This supplemental agreement made and entered into this 3 day of juo , 2021, by
and between the City of Richmond, Indiana, acting by and through its proper officials,
hereinafter referred to as the "Local Public Agency" or "LPA", and Clark Dietz, Inc., Consulting
Engineers,hereinafter referred to as the "Consultant".
WITNESSETH
WHEREAS, the Local Public Agency and the Consultant did, on August 10, 2018, enter into an
Agreement for providing engineering services for the design and preparation of construction
plans for the proposed Complete Streets Loop Project Phase 2 along Main Street, Fort Wayne
Avenue, and North E Street (Des. No. 1702769), and on April 22, 2021, executed Supplemental
Agreement No. 1 for additional services to perform RW Engineering Services and
Supplementary Environmental Services, and
WHEREAS, it has been determined by the Local Public Agency that additional services are now
required to perform electrical design and layout on Main Street, and
WHEREAS, in order to provide for completion of these services it is necessary to amend and
further supplement the original Agreement by$28,700.00.

NOW, THEREFORE, it is agreed by and between the parties hereto as follows:
A. The original Contract is amended to provide for the additional services required to
perform electrical design and layout on Main Street for the proposed Complete
Streets Loop Project Phase 2 along Main Street,Fort Wayne Avenue, and North E
Street. Electrical design and layout will include providing plan details,
specifications, quantity calculations, and construction cost estimate for the
addition of electrical facilities along Main Street from 7th Street to 10th Street.
B. Amount of Payment Section of Appendix D of the original Agreement is amended
to read as follows:
1. The total compensation for this Agreement will be a lump sum amount of
$433,445.00. This lump sum compensation includes salaries, payroll taxes and
insurance, employee fringe benefits, general overhead costs, profit, and project
related expenses.
2. The following is a breakdown of the items of work to be performed under this
agreement and the fees for each item:

3. Except as herein modified, changed and supplemented, all terms of the original
Agreement,dated August 10,2018, and Supplemental Agreement No. 1, dated April
22,2021,shall continue in full force and effect.
IN WITNESS WHEREOF, the parties hereto have made and executed this SUPPLEMENTAL
AGREEMENT the day and year first mentioned above.
