Agreement shall be
amended to reflect that Contractor shall provide land surveying services to the
Department of Metropolitan Development and, in such capacity, Contractor shall be
supervised by, and report to, City Planner, Sarah Mitchell.
2. Contractor shall perform said services for the Department of Metropolitan
Development under the same quotes rates described in said Agreement, and City shall
pay Contractor the same quotes rates for the land surveying services performed by
Contractor for the Department of Metropolitan Development. Said services shall be
performed in a manner consistent with Contract No. 23-2016. Contractor shall be
permitted to utilize the same support services and facilities located in the Department
of Public Works and Engineering as set forth in Contract No. 23-2016.
3. City and Contractor expressly agree that all other terms, conditions, and covenants
contained in Contract No. 23-2016, shall be applicable to the work performed
pursuant to this Addendum, unless such term, condition or covenant conflicts with
this Addendum Number 1, in which case this Addendum Number 1 shall be
controlling.

4. Both parties hereby agree that with the exception of the changes of the provisions of
services being provided to the Department of Metropolitan Development and the
supervision and reporting changes, Contract No. 23-2016 shall remain intact and
unchanged and in full force and effect.
Both parties agree that any person executing this Contract in a representative capacity
hereby warrants that he or she has been duly authorized by his or her principal to
execute this Addendum.
