ADMINISTRATIVE SERVICES AGREEMENT
ADDENDUM "A"
The ADMINISTRATIVE SERVICES AGREEMENT (Workers'
Compensation) between: JWF Specialty Company (the "Administrator")
and City of Richmond (the "Client") has been changed as follows:
Section 2. Term of Agreement.
This Agreement shall commence May 01, 2017, and shall remain in full force
and effect until May 01, 2019, or until earlier terminated as hereinafter
provided.
Section 7. Fees
The Client shall pay the Administrator an Annual Flat Fee (the
"claim service fee") of $21,000 (Twenty One Thousand dollars) for the
Administrator's services provided during each year of this Agreement. Each
year of this Agreement is from May 01, to May 01, of the following year.
The total compensation due the Administrator shall be based on a claims
service fee provided, and covers claims administration for the life of the
claim.
The amount of the Annual Claims Service fee shall continue on an
Addendum No. 1 to Contract No. 136-2015
annual basis under the same terms and conditions.
All other terms will remain unchanged.
