(Skyline) is pleased to provide the Borough of Carteret with the following occupational safety and health program training services proposal for 201. G The professional services provided to the Borough of Carteret will be conducted in accordance with the State of New Jersey's Public Employees Occupational Safety and Health Act (PEOSHA or PEOSH) and the United States Department of Labor, Occupational Safety and Health Administration (OSHA) standards, the State of New Jersey Department of Health (NJDOH), the State of New Jersey Department of Environmental Protection (NJDEP) and the United States Department of Homeland Security regulations (DHS). This proposal is divided into the following sections: 1.0 Scope of Services 2.0 Owner's Responsibilities 3.0 Project Schedule 4.0 Estimated Project Fees 1.0 SCOPE OF SERVICES Skyline proposes to provide the following occupational safety and health services to designated Borough of Carteret employees. All site - specific training sessions will be scheduled and conducted at mutually convenient time periods and should be limited to 25 employees. Task 1.0 Provide Lockout/Tagout Training and related Drills. Task 2.0 Provide Confined Space Entry Training and Surface Rescue Drills Task 3.0 Provide PPE Hazard Assessment Inspection. 740 Highway 34 - Suite E • Old Bridge 'township • Matawan, New Jersey 07747 Phone: (732) 583 -2500 • Fax: (732) 583 -5200 • www.skylineeimcom 2.0 OWNER'S RESPONSIBILITIES It is necessary that the Borough of Carteret provide the following assistance in a timely manner, so as not to delay in the performance of the aforementioned Scope of Services: • Designate an authorized representative to provide assistance at each Borough of Carteret facility. • Provide Skyline personnel regular access to the various Borough of Carteret facilities. • Furnish access to existing safety and health records, documentation and other pertinent information deemed necessary by Skyline to complete the Scope of Services. • Provide facilities and employee scheduling for each safety and health training sessions. • Conduct walk/drive- through site inspections with Skyline personnel, as needed. 3.0 PROJECT SCHEDULE Skyline will begin to complete the Scope of Services upon receipt of a Borough of Carteret contract purchase order or a letter of authorization to proceed. 4.0 ESTIMATED PROJECT FEES The following is the summary of the total estimated budget fees associated with providing Task 1.0 and 2.0 (per each occupational safety and health employee training session); and Task 3.0, as described in the Scope of Services above of this proposal.
