SUPPLEMENTAL AGREEMENT TO CONTRACT #56-2004
ENGINEER — BURGESS AND NIPLE, INC.
The Phase I Environmental Site Assessment for the Salisbury Road Widening
Project recommends that a Phase 2 Environmental Site Assessment be conducted.
Soil borings will be taken in the areas recommended in the Phase I Environmental
Assessment and analyzed for contaminants. The area of land being tested is located in
front of 2000 Salisbury Road. This site has been used to store junk cars for years.
I recommended approving the attached fee schedule as presented by the Burgess and
Niple. Total fees for the Phase 2 Environmental Site Assessment shall not exceed
$8,975.00.

Re: Request for Supplemental Agreement for
Salisbury Road Design Project
The purpose of this letter is to advise you of the proposed costs to perform a Phase It
Environmental Site Assessment, and to request a supplement to our contract fees as a result
of this requirement.
We had previously advised you of this change in our Grade Review meeting that was held
on November 8, 2004. The Phase II Environmental Site Assessment was recommended
during the Phase I ESA, and thus will be required to complete the Environmental
Document.
Our supplemental fee request for these additional services to be performed by CTL
Engineering is itemized as follows:
Field Testing (Borings will be taken and sampled in the field) $3,295
Laboratroy Testing (Eight soil samples will be analyzed for contaminants) $3,780
Reporting (Written report documenting activities and interpreting data) $1,100
Miscellaneous (Includes project management and other fees) $ 800
Total for Phase II Environmental Site Assessment $8,975

This transmittal will serve as a letter form agreement to supplement the original agreement
and is subject to all of the conditions of the original agreement with the expanded scope of
work and modified fees outlined in this letter. Included are two copies of this letter form
agreement to amend our contract for the additional services. If you concur with these
changes please sign and return one copy of the agreement to us for our files. We will
consider the signed document as our notice to proceed with the additional work. Please
keep the other copy for your files.
