Whereas, the Standard Specifications for this contract provides for such work to be performed, the following change is recommended.: General or Standard Change order Explanation It was agreed by IN DOT, City of Richmond, and Milestone that 60 days be added to the contract. PM HT Pham has been notified of the change order. See Attachments in Header for further details. Additional documentation included with this Co is Milestone's formal request for days. 60 calendar days represents the total number of days for the redesign to be issued, review, and approved by 1NDOT techhical review and placed on ERMS for the contractors use. CO 21 establishes the new contract completion date of August 30, 2019. Milestone received an email from Mike Prather on 7-9-19 stating the drawings. were uploaded to INDOT's website. We were able to view the drawings on the site on 7-11-19. For this reason, Milestone Js requesting that the completion date be changed from 6-30-19 to a new date of 8-30-19 to complete this contract. City Engineer City of Richmond 50 N 5th Street Richmond, IN 47374 RE: Contract No: R-37463 Project: 1382810 E Main Street, 7th Street and 10th Street Progress Meeting 33 Dear Mr. Stiens: A Progress Meeting was held on June 26, 2019 in Construction Field Trailer. See attached sign -in sheet for those in attendance: The objective of the meeting was to review the contract requirements, project schedule, any potential utility concerns or relevant items associated with the execution of the contract for the R-37463 project. During the meeting, the following information was discussed: • Safety Concerns • Work Zone Safety — None • Pedestrian Traffic -- East Main Street is closed to vehicle traffic between 6th and 7th Streets and 51h to 61h Street. Pedestrian traffic is allowed on the South Side of East Main Street while existing street is being removed. The streets will remain closed until after Busch Landscaping, RP & L and Milestone has completed any outstanding work (Planting Trees, Shrubs, Setting Street Lights, and Placing Sidewalks etc.). Kate Barber stated -that the sidewalk signage is confusing. Jason Price stated that he will discuss with Kate Barber but thinks the signage is adequate. • Moving Equipment - None • if any needles are found on the job site. Please contact Marianne Bain -3'17-903-2176 to schedule for an inspector (One week notice is required)'. Milestone, adjusted the originally meter pits and valves on South 1 Oth Street and gave IAW the option to adjust. Jason Price stated that any additional adjustments on any meter pits or valves would be up to IAW to complete. Progress Meeting 34 Minutes July 1,2019 , Page 2 of 6 ■ Storm Structure 321 in the Northwest Corner of 6th Street and East Main Street will have to be moved to avoid conflict with the existing water line and low area. Also Richmond Power and Light has an . existing duct bank with fiber optics in it. Milestone has received approval for moving the structure from Elijah Welch Richmond - Sanitary District. • Storm Structure 323 in the Northeast Corner of 6th Street and East Main Street will have to be moved to avoid conflict with the lour area. Milestone has received approval for moving the structure from Elijah - Welch Richmond Sanitary District. • Indiana American water will need to fix their utility patches on North A Street and North 7th Streets. Joe Buckler will be coordinating with Marianne Bain with a list of patches to address. ■ School is scheduled to be out on May 231d. Greg wants to wait to address 71h and North A repair until school is out. Marianne will place this date on I NAW calendar. • Richmond Power & Light -- Will coordinate with Milestone to install conduit on East Main Street between 51 Street and 6th Street. Brent Wolfe stated that the proposed street light poles that were delivered to RP & L were the wrong color (Silver was delivered and Black is the proposed color). When the street lights have been placed and working between 6th and 7th Streets on East Main Street the tower light may be removed. ■ Any road closures for utility relocation must submit 'a request with the _ board of works in writing. • Schedule , ■ Milestone did not have an updated schedule. • Target date to start Paving on East Main Street between 51h- and 6th Streets is Tuesday, July 16, 2019. • Milestone said that they have a few more concrete pours to make on Main Street between 6th Street and 7tr' Street but once that work is complete in 2-- 3 days they can open to traffic if the City wants to. They will still need to have temporary flagging for some work including setting RP&L, light poles. - ■ Decorative Paving has a target date of Wednesday, July 3, 2019 to complete the 5oufhside of East Main Street between 6th and 7th Streets. Decorative plans on adding additional -crew. Busch Landscaping did not provided a updated schedule. • Contract Completion Date is Sunday, June 30, 2019. Milestone plans on asking for a time extension. 1 Progress Meeting 34 Minutes July 1, 2019 . Page 3of6 • Independence Day -Milestone will work a, full day on Wednesday, July 1, 2019. Work will be suspended from the end of day Wednesday, July 1, 2019 until sunrise Monday, July 8, 20,19. • Beth Fields stated that Phillips Drugs have filed an insurance claim and plan on filing a tort claim against Richmond. • Contractors must provide a 24-hour notice to Bill Stanley before starting work on the job site. If a 24-hour notice is not given, then ,the Contractor will not be paid for the day's -work. • Submittals- Plant Inspection -- Beth Fields does want REA involved in the inspection of the one year tree inspection. General Discussion Topics The Curb Ramps in the Northwest and Southwest corners of North D Street and North 101'. Street will need Decorative Paving to rework the existing brick sidewalk in an area approximately five feet behind the curb ramps to correct any trip hazards. This work has not been started. • Damaged Plant Beds - The City is not planning to remove any of the damaged plant beds currently. • Drainage Questions - Seven new inlets are being added in various locations to help address some drainage issues due to unforeseen conditions that resulted from the Iimits of the project survey. The City of Richmond Public Works & Engineering produced design plans to add these structures based on GIS and additional field survey that they collected. Milestone has already ordered these structures per Mr. Stiens' direction. Richmond will add the structures at the end of the project. Standing water at Park Place and South 1 0t' Street will be addressed by instaIIing new inlets. Richmond Sanitary District has reviewed and approved. • Fast Eddy's Building has not had any additional issues now that brick pavers are in place. The gas line in the alley between Richmond Civic Theatre and the annex will be relocated to allow for slotted drain to be installed in the alley. • Pedestrian crossings on 71h& 10ffi North Streets need to have signage and thermoplastic pavement markings such as the mid-block'crossings have. Bill Stanley and Jason Price will field check for locations that will need any additional signage and pavement markings. • New structure will be placed In the Southwest Corner of North 7�1 Street and North A' Street and a solid lid will be placed on the existing structure that is conflict will the proposed curb ramp. f • Depressed bricks on North 1 T, Street are on Milestone's radar and will be addressed. Decorative will do the work. Progress Meeting 34 Minutes July 1, 2019 Page 4 of 6 • Starr Historic Interpretive Sign -- Richmond is choosing to delay the commitment'to install the interpretive sign. Stipulation I.C. in the 2016 memorandum of agreement (MOA) calls for the production and installation of an interpretive sign within the Starr Historic District. All the terms of the MOA must be implemented within (5) years of the onset of construction. Bronze Street Markers - The bronze street markers (a.k.a. Sign Type 3) have not been installed. Milestone salvaged all of the existing bronze street markers and.have them in their possession. Milestone said that per the special provision they need the City to provide direction for which of the existing bronze street markers that were salvaged can be reused and which ones need to be replaced with a replica. Mr. Price said that he did not realize that even the makers that were not present prior to construction needed to be replicated and installed. Mr. Dills reviewed the Sign and Sign Type 3 Salvage and Glean Special Provisions as well as the related Commitments Report and agreed that as they are written, the Special Provisions accomplish the required Commitments. Milestone must re -install all of the bronze. street markers that are salvageable, and must provide and install replicas of bronze street markers at all of the locations shown on the plans where either the existing marker is determined to not be salvageable, or where one did not'exist prior to construction. These will be paid for as the pay item Sign Type 3. Two Bronze Street Markers will be added in the Northwest corner of North 10th Street and North A Street. Richmond will provide a compass from Greg Stiens office. • Structure #135/320 o Moving Structure #135/320 to the West and placing a curb turn out has been approved by Richmond Sanitary District. United and REA had not been consulted per Beth Fields request. • Non -Complaint Curb Ramps o The curb ramp on the Southeast corner of East Main Street & north 1 0t' Street Intersection. REA recommends adding a concrete wing. Greg Stiens will review. REA will send Milestone a detail of Aluminum Railing. o The sidewalk added on the Southwest corner of East Main Street and South 7th Street is non -complaint due to field changes. Jason Price stated that tooled joints define the limits so the sidewalk does not need to be 2%. Milestone will send topo and elevation of the sidewalk to REA for review. • Plants --The planter beds were not ready to have plants planted in them from March 15, 2019 thru May 15th 2019. Jason Price stated that to reduce cost due to mobldemob and the plants were not approved. REA was not ask to review or inspect the plants. • All Plants will not be out of Richmond's Greenhouse for another month. • Milestone hopes to start receiving Topsoil Structural and Amended by Friday, June 28, 2019. 4 f r ' a � 3 ti Progress Meeting 34 Minutes July 1, 2019. City Requested Design Changes: • Redesign of East Main Street at 2nd Street & 3rd Streets. Mike Prather and HT Pham have concurred with Construction Change #3. The Construction Change has not been uploaded. REA will follow up with Mike Prather to see if he can help. Greg Stiehs plans on contacting INDOT. - r • Chan a orders • The Group discussed status of current Change Orders. See Attached Change Order Summary Sheet. • CO# 13 -- United noted that 4 days need to be added. Milestone will work on getting the Change Order updated. • CO# 15 -- United noted that Busch ,needs to provide a better breakdown. • Missing Change Order Information: Down Spouts Drains, Decorative Rock, Pipe Type Changes, Additional Storm Sewer Work, Common vs Pavement Excavation. Jason Price stated that the Clam related to shutdown is still compiling. United has not received any information on these. • Drew placed the Mast Arm Extension at North 7t' Street and North -A Street Monday, June 24, 2019. • Coordination with Fire Depart • Milestone will keep the Fire Department updated on road closures. • Progress .Meeting = • The next Progress Meeting will be held. on Wednesday, July 10, 2019 at 10:00 a.m. The above minutes reflect our understanding of the discussions and decisions made at this meeting.
