Authorization
I hereby authorize Specialized Mechanical to complete the proposed service, repair,
or replacement and agree to pay the invoiced amount upon completion. I additionally
certify that I am fully authorized to authorize this work and commit to payment.
Work to be performed during normal business hours unless otherwise specified. Labor & material will not exceed the quoted price for
this scope of work. Additional labor & material charges for any unforeseen work will be charged at our current labor rates upon
customer approval of any change orders. Removal of hazardous waste not included. Any roof or structural repairs are the responsibility .
of the customer. Customer to coordinate roof repairs due to any roof penetrations required in the scope of this project. Prices quoted
are valid for 30 days unless otherwise indicated. This is an estimate, not an invoice. A signature. is required for acceptance of this .
proposal.
