THIS ADDENDUM, Number 2 to Contract No. 47-2015) is made and entered into this
day of , 2017, by and between Richmond, Indiana, a municipal
corporation acting throw its Board of Public Works and Safety with its office at 50 North 51' Street,
Richmond, Indiana, 47374 (hereinafter referred to as "City"), and Heartland Environmental
Associates, Inc., 3410 Mishawaka Avenue, South Bend, Indiana, 46615 (hereinafter referred to as
"Contractor").
WHEREAS, City and Contractor entered into Contract No. 47-2015 on April 24, 2015, which
Agreement retains Contractor to provide professional services in connection with
asbestos inspection and remediation plan for up to eighty (80) structures for the
City's Blight Elimination Program (`BEP") per Indiana Housing and Community
Development ("IHCDA") Guidelines at the rate of Two Hundred Forty Dollars and
Zero Cents ($240.00) per property ordered by the City; and
WHEREAS, City and Contractor entered into Addendum No. 1 to said Agreement on August 6,
2015, adding an additional eighty (80) structures to the Project, at the same rate, in
accordance with the scope of said Agreement; and
WHEREAS, The parties to said Agreement wish to add an additional twenty (20) properties (for a
total number of "up to" two hundred (200) properties) in connection with said
Agreement under the same rate, terms, and conditions as set forth in Contract No.
47-2015.
NOW, THEREFORE, in consideration of the mutual promises and covenants herein contained,
including the above stated recitals, City and Contractor hereby agree as follows:
1. The City and Contractor executed and entered into Contract No. 47-2015, on April
24, 2015, and entered into Addendum No. I to said Agreement on August 6, 2015.
Pursuant to Contract No. 47-2015, Section XII Miscellaneous, the Agreement may
be altered or amended in whole or in part at any time by filing with the Agreement a
written instrument setting forth such changes signed by both parties. As set forth
above, the parties agree to add an additional twenty (20) structures or properties (for
a total number of "up to" two hundred (200) properties) to be included within
professional services of asbestos inspection and remediation plan in connection with
said Agreement under the same rate, terms, and conditions as set forth in Contract
No. 47-2015. Such services shall be performed in a manner consistent with Contract
No. 47-2015.
2. City and Contractor expressly agree that all other rates, terms, conditions, and
covenants contained in Contract No. 47-2015, shall be applicable to the work
performed pursuant to this Addendum, unless such rate, term, condition or covenant
conflicts with this Addendum Number 2, in which case this Addendum Number 2
shall be controlling.
3. Both parties hereby agree that with the exception of the addition of the above
described additional properties, Contract No. 47-2015 shall remain intact and
unchanged and in full force and effect.

4. Both parties agree that any person executing this Contract in a representative capacity
hereby warrants that he or she has been duly authorized by his or her principal to
execute this Addendum.
