IMIP offers coverage through the Argonaut Group of Companies. In addition,
reinsurance support for the program is provided by Swiss Reinsurance of America. Both
organizations deliver solid financial support and service to our policyholders.

Service for IMIP is shared between H lant Administrative Services Inc. and Trident
Insurance Services LLC. Both organizations are dedicated to servicing the needs of
preferred governmental agencies throughout the state of Indiana.
Trident's CLAIMS department brings the experience of having settled over 75,000
Public Entity claims in the past 15 years. This brings an unsurpassed level of knowledge
and expertise to the program.
Hylant's UNDERWRITING staff specializes only in governmental entities. We help to
identify the needs of our customers and provide solutions to those needs.
H lant's RISK MANAGEMENT staff will assist your public entity in identifying existing or
potential property and liability exposures. Technical assistance and resource materials
are available to our customers at no cost.
H lant's MARKETING department partners with local Indiana independent agents to
seek out and gather information from select governmental entities. This results in a
tailored insurance package that focuses on the specific need of each entity.

INSURANCE COVERAGE
You are hereby notified that under the Terrorism Risk Insurance Act, as amended, that you have a right to purchase
insurance coverage for losses resulting from acts of terrorism, as defined in Section 902(1) of the Act The term "act of
terrorism" means any act that is certified by the Secretary of the Treasury —in concurrence with the Secretary of State,
and the Attorney General of the United States —to be an act of terrorism; to be a violent act or an act that is dangerous to
human life, property, or infrastructure; to have resulted in damage within the United States, or outside the United States in
the case of certain air carriers or vessels or the premises of a United States mission; and to have been committed by an
individual or individuals as part of an effort to coerce the civilian population of the United States or to influence the policy
or affect the conduct of the United States Government by coercion.
YOU SHOULD KNOW THAT WHERE COVERAGE IS PROVIDED BY THIS POLICY FOR LOSSES RESULTING FROM
CERTIFIED ACTS OF TERRORISM, SUCH LOSSES MAY BE PARTIALLY REIMBURSED BY THE UNITED STATES
GOVERNMENT UNDER A FORMULA ESTABLISHED BY FEDERAL LAW, HOWEVER, YOUR POLICY MAY CONTAIN
OTHER EXCLUSIONS WHICH MIGHT AFFECT YOUR COVERAGE, SUCH AS AN EXCLUSION FOR NUCLEAR
EVENTS. UNDER THE FORMULA, THE UNITED STATES GOVERNMENT GENERALLY REIMBURSES 85% OF
COVERED TERRORISM LOSSES EXCEEDING THE STATUTORILY ESTABLISHED DEDUCTIBLE PAID BY.THE
INSURANCE COMPANY PROVIDING THE COVERAGE. THE PREMIUM CHARGED FOR THIS COVERAGE IS
PROVIDED BELOW AND DOES NOT INCLUDE ANY CHARGES FOR THE PORTION OF LOSS THAT MAY BE
COVERED BY THE FEDERAL GOVERNMENT UNDER THE ACT.
YOU SHOULD ALSO KNOW THAT THE TERRORISM RISK INSURANCE ACT, AS AMENDED, CONTAINS A $100
BILLION CAP THAT LIMITS U.S. GOVERNMENT REIMBURSEMENT AS WELL AS INSURERS' LIABILITY FOR
LOSSES RESULTING FROM CERTIFIED ACTS OF TERRORISM WHEN THE AMOUNT OF SUCH LOSSES IN ANY
ONE CALENDAR YEAR EXCEEDS $100 BILLION. IF THE AGGREGATE INSURED LOSSES FOR ALL INSURERS
EXCEED $100 BILLION, YOUR COVERAGE MAY BE REDUCED.

FLEET AUTOMATIC
The premium quoted is the final annual premium. Subject to the conditions described in item #2
below, this coverage will not be audited.
#1 — Commercial Automobile Liability
With symbol 1, coverage is automatically provided for ANY AUTO the entity owns or acquires during the
Policy Period.
#2 — Commercial Automobile Physical Damage
With symbol 10, coverage is provided on a "Fleet Automatic" basis. This means that any auto
the entity acquires during the term of the policy will be covered automatically, provided it is an
additional new auto of the same like and usage currently reflected on the schedule, or replaces an
auto the entity previously owned which had Physical Damage coverage. The entity must submit
a request for coverage for any unit which does not meet the conditions outlined above.
IMPORTANT NOTE: The insured should continue to submit all change requests to their agent
and the agent should submit copies of all requests to Hylant Administrative Services to insure
accurate record keeping and claims verification.

REJECTION OF UNINSURED MOTORIST COVERAGE AND UNDERINSURED
MOTORIST COVERAGE OR SELECTION OF LOWER LIMIT OF LIABILITY
(Indiana)
The Indiana Insurance Code (Section 27-7-5-2) requires an insurer to provide bodily injury Uninsured
Motorist Coverage and Underinsured Motorist Coverage equal to the limit for Bodily Injury Liability
Coverage in the policy and Property Damage Uninsured Motorist Coverage at the basic financial
responsibility limit. Property Damage Uninsured Motorist Coverage is only available if Bodily Injury
Uninsured Motorist Coverage is afforded under the policy.
A named insured can reject either or both Uninsured Motorist Coverage or Underinsured Motorist
Coverage, reject the Property Damage portion of the Uninsured Motorist Coverage, or select a limit of
liability lower than the limit of Bodily Injury Coverage. (Note: Limit selected for Underinsured Motorist
Coverage cannot be less than $50,000/$50,000 Split Limit or $50,000 Single Limit).
Uninsured Motorist Coverage and Underinsured Motorist Coverage provide insurance for the protection of
persons insured under the policy who are legally entitled to recover damages from owners or operators of
uninsured or underinsured motor vehicles because of bodily injury, sickness, or disease, including death,
and for the protection of persons insured under the policy who are legally entitled to recover damages
from identifiable owners or operators of uninsured motor vehicles for injury to or destruction of property
resulting therefrom.

to the current outstanding recommendation within the next 30 days.

Commercial Automobile

Note: This quote is a package offer. The premiums stated in the proposal are applicable only when
an order is received for all lines quoted. Please contact your Sales Manager or Underwriter if you
would like to receive an alternative quote.
This proposal is based on information provided to HylanttTrident by the Agent. An application
signed and dated by an official of the entity and the agent/broker must be received within thirty
(30) days of binding coverage.
The quotation in this proposal does not necessarily match the coverage or limits requested in the
bid specifications and/or application. No warranty is made or implied with respect to the total
compliance to bid specifications or applications.
Each individual policy contains the actual terms, conditions and exclusions. This proposal
highlights certain features and benefits of the Trident Program. Specifically excluded liability
exposures include, but are not limited to: mechanically operated amusement devices, lead
contamination and pollution liability.
Any professional liability lines quoted, (Public Officials Liability, Employment Practices Liability
and Law Enforcement Liability) include defense costs within the deductible.
