ALL EQUIPMENT AND MATERIALS SHALL BE USED OR INSTALLED IN ACCORDANCE WITH MANUFACTURER'S RECOMMENDATIONS. 6. THE CONTRACTOR SHALL REMOVE AND IMMEDIATELY REPLACE, RELOCATE, RESET OR RECONSTRUCT ALL OBSTRUCTIONS IN THE WORK AREA, INCLUDING, BUT NOT LIMITED TO SIGNS, LANDSCAPING, LIGHTING, PLANTERS, CULVERTS, DRIVEWAYS, PARKING AREAS, CURBS, GUTTERS, FENCES, OR OTHER MAN-MADE OR NATURAL OBSTRUCTIONS. TRAFFIC CONTROL, REGULATORY, WARNING, AND INFORMATIONAL SIGNS SHALL REMAIN FUNCTIONAL AND VISIBLE TO APPROPRIATE LANES OF TRAFFIC AT ALL TIMES, WITH THEIR RELOCATION KEPT TO A MINIMUM DISTANCE. THE COST SHALL BE INCLUDED IN THE COST OF ITEM BIDS. 7. CONTRACTOR SHALL BE RESPONSIBLE FOR MAINTENANCE OF TRAFFIC IN WORK AREAS DURING CONSTRUCTION OPERATIONS. 8. THE CONTRACTOR SHALL BE RESPONSIBLE TO INSURE THAT PAVING IS INSTALLED TO THE ELEVATIONS AND THAT NO PONDING OF WATER EXISTS AFTER PAVING IS COMPLETE. IF PONDING AREA EXISTS AFTER PAVEMENT WORK IS COMPLETE THE CONTRACTOR WILL BE REQUIRED TO REMOVE THE PAVED AREA A MINIMUM OF 50 FEET ON EACH SIDE OF THE PONDING AREA FOR THE FULL WIDTH OF THE STREET AND REPLACE WITH NEW PAVING, AT NO ADDITIONAL COST TO THE OWNER. NO SMALL PATCHES OP REHEATING OF ASPHALT TO CORRECT DEFICIENCIES WILL BE ALLOWED. 10. ALL ADJACENT ROADWAYS AND SIDEWALKS (i.e., J. HART CLINTON DRIVE AND THE PEDESTRIAN PATHWAY) ARE TO BE MAINTAINED FREE OF SEDIMENT. MAINTENANCE COSTS SHALL BE INCURRED BY CONTRACTOR. 1 1. CONTRACTOR SHALL GRADE, TOPSOIL, SEED AND MULCH ALL DISTURBED AREAS. 12. CONTRACTOR SHALL ADJUST TO FINISH GRADE, AS REQUIRED, ANY VALVE BOXES, MANHOLES, CATCH BASINS, ETC. PRIOR TO PAVING OR RESURFACING. 13. INLET PROTECTION SHALL BE INSTALLED ON ALL EXISTING AND PROPOSED CATCH BASINS IN OR NEAR THE CONSTRUCTION AREA OR EFFECTED BY THE CONSTRUCTION AREA. 14. ALL ROADWAYS, DRIVEWAYS, PAVED PATHWAYS, ETC., THAT ARE DAMAGED, OTHER THAN WHAT IS DESIGNATED ON THE PLANS TO BE REMOVED AND REPLACED, SHALL BE RESTORED TO ORIGINAL CONDITION BY THE CONTRACTOR AT HIS/HER OWN EXPENSE. 15. CONTRACTOR SHALL PROVIDE STAKEOUT SURVEY NECESSARY FOR THE SITE WORK AND THE INSTALLATION OF UTILITY WORK AND APPURTENANCES. UTILITY FEATURES TO BE INSTALLED IN UNPAVED AREAS SHALL BE SET TWO (2) INCHES ABOVE FINISH GRADE. 16. DRAWINGS DO NOT INCLUDE NECESSARY COMPONENTS FOR CONSTRUCTION SAFETY, ALL CONSTRUCTION MUST BE DONE IN COMPLIANCE WITH THE OCCUPATIONAL SAFETY AND HEALTH ACT (OSHA) OF 1970, AS AMENDED AND ALL RULES AND REGULATIONS THERETO. 17. THE CONTRACTOR SHALL BE RESPONSIBLE FOR INSURING THAT ALL CONSTRUCTION WORK MEETS APPROPRIATE BUILDING CODES. 18. THE CONTRACTOR SHALL NOTIFY THE CSM CONSTRUCTION MANAGER TWO WEEKS PRIOR TO THE START OF CONSTRUCTION AND SHALL APPRAISE AND COORDINATE DURING ALL PHASES OF CONSTRUCTION. 19. ALL REVISIONS TO THESE PLANS MUST BE REVIEWED AND APPROVED IN WRITING BY THE DESIGN ENGINEER AND MAY ADDITIONALLY REQUIRE APPROVAL BY JURISDICTIONAL AUTHORITIES PRIOR TO CONSTRUCTION OF AFFECTED ITEMS. 20. EXCAVATIONS SHALL BE ADEQUATELY SHORED, BRACED AND SHEATHED SO THAT THE EARTH WILL NOT SLIDE OR SETTLE AND SO THAT ALL EXISTING. IMPROVEMENTS OF ANY KIND WILL BE FULLY PROTECTED FROM DAMAGE. ANY DAMAGE RESULTING FROM A LACK OF ADEQUATE SHORING, BRACING AND SHEATHING, SHALL BE THE RESPONSIBILITY OF THE CONTRACTOR AND HE SHALL COMPLETE NECESSARY REPAIRS OR RECONSTRUCTION AT HIS OWN EXPENSE. WHERE THE EXCAVATION FOR A TRENCH, AND/OR STRUCTURE IS FIVE (5) FEET IN DEPTH, THE CONTRACTOR SHALL PROVIDE SHEATHING, SHORING AND BRACING IN CONFORMANCE WITH THE APPLICABLE CONSTRUCTION SAFETY ORDERS. THE CONTRACTOR SHALL COMPLY WITH ALL OSHA REQUIREMENTS AT ALL TIMES. 21. THE CONTRACTOR SHALL PROVIDE DUST CONTROL FOR THE ENTIRE PROJECT SITE AT ALL TIMES. THE WORK AREAS AND ACCESS ROADS SHALL BE SPRINKLED AS NECESSARY TO PREVENT DUST NUISANCE. IN THE EVENT THE CONTRACTOR NEGLECTS TO USE ADEQUATE MEASURES TO CONTROL DUST, THE OWNER RESERVES THE RIGHT TO TAKE WHATEVER MEASURES ARE NECESSARY TO CONTROL DUST AND CHARGE THE COST TO THE CONTRACTOR. 22. IF, DURING CONSTRUCTION, ARCHAEOLOGICAL REMAINS ARE ENCOUNTERED, CONSTRUCTION IN THE VICINITY SHALL BE HALTED AND THE OWNER AND ENGINEER SHALL BE NOTIFIED. 23. THE CONTRACTOR SHALL POST EMERGENCY TELEPHONE NUMBERS FOR POLICE, FIRE, AMBULANCE, AND THOSE AGENCIES RESPONSIBLE FOR MAINTENANCE OF UTILITIES IN THE VICINITY OF THE JOB. 24. THE CONTRACTOR IS RESPONSIBLE FOR CONFORMING TO SURROUNDING ELEVATIONS AND CONDITIONS AND OTHER IMPROVEMENTS WITH A SMOOTH TRANSITION. AND TO AVOID ANY ABRUPT OR APPARENT CHANGES IN GRADES OR CROSS SLOPES, LOW SPOTS OR HAZARDOUS CONDITIONS. UTILITIES 1. A MINIMUM OF TWELVE (12) INCHES VERTICAL CLEARANCE SHALL BE PROVIDED BETWEEN ADJACENT UTILITIES AT ALL UTILITY CROSSINGS UNLESS OTHERWISE NOTED. RECORD DRAWINGS 1.
