( "I- PEP ") MEMBERSHIP AGREEMENT WHEREAS, The Indiana Public Employers' Plan, Inc. is an Indiana not - for - profit corporation (hereinafter referred to as "I -PEP" or the "Plan ") in which government entities may participate as members of a group self - funded program with the purpose of paying and administrating obligations to government employees and dependents under the Indiana Worker's Compensation Act (Indiana Code (IC) 22 -3 -2 through IC 22 -3 -7), the Indiana Employers' Liability Act (IC 22 -3 -9), in accordance with IC 23 -17 -1 et seq., hereinafter referred to as the "Acts "; and WHEREAS, The City of Richmond Board of Public Works and Safety wishes to authorize and grant authority of the below -named persons to execute the Agreement with I -PEP, after its approval by the Richmond Law Department, and to provide and execute such other documents as are necessary to affect membership in and the purposes of the Plan. NOW, THEREFORE, BE IT RESOLVED by the Board of Public Works and Safety of Richmond, Indiana, that said Board authorizes Sue Roberson, Director of Human Resources for the City, to execute for and on behalf of the Board of Works and the Mayor a Membership Agreement with the Indiana Public Employers' Plan, Inc. ( "Plan "), provided that said Agreement has been approved in its final format by the Law Department, and to provide and execute such other documents as may be necessary to affect membership and the purposes of the Plan. NOW, THEREFORE, BE IT FURTHER RESOLVED by the Board of Public Works and Safety of Richmond, Indiana, that said Board authorizes Tammy Glenn, Financial Controller of the Department of Finance for the City, to pay or cause to be paid any and all amounts required of the Plan in accordance with the Agreement in order to affect membership in and the purposes of the Plan, with funds previously appropriated for the 2012 year within the City's Workers Compensation Funds.
