General or Standard Change Order Explanation After Progress Meeting 23 held on Wednesday, October 31, 2018 Brian Pierson Design Engineer with United Consulting reviewed the Northeast comer of North 1 Oth Street and North D Street that Milestone brought up during the Progress Meeting. Brian Pierson stated that there appeared to be water being trapped by the curb bump -out on North D Street. Brian Pierson also stated that the reason a structure was not placed in this location originally is that the extents of the survey shows North D Street as being generally flat, with a very slight drop in elevation to the east. It was suggested by Brian Pierson that by adding structures and tying into the nearest proposed structure 351 in the Southeast comer of Forth 1 Oth Street and North D Street the trapped water concerns could be elimated. Milestone submitted a cost breakdown summary for the new structures. , United reviewed the breakdown and compared the overall cost with Site Manager average unit price list. Due to limited use of the item over the past couple years there is only one available., Considering the limited data and limited use of the item in the recent past, United would recommend accepting the cost submitted by Milestone be based on their cost breakdown per unit, rather than 1NDOT average unit price. PM HT Phan has been notified of this change order. See Attachments in Header Tab for details. No time modification or additional workdays were requested by Milestone as part of this change. See attached sign -in sheet for those in attendance: The objective of the meeting was to review the contract requirements, project schedule, any potential utility concerns or relevant Sterns assoclat6d with the execution of the contract for the R-37463 project: During' the meeting, the following information was discussed: + safety concerns Work Zone Safety --- street cuts on 71h Street. has. been brought to Vectren's restoration contractor's attention. The restoration will. be cornoleted this year before the asphalt plants shut down for the winter. As of this time Vectren. does not have a completion date for the repairs. A Fender Sender car accident happen at 2nd Street and Main Street on Tuesday, November 1.3, 2018 at 2:00 p.m. An accident report was submitted. The driver did not see the stop sign but stopped and then pulled into out .into Main Street. The signs were visible and not obstructed. The accident was not caused by construction: 0 Milestone placed #53 stone -in the Southwest and Southeast corners of'Main Street and 2nd Street and all of the Inlets in the area will need Temporary lntet Protection. * Pedestrian Traffic — None * Moving Equipment - None 0 Saint Mary School -No issues on North "B" Street picking up and dropping off kids for school. Temporary Tape is currently being used on north B Street for two-way pavement markings. The city is still trying to decide if they will keep two-way. traffic configuration.. city does: not want,to put down permanent paint yet. Temporary Paint will be placed on North "B"Street as directed by Greg Stiens. ITS will need an prnbient temperature of 50 degrees tb paint. No utility company representatives were at the meeting. • Indiana. Amedcan. Water — Done with the valve boxes on 1091 Street. Patch work on North "D" Street in front of the Toward Place Apartments will be dome with temporary concrete for the. winter. The City is okay with this solution. • RP&L --- Milestone has requested the locations of the street lights on South 1 gth Strut. Street Lights on Fain Street at 3rd. & 4th Streets need to be replaced. The tag outs are still in place on! 71 Street and Main Street. • Any road closures for utility relocation roust submit a request mdh the board. of works in writing. + Scheddle • Milestone prpvided 'an updated schedule. (See. Attached) Milestone said the Decorative Paving plans to get caught up with were Milestone is complete with the slab.. They are placing as much HMA setting- bed as they can because the asphalt plants are closing the week after Thanksgiving. Decorative Paving is responsible for snot removal. Decorative Paving plans to continue laying- pavers even after Milestone shuts down November 15th. • Milestone requested working a full day on Wednesday,. November 21, 2018 due to favorable. Breather conditions. Mike Prather said that he would allow it without a claim for credit. Work shall be suspended from Thanksgiving Day+ until sunrise Monday, November 25, 2018. • The City (Richrhond Parks and Recreation) requested that. the pre - emergent herbicide be applied this fall and again earl} next spring. Busch Landscaping. is requesting an additional payment of $1, 500 for mob/demob and materials to reapply the pre -emergent herbicide in. the spring. The City gave verbal approval of additional application of herbicide. Milestone will submit the pre -emergent herbicide for approval. Once it lit approved. they will apply the herbicide and mulch -the plants. • Submittals • Submiftal 15 (Tree orates) (3-2.7-1.8) needs to be corrected & resubmitted, by Milestone. + Submittal 2-2 (irrigation) (8-16-18) needs to be corrected & resubmitted by milestohel. • Submittal 25 (Specialty Signs) have been approved (11-13-18). Milestone said that they can make option 1 work by using HDPE pipe since it has a thinner wall-, and by backfilling over the pipe with concrete with a thin HMA surface. The City gage direction to add the structures. Milestone said that there is a 1-2 week lead time for 2x3 structures but they are able to get 24 structures with' 2' chair batik casting. The City checked with Richmond Sanitary District to see if they have any 2x3 structures that could be traded out,. but they did not. -INDOT, city, Milestone, United and REA reviewed the site conditions on -site after the meeting. The city asked if Unkd could run some rough drainage: calculation and comment on using the 2x2 structures. United said that since the structures are in low areas that the 2x2 structures are okay because they are very efficient. If they were in a flow line where the water was just flo"ng by and we were .trying to capture more volume and reduce spread then the size of*the casting- opening would be more of a factor. L INDOT (Rob) suggested that the City might want to require the ownerto file a permit -for this. work. RFl 22 --Down spouts at warner School on 7th Street. Milestone recommends running underdrain along the hack side of the curb to the existing let South of darner School's Parking Lot. The city said that they downspouts have not functioned for a long time, but they have given Milestone. direction to connect the downspouts to Structure #169. L There was man who lives on r- Street near the school at the meeting. He asked why curb was built in front of structure ##169. REA said that the plans called -for Str. #169 to be adjusted to grade. Milestone said, that due to the type of existing castingthat it would be difficult to adjust it to grade. Milestone said that they could have constructed a curb turn out, but the city gave them direction to plug and fill- the structure. Discussion '#'o: Milestone handed out a letter stating their proposal' regarding the start dote of the one-year warranty period of the plants that have been installed to date as well as revoking the one --year warranty period for the plants that. the City has agreed to store. Milestone also -.provided spreadsheet. that iternizes the plants that were delivered to the city for Storage. Milestone is requesting that Richmond confirrns the number, of plants that were delivered to Richmond's greenhouse.. o INDOT and the city agreed that it is not reasonable to hold the landscape contractor liable f6r costs related to the project delays that affect their work since the delays were outside of them- control. They asked Milestone to prepare the. Change order and include a detail breakdown of cost for further consideration. o REA cautioned the City that if they do not have the capability to properly store and maintain the plant material that this agreement could, actually cost therm significantly more if the plants are not viable next spring. o REA pointed out that none of the plant beds are 1 00%o complete. ■ As a- result, the plant coeds will have a combination of plants that are planted this, fall, some plants that have been stored. by the City (which would have no warranty), and the woody. shrubs and trees that have not been reviewed (which will have a third warranty period). This will impact the care given when planting new plants in r Progress Meefing 24 Minutes November: 27, 2018 Page 4 of 7 " and around existing plants without disturbing the existing plants, and will- make it difficult to track which plants are covered' by the different warranty periods. . ■ INDOT/City requested that an as -built set of drawings . identifying which plants were planted in 2018, and a photo log of the plant beds be completed. o REA asked if the price to pay Milestone to maintain the plants is greater than the credit that Milestone is offering for the value of the warrantee that is being revoked. Milestone said th4t even K they were paid to. maintain the plants that they would still not have a warranty. o REA noted that the Liriope muscari which has been rejected as a substitute for Liriope- spicata was included in the plants that were 4eiivered to, the citVs greenhouse. Progress beefing 24 Minutes Novefber 27, 201-8 Page 5 of 7 + After the. normal Construction progress Meeting, we (i.e. Greg, Jason, Sill & Phil) meet on site to discuss the three following areas as they relate to semi turning movements. o 1 ) North 1 0t' Street & North D Street Intersection: ■ Mr. Stiens said that there is a lot of semi traffic that comes down North D Street from US27 to 'l of Street. Mr. Stiens said that with the new curb constructed on the east side that semi -trailers rear wheels are tracking_ across the existing sidewalk the NW corner by a few feet. ■ Mr. Stiens asked Mr. Arden to model semi turning movements for eastbound North D Street to northbound and. southbound 10th Street. * This location was not previously identified by. the city as a location that needed to be analyzed -for semi -truck turning movements but. REA can add. and complete a turning movement analysis at this location. ■� The. plans indicate that the NW and SW comers of the intersection will be reconstructed as part of the project with a larger radii but these corners have not been constructed yet. o 2) curbed Islands adjacent to the cycle tracts between Elm. Street and North E Street: ■ Mr. Stiens had previously given Milestone verbal. direr �~tioh- to eliminate these curbed islands. At the time of our site visit, all of the changes per the City's direction were already constructed.. Mr. Arden expressed, concerns about the impact that these changes would have on the safety of bicyclist and . motorist Mr. Stiens said that he will review the modifications that have been built with Mrs. Fields and then give final direction if the built- work should stag in place or if It should be removed and built per the plans. t ■ Mr. Arden told Mr. Stiens that REA will need direction from the City it they- want REA to evaluate what modificatiops could be made to the cycle and curbed islands between Elm Street and North E Street to better accommodate a vvS-5b. o 3) curbed Islands across from Sailor Street: . ■ We looked at the islands that have already been constructed per -the plans. and what. modificaflon would need to be made to accommodate a semi -- truck. We reviewed the exhibits that REA sent the city on 1112118 modeling the turning movement of a WB- 40 and the exhibit that FLEA prepared showing modifications that could be made. It was no%ted that REKs exhibit showed narrowing the island to a 2' wide garbed island. It was noted that the turning movement model shows the truck'tire of the semi right next to the easternmost curb if the island and the I Progress Meeting 24 Minutes November 2 2018 Page 6 of 7 I truck still comes very close to hitting the -building, the fire hydrant and the light pole. It was _noted that. in it would reguire'the semi to actually encroach onto the: . cycle track to give theca any more room. Mr. Stiens asked Mr. Arden why Milestone could not just use the exhibit that was sent to reconstruct the area. Mr. Arden said that this was a preliminary exhibit showing the modifications that could be made and that this needed to be thoroughly studied and detailed once RED received feedback from the city about how to proceed. ■ Mr. Stiens asked that REEA� review this location further and provide r6commendations regarding what modification can. be made 'to the cycle track and curbed islands across from Sailor Street to better accommodate a WB-40. ■ Milestone noted that since it is already built that a decision is not critical to the schedule and that if needed they can add ,some ternporary ston6 in the plant beds ifthe City wants.
