APPOINTING A C -3 LICENSED OPERATOR
FVfIEREAS, the Borough of Carteret (the "Borough) is required to have a C -3 Licensed
Operator of its Sewer Treatment Facility; and
WHEREAS, Joseph T. Kotowski is qualified and holds the requisite C -3 License as a
Sewer Treatment Facility Operator.
NOW, THEREFORE, BE IT RESOLVED by the -Mayor and Council of the Borough
of Carteret, County of Middlesex, State of New Jersey, as follows: -
1. Joseph T. Kotowski is hereby retained as the Licensed C -3 Operator
Carteret Sewer Treatment Facility, effective January 1, 2015 to December
31, 2015.
2. Joseph T. Kotowski shall be compensated at $900.00 monthly, not to
exceed $10,800.00 per annum, and shall not receive any other benefits or
compensation.
3.- Said Services shall include the preparation and filing of all operation
reports, performance of all services required by the State of New Jersey
and any other regulatory bodies of a C -3 Licensed Operator of the Carteret
Sewer Treatment Facility and any and all services, emergency and non-
emergency, that require the on -site supervision of Joseph T. Kotowski.
4. The Mayor and appropriate Borough Officials are hereby authorized to
enter into an agreement with Joseph T. Kotowski for the performance of
said Professional Services.

Borough Hall, 61 Cooke Avenue, Carteret, New Jersey 07008
As per Resolution Number 15 -39.
THE CONTRACTOR does hereby agree to perform the following: APPOINTING C -3 LICENSED
OPERATOR FOR SEWER TREATMENT FACILITY OPERATOR.
NOT TO EXCEED $10,800.00
According to the terms set forth in the Resolution.
THE BOROUGH does hereby agree to pay the Contractor the sum set forth in the Resolution.

presents to be signed by their proper corporate offices and their corporate seals to be hereto
affixes, the day and year written above.
