review standards with the following conditions:

1) Electric Service line shall be buried if they are not already.
2) The dock, boat and lifts will conform to the section 66 -369 in entirety.
3) Prior to the issuance of a Certificate of Occupancy the owner of the property shall
prepare and record a deed restriction which shall provide that all unenclosed roof
projection areas as approved shall remain so for as long as the structure is in existence
or exceeds the maximum permissible FAR.
4) Prior to the issuance of a Certificate of Occupancy the owner of the property shall
prepare and record a Gulf Stream Driveway Removal Agreement.
S) Any minor modifications in the landscape plan shall be submitted to the Town Manager for review
and approval and any major modifications shall be brought back to the ARPB for review and approval,
prior to commencement of landscaping.

NOTICE OF APPLICATION FOR DEVELOPMENT APPROVAL
NOTICE IS HEREBY GIVEN that the Architectural Review and
Planning Board of the Town of Gulf Stream will hold a Public
Hearing on Thursday, June 25, 2009 at 8:30 A.M., and the Town
Commission will hold a Public Hearing on Friday, July 10, 2008 at
9:00 A.M., both in the Commission Chambers of the Town Hall, 100
Sea Road, Gulf Stream, Florida, at which the following will be
considered:
An application submitted by William Wietsma, agent for
Quarter Track LLC, owners of property located at 555 Palm
Way, Gulf Stream, Florida, legally described as Lot 20,
Polo Cove Subdivision, for the following:
1. LAND CLEARING PERMIT to clear building site for
for construction of new structures.
2. SPECIAL EXCEPTION to permit 300 sq. ft. of covered un-
enclosed roof projection that exceeds the maximum
permitted floor area ratio.
3. LEVEL 3 ARCHITECTURAL /SITE PLAN REVIEW to permit the
construction of a two -story single family Gulf Stream
Bermuda style dwelling with 2 car garage, consisting of
5,568.5 square feet and a swimming pool.
The Architectural Review and Planning Board shall make a
recommendation to the Town Commission, and the Town Commission
shall make a final decision regarding the subject application at
the meetings noticed above. These meetings may be adjourned from
time to time and place to place as may be necessary to hear all
parties and evidence.
The complete application materials are on file in the Office
of the Town Clerk located at 100 Sea Road, Gulf Stream, Florida
33483, and may be reviewed during regular business hours, which
generally include non - holiday weekdays from 9:00 A.M. to 4:00 P.M.
ALL PARTIES INTERESTED IN THESE MATTERS may appear before the
Architectural Review and Planning Board and the Town Commission of
the Town of Gulf Stream at the times and place aforesaid and be

Construction Traffic Management Plan
Pursuant to Town Commission Policy, proposed plans for the orderly management of
construction related traffic and parking shall be submitted concurrent with an application
for Architectural /Site Plan Review or building permit. The plan must address the following:

Construction- Damage Prevention Policy
The following policy is established to assist in protecting residential lawns from damage
caused by construction vehicles.
• As an exception to the Town's right -Of -Way Policy, Town residents who own
property in the vicinity of an active construction site will be permitted to install
white (or painted dark green) capped PVC pipes, 36" above grade along the front
of their property, 2' from the road's edge. These pipes should be approximately
9' apart. The maximum pipe size shall be 2" in diameter. The pipes shall be
removed immediately following the issuance of Certificate of Occupancy for the
construction site.
• As part of the building permit application process, the contractor shall agree to
and sign a "Construction Traffic Management Plan" that has been prepared by the

This is to confirm that at the Public Hearing held by the Town
Commission on July 10, 2009, your application to demolish the
structures on property you own located at 555 Palm Way in Gulf
Stream and construct a two -story Gulf Stream Bermuda style single
family dwellingwith attached 2 car garage consisting of 5,568.5
square feet, and a swimming pool, was considered.
Your application was approved as presented with the following five
(5) conditions: 1) Electric Service line shall be buried if it
isn't already buried; 2) The dock, boat and Iifts shall conform to
the section 66 -369 in it's entirety; 3) Prior to the issuance of
a Certificate of Occupancy, the owner of the property shall prepare
and record a deed restriction which shall provide that all unenclosed
roof projection areas as approved shall remain so for as long as the
structure is in existence or exceeds the maximum permissible FAR;
4) Prior to the issuance of a Certificate of Occupancy the owner of
the property shall prepare and record a Gulf Stream Driveway Removal
Agreement; 5) Any minor modifications in the landscape plan shall
be submitted to the Town Manager for review and approval and any
major modifications shall be brought back to the ARPB for review and
approval, prior to commencement of landscaping.
If a building permit including the aforementioned improvements is
not applied for by July 10, 2010, the approvals will expire.
