THIS ADDE SUM, Number 2 to Contract No. 64-2016) is made and entered into this
day of ,�, 2016, by and between Richmond, Indiana, a municipal
corporation acting throu h its Board of Public Works and Safety with its office at 50 North 5`h Street,
Richmond, Indiana, 47374 (hereinafter referred to as "City"), and Robert Donathan D/B/A Donathan's
Inspections and Removal Services, 3504 North Linden Street, Muncie, Indiana, 47304 (hereinafter
referred to as "Contractor").
WHEREAS, City and Contractor entered into Contract No. 64-2016 on June 30, 2016, which
Agreement retains Contractor to provide services in connection with asbestos
abatement for two (2) properties for the City's Blight Elimination Program (`BEP"
per Indiana Housing and Community Development ("IHCDA") Guidelines; and
WHEREAS, City and Contractor entered into Addendum No. 1 to said Contract No. 64-2016 on
July 21, 2016, for one (1) additional property; and
WHEREAS, The parties to said Agreement wish to add an additional property (bid out as Round 2,
Group C) located at 818 North H Street, Richmond, Indiana, to said Agreement under
the same terms and conditions as set forth in Contract No. 64-2016.
NOW, THEREFORE, in consideration of the mutual promises and covenants herein contained,
including the above stated recitals, City and Contractor hereby agree as follows:
1. The City and Contractor executed and entered into Contract No. 64-2016, on June
30, 2016 and Addendum No. 1 to said Agreement on July 21, 2016. Pursuant to
Contract No. 64-2016, Section XII Miscellaneous, the Agreement may be altered or
amended in whole or in part at any time by filing with the Agreement a written
instrument setting forth such changes signed by both parties. As set forth above and
as set forth on Contractor's quote, attached as Exhibit A to this Addendum, services
in connection with the abatement of the additional property located at 818 North H
Street, Richmond, Indiana (bid out as Round 2, Group C), shall be provided as set
forth within Contract No. 64-2016. Such services shall be performed in a manner
consistent with Contract No. 64-2016.
2. Section III Compensation of said Agreement shall be altered in order that City shall
pay Contractor an additional amount of Four Thousand Hundred Dollars and Zero
Cents ($4,000.00) for the additional services, with said final contract amount to be
increased to a total amount not to exceed Fourteen Thousand Two Hundred Fifty
Dollars and Zero Cents ($14,250.00).
3. City and Contractor expressly agree that all other terms, conditions, and covenants
contained in Contract No. 64-2016, shall be applicable to the work performed
pursuant to this Addendum, unless such term, condition or covenant conflicts with
this Addendum Number 2, in which case this Addendum Number 2 shall be
controlling.

4. Both parties hereby agree that with the exception of the additional property abatement
services to be provided and increased contract amount, Contract No. 64-2016 shall
remain intact and unchanged and in full force and effect.
5. Both parties agree that any person executing this Contract in a representative capacity
hereby warrants that he or she has been duly authorized by his or her principal to
execute this Addendum.
