An hourly rate of $30.00 an hour not to exceed $10,000.00 50 North 5tn Street — Richmond, IN 47374 phone (765) 983-7232 — fax (765) 983-7212 kchasteen@richmondindiana.gov CHANGE ORDERS ADDENDUMS ADDENDUM NO. 1 TO CONTRACT NO 15-2013 — RICHMOND CITYH EMPLOYEES FEDERAL CREDIT UNION To adjust the monthly payment to $800.00 a month for twelve (12) months for the past due rent payable on the 15th of each month in the calendar year 2019 and changing the contact information to Adam Tidrow, General Manager • ADDENDUM NO. 1 TO CONTRACT NO. 21-2018 — CUMMINS ELECTRIC, IN To continue to provide on -call and as -needed general maintenance and electric repairs for the Richmond Fire Department under the same terms and conditions as the original contract. • ADDENDUM NO. 1 TO CONTRACT NO. 25-2018 — OVERHEAD DOOR COMPANY OF RICHMOND To continue to provide on -call and as -needed general maintenance to the overhead doors and openers located at the Fire Stations 1,2,3,4,5 & 6 for the Richmond Fire Department under the same terms and conditions as the original contract • ADDENDUM NO. 2 TO CONTRACT NO. 124-2016 — REYNOLDS PLUMBING, INC To continue to provide general maintenance and plumbing repairs for the Richmond Fire Department under the same terms and conditions as the original contract. • ADDENDUM NO. 2 TO CONTRACT NO. 140-2016 — PHOENIX SAFETY OUTFITTERS, LLC To continue to provide services in connection with the cleaning, repair, and maintenance of turnout gear for the Richmond Fire Department in the amount of $27,972.00 • ADDENDUM NO.2 TO CONTRACT NO. 154-2016 — TESTING FOR PUBLIC SAFETY, LLC DBA THE INSTITUTE FOR PUBLIC SAFETY PERSONNEL To continue to provide consulting and testing services in connection with the firefighter application and hiring process for the Richmond Fire Department under the same terms and conditions as the original contract. • ADDENDUM NO. 2 TO CONTRACT NO. 164-2016 — BENJAMIN SIMMONS To continue to provide specialized instruction services and assistance to the Richmond Fire Department in connection with classroom training and instruction to civilians for Emergency Medical Technician (EMT), Cardiopulmonary Resuscitation (CPR), Pediatric Advanced Like Support (PALS) and Advanced Cardiac Life Support (CPR) training under the same terms and conditions as the original contract • ADDENDUM NO. 4 TO CONTRACT— PEDRO FELICIANO To increase the hourly rate for service to all fire vehicles and apparatus from $46.00 per hour to $47.00 per hour for the Richmond Fire Department • ADDENDUM NO. 1 TO CONTRACT NO. 106-2018 — SARAH MITCHELL To extend the contract for one year until December 31, 2019 50 North 5tn Street — Richmond, IN 47374 phone (765) 983-7232 — fax (765) 983-7212 kchasteen@richmondindiana.gov • ADDENDUM NO. 1 TO CONTRACT NO. 122-2018 — THE PUBLIC GROUP, LLC They provide the use of special internet auctioning services for the City of Richmond.
