GRSM & First Bank Richmond are proposing a new roof installation and would like to block off
the alley east of the loan center (North East Corner of 9th & N A) between 02/19/2018 and
03/12/2018. They will only need it blocked during the day and then can move their equipment at
night to the parking lot.

• CONTRACT NO. 29-2018 — MURPHY ELEVATOR
To perform elevator maintenance of all the elevators located at the City Building, City parking
garage, the Richmond Sanitary Intermediate Pump Building and the Richmond Sanitary Laboratory
Building. This Contract is for four years from March 1, 2018 through February 28, 2022.
The monthly cost will be $701.20 a month for the first two years and will be subject to an increase
or decrease per the price adjustment provision contained in the maintenance agreement based
upon the index of the wholesale commodity prices.

To provide services for certain reporting requirements in accordance with the Federal Affordable
Care Act (ACA) that must be submitted to the Internal Revenue Service (IRS), including
preparations and electronic submission of information to the IRS about the health plan coverage
offered by the City.
Not to exceed $5,440.00
• CONTRACT NO. 31-2018 — GEORGE'S FAMILY PHARMACY OF BATESVILLE
They provide Durable Medical Equipment at no cost to the employees that are on the City's Health
Insurance, including free delivery.
The pricing is the same as 2017

This addendum is to amend the expiration date from January 31, 2018 to read January 31, 2019.

RESOLUTIONS
• RESOLUTION NO. 4-2018 — TO APPROVE THE TRANSFER OF A PERMANENT RIGHT-OF-
WAY UTILITY EASEMENT
This resolution for the transfer by the City of Richmond Indiana real estate located at 213 NW 13tn
Street as well as nearby property, public right-of-way areas located adjacent to the bridge known
as Bridge No. 711 over Clear Creek on Peacock Road to Indiana American Water for Utility
Easements
The City will be compensated in the amount of $462.66 for 0.043 acres of land.
