Whereas, the Standard Specifications for this contract provides for such work to be performed, the following change is recommended.
General or Standard Change Order Explanation
On May 3. 2018 a storm structure meeting was held. Per the preference or Elijah Welch RSD to see all storm water connections tied directly into
the sides of the new inlet structures. Milestone was directed to tie all storm water connections directly into the sides of the new inlet structures
Existing pipe connections are to be made to the proposed structures without any skew. Elijah Welch RSD wanted to change the current design to
change manholes large enough to avoid any skews. Milestone submitted a cost breakdown summary for the new structures. United reviewed the
breakdown and compared the overall cost with the Site Manager average unit price list. Due to limited use of the item over the past couple years
there are only a couple available. considering the limited data and limited use of the item in the recent past, United would recommend accepting
the cost submitted by Milestone be based on their cost breakdown per unit, rather than the INDOT average unit price. PM HT Phan has been
notified of this change order. See Attachments in Header Tab for details. No time modification or additional workdays were requested by
Milestone as part of this change order. Please rind attached to this change order the following supporting documentation Site Manager Average
unit price list and Milestones strucutre costs.
