ADDENDUM
THIS ADDENDUM, (Number 2 to Contract No. 8-2015) is made and entered into this
day of G' i�r_, 201& , by and between Richmond, Indiana, a municipal
corporation acting through its Board of Public Works and Safety with its office at 50 North 51n
Street, Richmond, Indiana, 47374 (hereinafter referred to as "City"), and Pedro Feliciano, d/b/a
Preventive Maintenance Services, 129 South 5 h Street, Richmond, Indiana, 47374 (hereinafter
referred to as "Contractor").
WHEREAS, City and Contractor entered into Contract No. 8-2015 on January 28, 2015, and
Addendum No. 1 to said Agreement on November 12, 2015, which Agreement
retains Contractor to provide services in connection with the general maintenance
and repair of all Rose View Transit vans and buses, which repair and maintenance
includes, but is not limited to oil changes, brake repairs, A/C charges, tires, etc.;
and
WHEREAS, The City wishes to continue to retain contractor to continue to provide said
services for the 2017 calendar year, under the same terms and conditions as set
forth in Contract No. 8-2015.
NOW, THEREFORE, in consideration of the mutual promises and covenants herein contained,
including the above stated recitals, City and Contractor hereby agree as follows:
The City and Contractor executed and entered into Contract No. 8-2015, on
January 28, 2015, and Addendum No. 1 to said Agreement on November 12,
2015. Pursuant to Contract No. 8-2015, Section IV Term of Agreement, the City
has an option to renew the Agreement for a third calendar year with the same
terms and conditions. Contractor shall continue to provide the services set forth
and identified within Contract No. 8-2015 at the same hourly rate, as set forth in
Contractor's quote attached as Exhibit A, for the 2017 calendar year. Such
services shall be performed in a manner consistent with Contract No. 8-2015.
2. City and Contractor expressly agree that all other terms, conditions, and
covenants contained in Contract No. 8-2015, shall be applicable to the work
performed pursuant to this Addendum, unless such term, condition or covenant
conflicts with this Addendum Number 2, in which case this Addendum Number 2
shall be controlling.
3. Both parties hereby agree that with the exception of the extension of the contract
for the 2017 calendar year, Contract No. 8-2015 shall remain intact and
unchanged and in full force and effect.

4. Both parties agree that any person executing this Contract in a representative
capacity hereby warrants that he or she has been duly authorized by his or her
principal to execute this Addendum.
