A RESOLUTION ADOPTING THE CITY OF MOAB CAFETERIA PLAN AND SUMMARY PLAN
DESCRIPTION DOCUMENTS, INCLUDING DEPENDENT CARE AND HEALTH FLEXIBLE
SPENDING ACCOUNTS
WHEREAS, the City of Moab is desirous of offering a Cafeteria Plan Benefit to our classified
employees and elected officials; and
WHERAS, Cafeteria Plan and Summary Plan Description Documents (referred to as the
"Plan") should be approved by the Governing Body;
NOW, THEREFORE, we, the Governing Body of the City of Moab do herby resolve the
following:
1. That the form of the amended Cafeteria Plan including a Dependent Care Flexible
Spending Account and Health Flexible Spending Account effective September 1,
2010 is presented to this meeting and is hereby approved and adopted and that
the proper officers of Moab City are hereby authorized and directed to execute
said documents.
2. That the Plan Administrator shall be instructed to take such actions that are
deemed necessary and proper in order to implement the Plan, and to set up
adequate accounting and administrative procedures to provide benefits under
the Plan.
3. That the proper officers of the Corporation shall act as soon as possible to notify
the employees of the City of Moab of the adoption of the Cafeteria Plan by
delivering to each employee a copy of the summary description of the Plan in the
form of the Summary Plan Description presented to this meeting, which form is
herby approved.
This resolution shall take effect immediately upon passage.
Passed and adopted by action of the Governing Body of the City of Moab in open session
this 24th day of August, 2010.
