OKALOOSA GAS DISTRICT EASEMENT
KNOW ALL MEN BY THESE PRESENT that for and in consideration of the sum of $1.00 and other good and
valuable considerations, receipt of which is hereby acknowledged, the undersigned grantor, City of Crestview,
whose address is: 198 Wilson St, Crestview, Florida 32536, does hereby grant unto the OKALOOSA GAS DISTRICT,
P. O. Box 548, Valparaiso, Florida 32580, a political subdivision of the State of Florida, Valparaiso, Florida, herein
called Grantee, a Perpetual, Non -Exclusive Utility Easement and right-of-way for the purpose of constructing, re-
constructing, maintaining and repairing a gas main or mains, or other gas facilities, together with all necessary right
of access, ingress and egress thereto and therefrom, in, under, over and along the following described property
located in Okaloosa County, Florida, and hereto described as follows:
Lots 37, 38, and 39, Block 7, Town of Crestview, according to plat thereof on file in Plat Book 1, Page 72, in the
office of the Clerk of Circuit Court, Okaloosa County, Florida. Said land being located at 198 Wilson St,
Crestview, Florida 32536, and having a parcel identification number of 17-3N-23-2490-0007-0370.
TOGETHER with all the tenements, hereditaments and appurtenances thereto belonging to or in anywise
appertaining.
TO HAVE AND TO HOLD the said easement and right-of-way hereinabove granted to the said Grantee, its
successors and assigns forever, for the use and purpose aforesaid and for no other purpose.
AND the grantor hereby grants to said grantee the rights and permissions contained herein, that the
grantor has good right and lawful authority to convey the rights to said land.
IN WITNESS WHEREOF, the said grantor has signed and sealed these presents the day and year as
depicted.

➢ THE GRANTOR AND TWO (2) WITNESSES NAMES NEED TO BE TYPED OR PRINTED
BELOW SIGNATURES.
➢ YOU WILL NEED TWO WITNESSES TO EACH OF YOUR SIGNATURES (THEY CAN BE
THE SAME TWO PEOPLE FOR EACH OF YOU).
➢ ALL SIGNATURES MUST BE IN FRONT OF A NOTARY.
➢ NOTARY CAN BE A WITNESS.
➢ GRANTEE SIGNATURE WILL NOT REQUIRE A WITNESS OR NOTARY.
➢ A NOTARY CAN BE FOUND AT MOST BANKS.
➢ When the document is signed and notarized, mail the original document to:
