o/ THIS ADD IDLTM, (Number 1 to Contract No. 99-2020) is made and entered into this
day of , 2020, by and between Richmond, Indiana, a municipal corporation
acting through its Board of Public Works and Safety with its office at 50 North 51 Street, Richmond,
Indiana, 47374 (hereinafter referred to as "City"), and Maze Design, Inc., 124 South 8h Street, Richmond,
Indiana, 47374 (hereinafter referred to as "Contractor").
WHEREAS, City and Contractor entered into Contract No. 99-2020 on August 20, 2020, which
Agreement retains Contractor to provide professional services in connection with the
design and oversight of certain CARES Act and ADA-specific upgrade projects in the
City municipal building to be performed in response to COVID-19 for the City; and
WHEREAS, The Parties have agreed to increase the scope of the contract to include design services
for eight (8) additional restrooms and to include the preparation of the formal bid package
associated with the Project.
NOW, THEREFORE, in consideration of the mutual promises and covenants herein contained, including
the above stated recitals, City and Contractor hereby agree as follows:
The City and Contractor executed and entered into Contract No. 99-2020, on August 20,
2020, which Agreement is incorporated herein by reference. Pursuant to Contract No.
99-2020, Section XI (Miscellaneous), the Parties may alter or amend the Agreement in
whole or in part at any time by filing with the Agreement a written instrument setting
forth such changes signed by both Parties. The Parties agree design services for eight (8)
additional restrooms and professional services to prepare the formal bid package_
associated with the Project shall be added to the scope and shall be performed in
accordance with the scope of said Agreement. Said services shall be performed in a
manner consistent with Contract No. 99-2020, which is hereby incorporated by
reference and made a part hereof. Compensation paid to Contractor as described in
accordance with Section III (Compensation) of said Agreement for said additional
services shall be increased by the quoted amount of Six Thousand Dollars and Zero Cents
($6,000.00) as set forth on Contractor's quote, attached hereto as Exhibit A to this
Addendum and which quote is incorporated herein by reference. The new total amount
to be paid to Contractor under the Agreement shall be an amount not to exceed Twenty
Thousand Five Hundred Dollars and Zero Cents ($20,500.00) for satisfactory
performance of the Project.
City and Contractor expressly agree that all other terms, conditions, and covenants
contained in Contract No. 99-2020, shall be applicable to this Addendum, unless such
term, condition or covenant conflicts with this Addendum Number 1, in which case this
Addendum Number I shall be controlling.

3. Both Parties hereby agree that with the exception of increasing the scope to include
design services for eight (8) additional restrooms and professional services to prepare the
formal bid package, and with the exception of the corresponding compensation increase,
Contract No. 99-2020 shall remain intact and unchanged and in full force and effect.
4. Both Parties agree that any person executing this Contract in a representative capacity
hereby warrants that he or she has been duly authorized by his or her principal to execute
this Addendum.
