CAUTION : The engineer preparing these plans will not be responsible
for, or liable for, unouthorized changes to or uses of these plans.
All changes to the plans must be in writing and must be approved
by the preparer of the plans.
GENERAL NOTES
1. ALL CONSTRUCTION MATERIAL AND METHODS SHALL COMPLY WITH THE LATEST
EDITION OF THE STATE OF CALIFORNIA STANDARD SPECIFICATIONS (JULY 1992).
CONTRACTOR SHALL PROVIDE AN EMERGENCY PHONE NUMBER WITH THE SAN MATEO
POLICE AND FIRE DEPARTMENTS, THE CITY OF SAN MATEO, AND THE RESIDENT
ENGINEER.
3. CONTRACTOR AGREES THAT HE SHALL ASSUME SOLE AND COMPLETE RESPONSIBILITY
FOR JOB SITE CONDITIONS DURING THE COURSE OF CONSTRUCTION OF THIS PROJECT,
INCLUDING SAFETY OF ALL PERSONS AND PROPERTY, THAT THIS REQUIREMENT SHALL
APPLY CONTINUOUSLY AND NOT BE LIMITED TO NORMAL WORKING HOURS, AND THAT
THE CONTRACTOR SHALL DEFEND, INDEMNIFY AND HOLD THE OWNER AND THE
ENGINEER HARMLESS FROM ANY AND ALL LIABILITY, REAL OR ALLEGED, IN
CONNECTION WITH THE PERFORMANCE OF WORK ON THIS PROJECT, EXCEPTING FOR
LIABILITY ARISING FROM THE SOLE NEGLIGENCE OF THE ENGINEER.
4. ALL UNDERGROUND UTILITIES SHALL BE COMPLETE BEFORE PLACING OF BASE ROCK
UNLESS OTHERWISE NOTED.
CONTRACTOR SHALL NOTIFY THE PUBLIC OR PRIVATE UTILITY OWNERS 72 HOURS
PRIOR TO COMMENCEMENT OF WORK ADJACENT TO THE UTILITY UNLESS AN
EXCAVATION PERMIT SPECIFIES OTHERWISE.
6. CONTRACTOR TO CONTACT UNDERGROUND SERVICE ALERT U.S.A. 800-642-2444
FORTY-EIGHT HOURS PRIOR TO BEGINNING WORK TO HAVE THE LOCATION OF
EXISTING UNDERGROUND UTILITIES MARKED.
7. ALL MATERIAL SHALL BE FURNISHED AND INSTALLED BY THE CONTRACTOR UNLESS
OTHERWISE NOTED.
8. CONTRACTOR SHALL POST ON THE SITE, EMERGENCY TELEPHONE NUMBERS FOR THE
PUBLIC WORKS, AMBULANCE, POLICE, AND FIRE DEPARTMENTS, AND THE RESIDENT
ENGINEER.
9. PAVING CONTRACTOR TO RESET ALL MANHOLE RIMS AND VALVE BOXES TO GRADE
AFTER PLACING THE BASE AND IMMEDIATELY PRIOR TO THE LAST LIFT OF PAVING.
10. THE CONTRACTOR SHALL BE RESPONSIBLE TO PROVIDE AMPLE COVER FOR THE
PROTECTION OF ALL INSTALLED UTILITIES DURING THE CONSTRUCTION OF THIS
PROJECT.
11. THE CONTRACTOR SHALL SCHEDULE, ARRANGE, AND PAY FOR ANY WORK TO BE DONE
BY THE SERVING UTILITY COMPANIES IN CONJUNCTION WITH AND NECESSARY FOR
THE INSTALLATION OF THE COMPLETE AND OPERABLE UTILITY SYSTEMS SHOWN
HEREON UNLESS OTHERWISE NOTED.
12. THE CONTRACTOR IS RESPONSIBLE FOR MATCHING EXISTING STREETS, SURROUNDING
LANDSCAPE AND OTHER IMPROVEMENTS WITH A SMOOTH TRANSITION IN PAVING,
CURBS, GUTTERS, SIDEWALKS, GRADING, ETC., AND TO AVOID ANY ABRUPT OR
APPARENT CHANGES IN GRADES OR CROSS SLOPES, LOW SPOTS OR HAZARDOUS
CONDITIONS.
13. CONTRACTOR SHALL VERIFY LOCATION OF DRIVEWAYS, SEWER LATERALS,
AND WATER SERVICES BEFORE POURING CURBS AND SHALL MARK FACE OF CURBS A
LETTER "S" FOR SEWER LATERALS AND A LETTER "W" FOR WATER SERVICES.
14. UTILITIES AND UNDERGROUND FACILITIES INDICATED ARE FOR INFORMATION ONLY. IT
IS THE CONTRACTOR'S RESPONSIBILITY TO VERIFY THE LOCATION AND DEPTH WITH
THE APPROPRIATE AGENCIES. NEITHER THE OWNER NOR THE CITY NOR THE ENGINEER
ASSUMES RESPONSIBILITY THAT THE UTILITIES AND UNDERGROUND FACILITIES
INDICATED WILL BE THE UTILITIES AND UNDERGROUND FACILITIES ENCOUNTERED.
15. THE CONTRACTOR IS RESPONSIBLE FOR COORDINATING ALL NECESSARY UTILITY
RELOCATIONS, IF REQUIRED, WITH THE APPROPRIATE UTILITY COMPANIES.
16. THE CONTRACTOR SHALL CONDUCT ALL GRADING OPERATIONS IN SUCH A MANNER
AS TO PRECLUDE WIND BLOWN DIRT AND DUST AND RELATED DAMAGE TO
NEIGHBORING PROPERTIES. THE MEANS OF DUST CONTROL SHALL BE LEFT TO THE
DISCRETION OF THE CONTRACTOR. THE CONTRACTOR SHALL ASSUME LIABILITY FOR
CLAIMS RELATED TO WIND BLOWN MATERIAL. THE CONTRACTOR SHALL BE
RESPONSIBLE FOR PROPER EROSION AND SEDIMENT CONTROL.
17. CONTRACTOR SHALL BE RESPONSIBLE FOR OBTAINING ALL APPLICABLE PERMITS AND
FURNISHING THE OWNER WITH A COPY OF ALL PERMITS AND FINAL ACCEPTANCES
GRANTED BY THE UTILITY COMPANY AND/OR AGENCIES INVOLVED PRIOR TO
COMMENCEMENT OF WORK.
18. CONTRACTOR IS RESPONSIBLE FOR HAVING EXISTING GAS LINES, POWER LINES,
WATER LINES, ETC. DISCONNECTED AND RELOCATED IF NECESSARY. SEE
PLUMBING PLANS.
19. THE PERMITTEE MUST MAINTAIN THE STREETS, SIDEWALKS, AND ANY OTHER PUBLIC
RIGHTS -OF -WAY IN A CLEAN, SAFE, AND USABLE CONDITION. ANY SPILLS OF SOIL,
ROCK OR CONSTRUCTION DEBRIS MUST BE REMOVED FROM THE PUBLICLY OWNED
PROPERTY DURING CONSTRUCTION AND UPON COMPLETION OF THE PROJECT.
20. CONTRACTOR SHALL EXCAVATE AND EXPOSE ALL UTILITY CROSSINGS PRIOR TO
LAYING PIPE. THE ENGINEER SHALL BE NOTIFIED OF ANY ADJUSTMENTS
NECESSITATED BY THE WAY OF CONFLICT WITH EXISTING UTILITIES.
21. GRADE BREAKS TO BE ROUNDED OFF IN CONCRETE FORM WORK AND FINISH
SURFACING.
22. ALL WORK SHALL BE PERFORMED IN ACCORDANCE WITH THE PROJECT PLANS AND
SPECIFICATIONS HEREON AND GEOTECHNICAL INVESTIGATION REPORT PREPARED BY
HARDING LAWSON ASSOCIATES, JOB NO. 22823-1.0, DATED JAN. 29, 1993.
23. CONTRACTOR SHALL PROVIDE THE INSPECTING GEOTECHNICAL ENGINEER 48 HOURS
ADVANCE NOTICE OF WORK SCHEDULED FOR TESTS AND INSPECTIONS, PRIOR TO
COVERING UP OR CLOSING IN OF WORK INVOLVED.
24. CONTRACTOR SHALL REPAIR ALL TRENCHES TO MATCH THE EXISTING CONDITIONS.
25. EXISTING SITE CONDITIONS ON THE FOLLOWING PLANS HAVE BEEN PROVIDED BY A
BOUNDARY AND TOPOGRAPHIC SURVEY BY ACRES CONSULTING ENGINEERS, 246 FIRST
STREET, SAN FRANCISCO, CA. 94105 DATED, JANUARY, 1992. UPDATED SURVEY
