To provide pest control services for various City buildings and facilities from January 1, 2019 to
December 31, 2020
Not to exceed $4,540.00 per year.

To provide professional surveying services for the City in connection with a tract owned by the
City through Richmond Power & Light.

To provide floor cleaning and maintenance services for the City Building located at 50 North 5tn
Street, Richmond, Indiana
The cost is $1,328.70 per month for floor maintenance only and $2,400.00 to clean carpet in any
needed areas of the City Building once per year and spot clean as needed

To furnish and install two new dump truck beds on existing Street Department truck hoists for the
Richmond Street Department

• CONTRACT NO. 3-2019 — OVERHEAD DOOR COMPANY OF RICHMOND
To provide on -call and as -needed general and preventative maintenance repairs to the Richmond
Street Department Garage Doors for the 2019 calendar year
Regular Hours $65.00 per man hour and for Overtime Hours $95.00 per man hour

To provide bi-weekly cleaning services for shop -mats, scraper mats, center pull towels, and red
shop towels for the 2019 calendar year for the Richmond Street Department
Not to exceed $1,766.44
• CONTRACT NO. 5-2019 — D&D JANITORIAL SERVICE OF RICHMOND, INC
To provide janitorial and cleaning services of the offices located at the Richmond Street
Department for the 2019 calendar year to be performed two times per week.
Not to exceed $4,680.00

To provide necessary maintenance for the heating and air conditioning (HVAC) systems located
at the Richmond Street Department for the 2019 calendar year.
Regular hours $80.00 per man hour, Overtime hours $125.00 per man hour and 1.75% parts
mark up.

This is a Lease Agreement and this addendum extends the termination date from July 15, 2018 to
