RESOLUTION NO. 12 -2012
A RESOLUTION APPROVING THE ADDENDUM TO THE SUBSTANCE
ABUSE POLICY FOR ROSE VIEW TRANSIT DEPARTMENT
WHEREAS, On November 10, 2010, the Rose View Transit Substance Abuse Policy (Contract No. 160-
2010) for the Richmond Rose View Transit rural transit system as required by the Federal
Transit Administration (FTA); and
WHEREAS, Midwest Toxicology Services, Inc., is the substance abuse testing third party administrator
and has provided the attached Addendum to Substance Abuse Testing Third Party
Administrator Contract to be approved by all Rural Transportation Assistance Program
(RTAP) clients, which addendum is attached hereto as Exhibit A and incorporated by
reference; and
WHEREAS, Exhibit A updates the fee schedule associated with the substance abuse testing services
provided by Dr. Steven Moffatt (Midwest Toxicology Services, Inc., 603 E. Washington
Street, Suite 200, Indianapolis, Indiana, 46204); and
WHEREAS, It is in the best interest of the City of Richmond that the updated fee schedule be approved by
the Board of Public Works and Safety in connection with the above - described Substance
Abuse Policy previously approved by the Board of Public Works and Safety on November
10, 2010.
NOW THEREFORE, BE IT RESOLVED by the Board of Public Works and Safety of the City of
Richmond, Indiana, that said Board hereby approves the updated fee schedule attached as Exhibit A in
connection with the Rose View Transit Substance Abuse Policy (Contract No. 160 - 2010).
Passed and adopted this _ 5 th day of _ 2012, by the Board of Public Works and
Safety of the City of Richmond, Indiana.
