1. Incorporated District OfNce Lease Attachment. Lessor g=dlord) and Lessee

Lease Atfechtment:("Attselnneat') is incorporrntcd into and made pad of the Lease ("Lease's an:L
if applicable, District Office Lem Amendment (" Amendment') to which is is attached.
Z. Petforrnttnee. Lessor expmzssly acknowledges that ne$her the U.S. House of Representatives
(the `7iottse") nor its Offaeen are liable for the performuwcc of the Lease. Lessor furthcr
expressly acknowledges that payments made by the C i iefAdministrative Officer of the House
(the "CAO") to Lessor to satisfy Lessee's tent obligations under the Lease.- which payments are

3. Modkfta Bona. Any mniandrrarnt to the Lease must be in writing and signed by the i.mw and

the CAO ("Administrau've Counsel") mustreview and give approval ofau'y amendment to the
Lease prior to its execution.

acknowledge that the Lease shell not be valid, and the CAO will not authorize am disbarscment

S. Payments. The Lensc is a fixed term lease with monthly instelltoents for which payemt is due

agrees to contact the OMae of Finaace of the House at 202-225-1474 to attempt to resolve the
dispute before cotrtactiog XAMM.
6., Void Psovidans. Any provision in the Lease purporting to require the payment of a socurity
deposit shalt have no force or efficL t-arthe rmore, any provision in the Lesmse putpottigg to vary
the dollar amount of the not spocuw in the Lease by any cost of living clause, operating
WPM cksuse, pro rate exgenae clause, esaihteion clause, or any otheradjustment or mcwuru
during the carat of the Lease shall have no force or erwt.
7. Certain Charges- The parties agree,that any charge for default, early termination or cancellation
of the Lease which results f om actions taken by or on behalf of the Lessee shall be the sole
responsibility of the L.esseq errd shall not be paid by the CAO on behalf of the Lessee.

Death, Resignation or Removal. Tn the avemt Lessee dies, resigns or is removed fiom offiec
during the term of the lease, the Cleric of the House may, at his or her sole option, either. (a)

terminate the Lease by giving thirty (3o) days' prior written notice to Lessor. or (b) assume the
obligation of the Lem and continue to occupy the premises for a period not to exceed sixty (60)
days following the certification of the election of the Lessce's successor. In the event the Clerk
elects to terminate the Lease, the commencement date of such thirty (30) day termination notice
shall be the date such notice is delivered to the Lessor or, if mailed, the date on which such notice
is postmarked.
9. Term, The term of the Lease may not exceed the constitutional term of the Congress to ,which
the Lessee has been elected. The Lease may be signed by the Member -Elect before taking office.
Should the Member -Elect not take office to serve as a Member of the I 16th Congress, the ;:east
will be considered mill and void.
10. Early Ttrmination. If either i=or or Lessee terminates the Lease udder the terms of the Lease,
the terrni rang party agrees to promptly file a copy o f any termination notice with the Office of
lmimics, U.S. House ofRepresentatives, O'Neill Federal Building, Suite 3100, Attn: Office of
Financial Counseling, Washington, D.C. 20515. and with the Administrative Counsel by c-mtail at
leaws@cail.hotuc.gov.
11. Aavlgnments. Lessor shall not have the right to assign (by operation of law or otherwise) any of
its rights, interests and obligations under the Lease, in whole or in part, without providing thirty
(30) days prior written notice to Lessee, and any such p mp rted assignment without such notice
Shall be void. Lessor shah promptly file a copy of any such assignment notice with the
Administrative Counsel by e-mail at Ieascs@rmil.house.gov.
22. Sale or Trawfer of Leased Flremtws. Lessor shall: provide thirty (30) days prior written notice
to I .csseo in the event (C) ofany sale to a third party of any part of the leased premises, or (b).
Lessor transfers or otherwise disposes of any of the leased premises, and provide documentation
evidencing such sale or transfer in such notice, Lessor shallpromptly file a copy of any such sale
or transfer notice with the Administrative Counsel by e-mail at leas fl bouse.gov. .
13. Bankruptcy and Foreclosttra In the event (a) Lessor is placed in batthVtcy proct edings
(whether voluntarily or favolumtadly), (b) the leased premises is foreclosed upon, or (c) of any
similar accur errce, L ttcu agrees to pmnVdy notify Lessee in writing Lessor shall prampky file
a copy ofay such notice with the Office ofFiaaure; U.S. House of Representatives, O'Neill
Federal Buitding, Suite 3100. Attn: Office of financial Couttsdiug, Washington, D.C. 20515, and
with the Administrative Counsel by e-mail at IeasesQmailhouse.gov.
14. Estoppel Certilleates. Leese agrees to sip an estoppel certificate relating to tho [eased
prey dam (usually used in instances when the lessor b selling or refinancing the building) upon
the request of the Lessor: Such an estoppel certificate shall require the review of the
Administrative Counsel, prior to T awe signing the estoppel certificate. Lessor sWI promptly
provide a copy of any such estoppel certificate. to the Administrative Counsel by e-mail at
leases a@mail.how, gov.

13. Maintenance of Common Arem lessor agrees to maintain in good order, at its sole expense,

17. Lessor L hbfgty for Faila►re to MabntahL Lessor shall be liable for any damage, either to
persona or property, sustained by Lessee or any of his or her employees or guests. caused by

I& 3rrlthd Alteradom Lessor shall melee any initial alterations to the leased premises, as reputed
by Lessoe and subject to Lessor's ooment, which shall not be unreasonably withheld. The cost of
such initial alterations shall be included in the annuaircatat rate.
19. Federal Tort Clahns Act. Lessor agrees that the Federal Tort Claims Act, 28 US.0 §§ 2671-
80, satisfies any and all obligations on the pan of the Lessee to purchase private Liability
insurance. I.rssre shall not be required to provide say certificates of insurance to lessor.
24. Llnaltcuttou of Unbility. Lessor agrees that neither Lessee nor the House nor any of the House's
officas or employees will indcnmitjr..or hold harmless Lam against say liability ofLessor to
arty tb4d party that may arise during or as a nwAt of the Lam or Losste's tcnancy.
21. Compliance with Laws. Lessor shall be solely responsible for coruplying with all appUcable
Permitting and Wtuag or+dmances "requirements, and with all local and state building codes,
safety codes and handicap ecesscfbility codes (including the Americans with Disabilities Act),
both in the common ane<s.of the building and the leased space of the Lessee.
22. Elechva ie Funds Thmsftr. Lessor sprees to accept monthly rrsnt payments by Ekcbv is Funds
Transi'er and agrees to ptavide the Office ofFinance, U.S. House ofRep7eseotatives, with all
banking information necessary to facilitate such payments.
23. lltefoads. Lesser shall promptly refund to the CA,O, without foul demand, any payment made
to the Lessor by the CAO for any period for which bleat is not owed because the Lease has ended
or been terminated.
24. Conflict. Should any provision of this Attachment be ineonsit tent with any provision of the
attached Leese or attached Amendment, the provisions of this Attachment shall control, and those
inconsistent provisions of the Lease or the Amendment shall have no force ande#%ct to the
extent of such inconsistency.
