• STREET CLOSURES
-City Development Corp. is requesting the closure of the following streets for the
Holiday Parade on Friday, December 6th, 2019
-No parking on East Main Street between 5th and 10th; starting at 5pm
-Closing East Main Street between 5th and IOth beginning at 6:30 pm and
reopening at 8pm
-Closing of 6th Street from the alley to Main St. on both North 6th and South
6th St.
-Use of the City Parking Lot (former Elder Beerman lot) located on 6th St.
Beginning at 5:30 to stage parade participants.
-Closing of IOth St., between Main St. and North "A" Street beginning at 6:30 pm
for parade participants to disperse

-General maintenance and plumbing repairs to Richmond fire stations
-City shall pay Contractor at the per -man hourly rates as described and set forth on the
attached Exhibit A for the services provided under this Agreement with the material
cost markup as provided in said Exhibit A.
• CONTRACT NO.181-2019 — TESTING FOR PUBLIC SAFETY, LLC DBA THE
INSTITUTE FOR PUBLIC SAFETY PERSONNEL
-City hereby retains Contractor to provide consulting and testing services in connection
with the 2020 firefighter application and hiring process.
-City shall pay Contractor at the costs and rates quoted as set forth in the attached
Exhibit A for the complete and satisfactory performance of all work described on
"Exhibit All.
• CONTRACT NO. 182-2019 — PEDRO FELICIANO, DBA PREVENTATIVE
MAINTENANCE SERVICES
-City hereby retains Contractor to provide on -call and on -site general maintenance and
repair of all Richmond Fire Department apparatus, all EMS apparatus (ambulances),
and administrative staff vehicles.
-City shall pay Contractor a sum not to exceed Forty-seven Dollars and Fifty Cents
($47.50) per hour for services provided under this Agreement performed on fire (non -
ambulance) vehicles and apparatus and shall pay Contractor a sum not to exceed Fifty-
three Dollars and Fifty Cents ($53.50) per hour for services provided under this
Agreement performed on EMS Division ambulances. Mark up pricing shall be as
outlined on Exhibit A.

- RFD agrees that Pelham Students may participate in patient care activities
through this clinical affiliation with RFD within the discretion of RFD supervising
personnel. RFD will designate a qualified professional(s) to supervise Pelham
Students assigned to participate in the clinical experience with RFD. All
academic aspects of the Educational Program, including grading, examinations,
evaluations, academic record keeping and the like shall be the sole responsibility
of Pelham.

-Adds a pay item to compensate Milestone for the time and materials used to
connect several existing downspout drains to various storm water structures
including at the Warner School on 7th Street and on South loth Street.
-Not to exceed $34,798.92
ADDENDUMS
• CONTRACT NO.21-2018 — Addendum No. 2
-City wishes to continue to retain Contractor (Cummins Electric, Inc.) to continue
to provide services as an on -call licensed electrical contractor to perform as -needed
general maintenance and electrical repairs for the City of Richmond Fire
Department, as set forth within the scope of Contract No. 21-2018
• CONTRACT NO.25-2018 — Addendum No. 2
--City wishes to continue to retain Contractor (Overhead Door) to continue to
provide services in connection with on -call and as -needed general maintenance to
the overhead doors and openers located at Fire Stations 1, 2, 3, 4, 5, and 6 for the
City of Richmond Fire Department as set forth within the scope of Contract No.
25-2018
