Field Ynternsi hip Affiliation Agreement
This -agreement xs between Four County Career Center and iticlunond Fire Depailment,
Mehmiond, Indiana,
Shall, be effeeilve as of the slgnat6re date of this agreement and may be terminated by either
party with a sixty (60) day written notice, the agreement will automatically renew each year
unless the proper written 60 dq notice is received,
Per the agreement, the Field internship Providers will provide -an en-vi omnent for the pre-hosp�it�.I
train ing experiences for Basic;.Advanced; and Parat tedic EMT students as described in the Ohio
Department of Public.Safety-- EMS Division, approved Curriculum.
The Field Internship Provider and Four County Career Center hereby mutually agree with eaoh
other to provide and uphold. the fo l to wing:

the Basic, Inteimediate and Paramedic EMS' Programs at )FCCC may utilize for pro-

meet specific obj a cf ive s of the EMT in tralmng. The Field Internship days, hours,
and location shah be deteimbied mutually by the Pxogam Coordinator, and the Field

2. ResponsibiII ties of the Field Internship Provider: Vo provide pro -hospital -anal
observation experience for students who qualify under the Ohio Division of EMS
approved program, for Basio, Advanced, or Paramedic training through Four Cow*
Careet Center,
3, - Provide a conference area for students when needed.
4. Allow students to use Field Internship PxovIder comf `ort' facilities, such as cafeteria,
lounges, ;and restrooms.
5. The need for an orientation session p�ior to each new trai*g groltp to use the Field
Internship Provider,

confer Program Coordinator,
Students are NOT,considered agents or employees of the Field Internship Fxovider and are NOT
entitled to any em' ployee benefits, The student can not act as part of the regular health care team
-ofthe provider during internship time@

1. To provide an outline of goals and objeolives for Basic, Advanced, and Paramedic
trauung progrfulls.
2. Provide as evaluation process for each level of EI Tf training,
3. Provide the Field hiternship Provider a list of ,students in advance wishing to
participate with the designated providea.
4. Making -sure all students are -prepared to begin field intemship.
5. Any health care issues are up to date.
d. To abide by the existing rules and policies of the Field Internship Provider as they
apply to Basic, Advanced, or Paramedic training programs,
Mutual Responsibilities:
1. Neither Four County Career Center nor the Field Internship Provider will
discriminate against any student or. employee because ofrace, color, national origin,
sex, age or physical handicap,
Four County Career Center shall inden-mi fy and hold the Field Internship Provider listed and its
trustees, officers and employees harinless fioiu all claims, damages, and costs (Including
attorney fees) arising fiom any alleged act or omission of the student participating in the program,
