To provide testing services in connection with the City Employee Health & Financial Fair on Friday,
October 4, 2019
The cost is $60.00 per female participant and $70.00 per male participant.
• CONTRACT NO. 155-2019 — LAKE CITY BANK
This contract is to replace Contract No. 76-2019 with Huntington Bank that was a depository
agreement with the bank to serve as the depository for the City's funds held for investment. The
Board needs to rescind this contract and replace it with Contract No. 155-2019 with Lake City Bank
as the depository agreement with the City.
There is an annual fee of $500.00
