THIS ADDENDUM, (Number 1 to Contract No. 22-2014) is made and entered into this
a day of t rt& , 2018, by and between Richmond, Indiana, a municipal
corporation acting through its 14oard of Aviation Commissioners with its office at 50 North 5`h Street,
Richmond, Indiana, 47374 (hereinafter referred to as "City"), and Indiana University Health, Inc.,
d/b/a Lifeline Services, 340 West 10`h Street, Indianapolis, Indiana, 46202 (hereinafter referred to as
"Contractor").
WHEREAS, City and Contractor entered into Contract No. 22-2014 on February 12, 2014, which
Agreement is a Lease Agreement of the hangar commonly known as the Dana
airplane hangar located at 5169 South State Road 227 (Richmond Municipal Airport
location), in Richmond, Indiana, with Contractor as the Lessee and the City as a
Lessor of said property; and
WHEREAS, The parties wish to extend the Lease Agreement to and including February 12, 2019,
as set forth within the scope of Contract No. 22-2014.
NOW, THEREFORE, in consideration of the mutual promises and covenants herein contained,
including the above stated recitals, City and Contractor hereby agree as follows:
The City and Contractor executed and entered into Contract No. 22-2014, on
February 12, 2014. Contractor submitted its formal written request for a one-year
renewal on January 4, 2018, which request is attached hereto as Exhibit A. Pursuant
to Contract No. 22-2014, Section 5.01 (Renewal Privileges), the Lease may be
renewed for a one (1) year period and pursuant to Section 16.01 (Modification of
Agreement) any modification of the Lease shall be in writing and signed by all parties
in order for said modification to become effective. The parties wish to renew the
Lease for an additional year and the parties wish to maintain the same terms and
conditions of said Lease. The parties understand, acknowledge, and agree that
pursuant to Section 4.01 b.) (Amortized Monthly Hangar Improvement Payments),
the monthly Hangar Improvement Payments shall cease pending fulfillment of the
original $80,000 balance pursuant to said Lease Agreement
City and Contractor expressly agree that all other terms, conditions, and covenants
contained in Contract No. 22-2014, shall be applicable to this Addendum, unless
such term, condition or covenant conflicts with this Addendum Number 1, in which
case this Addendum Number 1 shall be controlling.
Both parties hereby agree that with the exception of the extension of the Lease
Agreement to and including February 12, 2019, Contract No. 22-2014 shall remain
intact and unchanged and in full force and effect.

Both parties agree that any person executing this Contract in a representative capacity
hereby warrants that he or she has been duly authorized by his or her principal to
execute this Addendum.
IN WITNESS WHEREOF, this Addendum is executed and approved by the undersigned
