TEST PITS TO LOCATE EXISTING UTILITIES MAY BE ORDERED BY THE ENGINEER TO DETERMINE WHETHER TO RAISE OR LOWER THE PROPOSED WATER MAIN TO CLEAR EXISTING UTILITIES OR VERIFY EXISTING UTILITY LOCATION, SIZE AND TYPE. 4. STONE WALLS, FENCES, MAIL BOXES, SIGNS, CURBS, LIGHT POLES, ETC. SHALL BE REMOVED AS NECESSARY TO PERFORM THE WORK AND REPLACED TO A CONDITION AT LEAST EQUAL TO THAT BEFORE CONSTRUCTION BEGAN. UNLESS OTHERWISE INDICATED, ALL SUCH WORK SHALL BE INCIDENTAL TO CONSTRUCTION OF THE PROJECT. 5. ALL PAVEMENT DISTURBED BY THE CONTRACTOR'S OPERATIONS SHALL BE REPLACED IN ACCORDANCE WITH THE SPECIFICATIONS AND AS SHOWN ON THE DRAWINGS. 6. ALL AREAS DISTURBED BY THE CONTRACTOR BEYOND PAYMENT LIMITS SHALL BE RESTORED AT NO ADDITIONAL COST TO THE OWNER. 7. UNLESS OTHERWISE INDICATED, CONCRETE USED FOR PIPE ANCHOR BLOCKS, BACKING, PIPE CRADLES, ARCHES, AND FILL SHALL HAVE A MINIMUM COMPRESSIVE STRENGTH OF 3000 PSI AT 28 DAYS. 8. APPROVED JOINT RESTRAINT METHODS SHALL BE PROVIDED FOR WATER MAINS WHERE ANY BENDS, TEES, PLUGS, OR WYES ARE INSTALLED. CONCRETE THRUST BLOCKS, ANCHOR BLOCKS AND TIE RODS MAY BE USED FOR 6-INCH AND 8-INCH PIPE WHERE JOINT RESTRAINT IS NOT FEASIBLE, ONLY AS APPROVED BY THE ENGINEER. 20. SOME WATER AND SEWER SERVICE CONNECTIONS MAY NOT BE SHOWN ON THE DRAWINGS. THE OWNER WILL MARK THE LOCATION OF SUCH CONNECTIONS, PROVIDED THE CONTRACTOR GIVES THE OWNER AT LEAST 5 WORKING DAYS ADVANCE NOTICE. 21. ELEVATIONS REFERENCED ARE ASSUMED VERTICAL DATUM OF 100'. 22. EXISTING UTILITY INFORMATION, TOPOGRAPHIC INFORMATION, EDGE OF PAVEMENT, UTILITY POLE LOCATIONS, AND LOCATIONS OF EXISTING ABOVE GROUND STRUCTURES WERE TAKEN FROM A FIELD SURVEY CONDUCTED BY WESTON & SAMPSON ENGINEERS, INC. 23. SEWER SERVICES WERE NOT SURVEYED BUT WERE LOCATED ON THE PLANS USING SEWER SERVICE CARDS AND EXISTING UTILITY MARK OUTS. 24. TEMPORARY WATER SERVICE PIPE WILL BE REQUIRED FOR ALL RESIDENTS WITHIN THE CONTRACT LIMITS WHERE THEY WILL BE WITHOUT WATER FOR A PERIOD OF 4 HOURS OR MORE AND ALSO WHERE REQUIRED BY THE ENGINEER. TEMPORARY GATE VALVES AND HYDRANTS ARE TO BE PROVIDED, AS REQUIRED BY THE ENGINEER, OR AT A MINIMUM IN THE SAME LOCATION AS EXISTING. 25. PROPOSED WATER MAINS SHALL BE INSTALLED IN THE SAME TRENCH AS THE EXISTING WATER MAINS, UNLESS OTHERWISE APPROVED BY THE ENGINEER OR SHOWN ON THE PLANS. CONTRACTOR SHALL REMOVE AND DISPOSE OF EXISTING WATER MAIN. 26. CONTRACTOR SHALL BE RESPONSIBLE FOR MAINTAINING TEMPORARY WATER MAIN 24/7 WHILE ACTIVELY BEING USED TO FEED DWELLINGS AND BUILDINGS. TEMPORARY GATE VALVES AND FIRE HYDRANTS ARE TO BE PROVIDED, AS REQUIRED. 27. ALL NEW AND TEMPORARY WATER MAIN SHALL PASS TWO CONSECUTIVE BACTERIA AND PRESSURE TESTS, TAKEN 24 HOURS APART. THE BACTERIA TESTS SHALL BE APPROVED BY A MASSACHUSETTS DEPARTMENT OF ENVIRONMENTAL PROTECTION (MassDEP) CERTIFIED LABORATORY. PRESSURE TEST TO BE 200 PSI FOR TWO HOURS ON NEW MAIN. 9, THE CONTRACTOR SHALL NOT STORE ANY APPARATUS, MATERIALS, SUPPLIES, OR EQUIPMENT ON DRAINAGE STRUCTURES OR WITHIN 100 FEET OF WETLANDS. 28. CONTRACTOR SHALL REPAIR ALL PAVEMENT MARKINGS INCLUDING, BUT NOT LIMITED TO, CROSSWALKS, ROADWAY LINES, AND STOP BARS. 10. NEW WATER MAINS AND SERVICES SHALL BE INSTALLED AT THE MINIMUM DEPTH FROM FINISH GRADE TO TOP OF PIPE AS SHOWN ON THE DRAWINGS. WHERE NECESSARY, NEW WATER MAINS SHALL BE INSTALLED AT A GREATER DEPTH TO CLEAR OBSTACLES SHOWN ON THE DRAWINGS AT NO ADDITIONAL COST TO THE OWNER. MINIMUM CLEARANCES TO UTILITIES, AS SHOWN ON THE DRAWINGS SHALL BE MAINTAINED. 11. EXISTING SERVICES SHALL NOT BE CONNECTED TO THE PROPOSED WATER MAIN UNTIL THAT MAIN HAS PASSED PRESSURE TEST AND DISINFECTION REQUIREMENTS. 12. WATER SERVICE SHALL NOT BE INTERRUPTED MORE THAN 4 HOURS WITHOUT PRIOR APPROVAL OF THE OWNER. 13. AS APPROVED BY THE ENGINEER, HYDRANTS SHALL BE EITHER DISPOSED OF OR SALVAGED. SALVAGED HYDRANTS SHALL BE DELIVERED TO A LOCATION TO BE DETERMINED BY THE OWNER. 14. ANY HYDRANT WHICH IS NOT IN SERVICE SHALL BE COVERED WITH A SECURELY FASTENED BURLAP BAG. 15. THE CONTRACTOR SHALL REMOVE ALL VALVE BOXES OF VALVES ON ABANDONED WATER MAINS. THESE AREAS SHALL BE BACKFILLED AND PAVED AS REQUIRED. VALVE BOXES SHALL BE DELIVERED TO A LOCATION TO BE DETERMINED BY THE OWNER. 16. EXISTING WATER MAINS OR SERVICES PARALLEL TO PROPOSED WATER MAINS OR SERVICES SHALL ONLY BE ABANDONED IF SHOWN ON THE PLANS OR APPROVED BY THE ENGINEER OR OWNER. ALL SERVICE CONNECTIONS SHALL BE TRANSFERRED TO THE NEW WATER MAIN AS REQUIRED. 17. THE LOCATION OF PIPES, CAPS, REDUCERS, BENDS, AND OTHER FITTINGS AT POINTS OF CONNECTIONS TO EXISTING MAINS IS APPROXIMATE. CONTRACTOR SHALL DIG A TEST PIT AT EACH LOCATION TO DETERMINE THE DIAMETER AND MATERIAL OF EXISTING PIPE AND THE LOCATION OF THE TIE-IN POINT. 18. ALL STREET EXCAVATIONS SHALL BE COMPLETELY CLOSED AT THE END OF EACH WORKING DAY BY BACKFILLING. COVERING WITH STEEL PLATES MAY BE ALLOWED IF APPROVED BY THE ENGINEER. IF STEEL PLATES ARE ALLOWED, THEY SHALL BE RECESSED AND PINNED WHEN UTILIZED IN THE ROADWAY DURING WINTER MONTHS. 19. THE OWNER WILL, ON 72 HOURS NOTICE FROM THE CONTRACTOR, OPEN AND/OR CLOSE ANY VALVES REQUIRED FOR DRAINING OR ADMITTING WATER TO THE VARIOUS SECTIONS OF THE WATER MAINS. HOWEVER, THE CONTRACTOR MAY OCCASIONALLY BE REQUIRED TO OPERATE VALVES IN ORDER TO OBTAIN SHUTDOWNS. THE CONTRACTOR IS RESPONSIBLE TO NOTIFY IN WRITING 48 HOURS IN ADVANCE ANY OCCUPANT THAT WILL BE WITHOUT WATER DUE TO A SHUTDOWN. ALL REQUIRED NOTIFICATION SHALL BE PROVIDED BY THE CONTRACTOR. ENVIRONMENTAL 1. THE CONTRACTOR SHALL PROVIDE AN EXCAVATED MATERIALS MANAGEMENT PLAN (EMMP) SUBMITTAL FOR ENGINEER APPROVAL PRIOR TO TRENCH EXCAVATION AND GENERATION OF SURPLUS SOIL. THE EMMP REQUIREMENTS ARE LISTED IN SECTION 02130 ITEM 1.03 - TRANSPORTATION AND DISPOSAL OF IMPACTED MATERIAL. SOIL REMOVAL SHALL NOT BEGIN UNTIL THE EMMP HAS BEEN SUBMITTED TO AND APPROVED BY THE ENGINEER. 2. THE CONTRACTOR SHALL BE AWARE OF PROPERTIES LISTED WITH THE MASSACHUSETTS DEPARTMENT OF ENVIRONMENTAL PROTECTION (MassDEP) WITH RELEASE TRACKING NUMBERS (RTNS) AS LISTED IN SECTION 00320 - SUBSURFACE DATA. THE WORK AREA ON NASH STREET WILL BE PERFORMED UNDER A UTILITY -RELATED ABATEMENT MEASURE (URAM) FILED WITH THE MassDEP. 29. TREE PROTECTION SHALL BE INSPECTED BY TOWN TREE WARDEN PRIOR TO COMMENCEMENT OF WORK. 30. CALCULATION OF PIPE SLOPES IS BASED ON ELEVATION CHANGES DIVIDED BY THE DISTANCE BETWEEN THE OUTSIDE EDGES OF THE MANHOLE WALLS. FOR FOUR FOOT DIAMETER MANHOLES, THIS DISTANCE WAS CALCULATED AS THE CENTERLINE STATIONING MINUS FIVE FEET. FOR FIVE FOOT DIAMETER MANHOLES, THIS DISTANCE WAS CALCULATED AS THE CENTERLINE STATIONING MINUS SIX FEET. FOR SIX FOOT DIAMETER MANHOLES, THIS DISTANCE WAS CALCULATED AS THE CENTERLINE STATIONING MINUS SEVEN FEET. 'IN' INDICATES UPSTREAM END OF MANHOLE, 'OUT' INDICATES DOWNSTREAM END OF MANHOLE. 31. THE CONTRACTOR SHALL COMPLY WITH ALL REQUIREMENTS OF MASSACHUSETTS GENERAL LAW CHAPTER 82A, TRENCH EXCAVATION AND SAFETY REQUIREMENTS, TO PREVENT THE GENERAL PUBLIC FROM UNAUTHORIZED ACCESS TO UNATTENDED TRENCHES. 32. ALL WORK PERFORMED WITHIN MassDOT LIMITS SHALL CONFORM TO THE PERMIT ISSUED FOR THIS PROJECT. 33. INSULATION SHALL BE PROVIDED FOR WATER MAINS WITH LESS THAN 4.5 FEET OF COVER. THE INSULATION SHALL BE AS SPECIFIED. 34. EXCAVATION SHALL BE COMPLETED IN ACCORDANCE WITH ALL APPLICABLE MassDOT STANDARDS, SPECIFICATIONS, AND PERMITS. 35. CONTRACTOR SHALL INSTALL TRENCH WIDTH PAVEMENT IN ACCORDANCE WITH ALL RELEVANT PLANS, SPECIFICATIONS, AND PERMITS. IN CASES WHERE THE EXISTING PAVEMENT DEPTHS EXCEED THESE REQUIREMENTS, CONTRACTOR WILL MATCH EXISTING PAVEMENT DEPTH, ELEVATION, AND SLOPE. 36. TEMPORARY PAVEMENT SHALL BE INSTALLED BY THE END OF THE WEEK. 37. RODENT CONTROL BOXES SHALL BE PLACED ABOVE GROUND WITH A SPACING AS SPECIFIED IN SPECIFICATION SECTION 01577 RODENT CONTROL. ABOVE GROUND LOCATIONS SHALL BE VISIBLE TO THE PUBLIC WHENEVER POSSIBLE. RODENT CONTROL BOX PLACEMENT SHALL EXTEND ONE BLOCK OUTSIDE OF CONSTRUCTION LIMITS. 4. IF THE CONTRACTOR'S HANDLING OF IMPACTED MATERIAL CONTAMINATES NON -IMPACTED MATERIALS, THE CONTRACTOR SHALL REMEDIATE, TRANSPORT, AND DISPOSE OF ALL NEWLY -CONTAMINATED MATERIAL AT NO ADDITIONAL COST TO THE OWNER. 5. THE CONTRACTOR SHALL HANDLE EXCAVATED MATERIALS, AS DESCRIBED IN SECTION 02113 - EXCAVATION AND STOCKPILING OF IMPACTED MATERIAL AND SECTION 02130 - TRANSPORTATION AND DISPOSAL OF IMPACTED MATERIAL, AND DEWATERING FLUIDS, AS DESCRIBED IN SECTION 02240 - DEWATERING, THE CONTRACTOR SHALL ALSO HANDLE THE EXCAVATED MATERIALS AND FLUIDS IN ACCORDANCE WITH SECTION 01380 - HEALTH AND SAFETY PLAN, SECTION 01562 - DUST CONTROLS, AND SECTION 01570 - ENVIRONMENTAL PROTECTION.
